5.A Chronic Disease Self-management Program for the Elderly in Korea.

OBJECTIVES: The present study proposed to evaluate 1st-month and 3rd-month health status, depression, self-efficacy and medical expenses of a community-based health promotive self-management program for old Koreans. METHOD: Participants in the CDSMP were selected from elders in a community health center through convenient sampling. The program included a 3-hour session per week for 14 weeks. Outcomes of evaluation in the 1st month and the 3rd month included modified Self-rated Health Status Scale, Center for Epidemiologic Studies Short Depression Scale, Self-Efficacy for Managing Chronic Disease 6-item Scale, and 1-item Medical Expenses Scale. RESULTS: Self-rate health scores increased significantly just after the intervention (16.22), in the 1st month (17.57) and in the 3rd month (19.04) (chi2=32.06, p=.000); Depression scores (reversed) also increased significantly just after the intervention (6.04), in the 1st month (7.75) and in the 3rd month (8.39) (chi2=29.92, p=.000); Self-efficacy score increased significantly just after intervention (12.87) but it decreased in the 1st month (12.73) and in the 3rd month (12.04). But all of the three scores were still higher than those before the intervention (8.65) (chi2=32.42, p=.000): Medical expense score decreased at the end of the intervention (1.57) but the cost score increased in the 1st month (2.48) and in the 3rd month (2.39) (chi2=7.81, p=.050). CONCLUSION: CDSMP is effective in increasing self-rate health and self-efficacy and in decreasing depression in the Korean elderly. However, no significant effect of the program was observed in decreasing the medical cost of the Korean elderly.

6.A Case of Acquired Multiple Cavernous Hemangiomatosis.

Acquired multiple cavernous hemangiomatosis appear on the skin and internaI organs, including in particular the skeleton, the brain and the liver in childhood or in adult life in the 4th and 5th decades. It is extremely rare for cutaneous hemangiomas associated with internal lesions to arise de novo in adults. The lesions persist indefinitely and symptom-free survival to old age is possible, despite the presence of hemangiomatosis in liver, heart or kidney. A 53 year-old male patient had multiple, non-tender, bluish to violet colored. pea to walnut sized, cutaneous, some pedunculated nodules on the entire body for 8 years. Histopathologically, there were non-capsulated vascuiar tumor consisting of large variable sized blood vessels in the dermis and some organizing thrombi. There was no evidence of malinant change.

7.Study on the Effects of Epidermal Growth Factor on Epidermal and Hair Growth in the Mouse.

Epiclerrnal growth factor(EGF), a low-molecular-weight polypeptide chain consisting of 53 amino acids, stimulates the growth and keratinization of epidermal cells. Recently, some authors reported that hair growth rate and follicular bulb growth were inhibited by EGF in neonatal mice. Since the hair and follicular cells are of epidermal origin, the effect of EGF would appear to be inconsistent with its role promoting the proliferation and differentiation of epidermal cells in vivo and in vitro. We have investigated the effects of EGF on the hair and epidermis in adult male mouse. The results are summarized as follows: l. 1'he hair length and maximum diameter were significantly reduced in EGF- treated regions compared with control group, and inhibition of hair growth was correlated with EGF dose. Thickness of the epidermis was significantly increased in EGF-treated regions compared with control group, and proliferation of the epidermis was correlated with EGF dose. 3 There were no significant differences in the hair length, maximum diameter, and egidermal thickness between untreated contralateral right mid-side regions of EGF-treated groups and control group.

8.Antiviral Effect of Antisense Phosphorothioate Oligonucleotides Targeted to Herpes Simplex Virus.

To search the effective antisense oligonucleotide that inhibit the growth of Herpes simplex virus type 1 (HSV-1), six kinds of phosphorothioate oligodeoxynucleotides (S-ODNs) were synthesized and the antiviral activity was assessed by measuring cytopathic effect in Vero cells infected with HSV-1. Of the three dodecamer S-ODNs, cornplernentary to the translation initiation site of IE2 (AS2) and scrambled S-ODN (AS1) showed more significant antiherpetic activity than AS4 complementary to the IE4. Accordingly, the antiviral effect of dodecamer S-ODN was not specific. In contrast to the no inhibitory effect of sense strand S-ODN of ICP8 (AS6), two S-ODNs complementary to the translation initiation site of ICP8 (AS3) and that of IE1 (AS5) showed potent antiherpetic activity assessed in vitro HSV-1 virus yield assay. Antiherpetic effect of AS3 was decreased in proportion to the addition of AS6. The synthesis of viral protein ICPS and IE1 were inhibited in AS3 and AS5 treated HSV-1 infected Vero cells, respectively. These findings suggest that antiherpetic effect of AS3 is specifically mediated by targeting ICPS. S-ODNs had no effect on Vero cell viability. The data suggest that the 19-mer S-ODNs may be effective in antiviral chemotherapy.

10.A Case of Malignant Pyoderma.

Malignant pyoperma is a rare, chronic, progressive, destructive ulcerating skin disease of unknown cause that affects the head and neck region of young adults. The disease is progressive but responds to high doses of systemically administered steroids. We present a case of malignant pyoderma developed on the left retroauricular area in 67 year-old male. The patient died of rapidly progressive, desructive, 15cm x 20 cm sized, phagedenic ulcer on the left retroauricular and neck area despite of various extensive local and systemic treatment. Histopathologic findings of tissue from the edge of the ulcer were non-specific, showing upper dermal necrosis and mixed inflammatory cell infiltrate in the deep dermis.
