2.Characteristics Involved in Internet Addiction Tendency of Adult Males with Attention Deficit/Hyperactivity Disorder.

OBJECTIVES: This study was conducted in order to investigate characteristics of temperament, depression, anxiety, attention, and impulsivity in adult males with Attention Deficit/Hyperactivity Disorder (ADHD) and internet addiction tendency. METHODS: The survey participants were 181 (121 patients and 60 healthy control subjects) adult males older than 19 years of age in Chung-Ang University Hospital and Gongju National Hospital. Subjects were divided according to ADHD with internet addiction tendency (AI), pure adults with ADHD (AD), and healthy control subjects (HC). All groups completed the Adult ADHD Scale (AADHD), Wender-Utah ADHD Rating Scale (WUADHD), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), the Korean Version of Young Internet Addiction Scale (YIAS-K), Barratt Impulsiveness Scale (BIS) and Temperament Character Inventory-Revised Short version for identification of relationship between ADHD and internet addiction tendency. RESULTS: AI groups were found to have higher AADHD, WUADHD, BDI, YIAS-K, and Novelty Seeking scores, compared to the AD and HC groups. The Cooperativeness score of the AI group was significantly lower than that of the AD group and HC group. The BAI and BIS scores of the AI group and AD group were significantly higher than those of the HC group. The Self-Directedness scores of the AI group and AD group were decreased, compared to the HC group. YIAS-K scores were partially related to BDI scores in the AI group. CONCLUSION: The results of this study indicate an association of higher score of BDI with internet addiction tendency in adult patients with ADHD. Management of temperament characteristics, depression, anxiety, attention, and impulsivity may be important for adults with ADHD and internet addiction tendency.

3.Fertility Outcomes after Myomectomy in Infertile Patients with Myoma Uteri.

OBJECTIVE: To investigate the fertility outcomes after abdominal or laparoscopic myomectomy in infertile patients with no other infertility factors except uterine myoma. METHODS: From January, 1992 to December, 1998, abdominal or laparoscopic myomectomy was performed in 76 patients with desire for children and no other recognizable infertility factors. The medical records were reviewed retrospectively. The mean duration of postoperative follow-up was 23.4+/-21.2 months(1~82 months). RESULTS: After myomectomy, 35 patients(46.1%) became pregnant during the follow-up period, especially with 24(68.6%) within 12 months. Excluding 7 cases of spontaneous abortion and 2 cases of intrauterine fetal death, 26 patients(74.3%) had a viable birth. Parity, duration of infertility, number, type and location of myoma, and the proportion of opening the intrauterine cavity during operation were not significantly different between the pregnant and nonpregnant groups. However, the size of uterine myoma was significantly larger in the pregnant group(p<0.05). CONCLUSION: We obtained a reasonable pregnancy rate in infertile patients with uterine myoma as a sole infertility factor. The size of myoma may influence the postmyomectomy pregnancy rate, and this may indicate a more beneficial effect of myomectomy in infertile patients with a relatively larger myoma.

4.Clinical Outcomes of Conservative Treatment and Arthroscopic Repair of Rotator Cuff Tears: A Retrospective Observational Study.

OBJECTIVE: To compare the clinical outcomes following conservative treatment and arthroscopic repair in patients with a rotator cuff tear. METHODS: In this retrospective study, patients aged >50 years with a symptomatic rotator cuff tear were reviewed. The rotator cuff tendons were evaluated using ultrasonography, shoulder magnetic resonance imaging or MR arthrography, and the patients with either a high-grade partial-thickness or small-to-medium-sized (≤3 cm) full-thickness tear were included in this study. The primary outcome measures were a pain assessment score and range of motion (ROM) at 1-year follow-up. The secondary outcomes were the rate of tear progression or retear along with the rate of symptom aggravation after the treatments. RESULTS: A total of 357 patients were enrolled, including 183 patients that received conservative treatment and 174 patients who received an arthroscopic repair. The pain assessment score (p<0.001) and the ROM in forward flexion (p<0.001) were significantly improved in both groups. The ROM in internal rotation did not significantly change after conservative treatment and arthroscopic repair. The pain assessment score and ROM were not significantly different between the two groups. Retear was observed in 9.6% of patients who had an arthroscopic repair and tear progression was found in 6.7% of those who underwent conservative treatment. The proportion of aggravation for pain and ROM did not significantly differ between the two groups. CONCLUSION: The effectiveness of conservative treatment is not inferior to arthroscopic repair for patients >50 years old with a less than medium-sized rotator cuff tear in a 1-year follow-up period. Further study is warranted to find the optimal combination of conservative treatment for a symptomatic rotator cuff tear.

Behcet's disease is a complex multisystem disease that features recurrent aphthous stomatitis, recurrent genital ulcerations, and eye lesions(uveitis or chorioretinitis). Among the systemic manifestat ions, pulmonary involvement is known to be rare and only a few cases have been documented. The most important features of pulmonary lesions in Behcet's disease are recurrent hemoptysis, which is often massive, and fatal pleuritic chest pain and recurrent high fever and fleeting nature of the pulmonary infiltrates. We report a case of Behcet's disease manifestated as high fever and pleural effusions which was complicated by pulmonary infarction ans a result of pulmonary arteritis.

6.One-year follow-up outcomes of tension-free vaginal tape (TVT) procedure for treatment of stress urinary incontinence.

OBJECTIVE: To evaluate one-year outcomes of tension-free vaginal tape (TVT) procedure in women with stress urinary incontinence (SUI). METHODS: From 2002 to 2005, all patients with stress urinary incontinence undergoing the TVT procedure were enrolled in this retrospective study. All cases were diagnosed as SUI by the evidences in history, physical examination, and urodynamic study. Outcomes and post-operative complications were investigated from review of medical records and interview during post-operative follow-up. RESULTS: Totally 164 cases were enrolled in this study. The success rates (cure or improved) of two-month and one-year after the TVT procedure were 94.5% and 87.9%, and the failure rates were 5.5% and 3.0%, respectively. The follow-up loss rate in one-year was 9.1%. There was no serious complication related to the procedure in one-year follow-up. CONCLUSION: This preliminary report indicates that the TVT procedure is a safe treatment modality for female SUI. Further investigation with long term follow-up is required to estimate cumulative success rate and late complications.

9.Plantar Dermatofibroma Misdiagnosed as Viral Warts.

Dermatofibroma is a common benign fibrohistiocytic tumor. It can occur anywhere on the body surface, but it has a propensity for the extremities. However, dermatofibroma may show a wide variety of clinicopathologic variants and so it is often misdiagnosed. Our case was a 13-year-old male who presented with a 1-year history of a solitary skin-colored hyperkeratotic nodule on the right sole. The nodule had recurred after topical therapy for viral wart. Histopathologic examination of the patient's lesion revealed a poorly circumscribed nodular tumor composed of collagen bundles, fibroblasts and histocytes, which was all consistent with plantar dermatofibroma.

10.A Statistical Analysis and Clinical Evaluation of Dermatologic Consultations in Obstetrics and Gynecologic Inpatients.

BACKGROUND: Dermatologic comorbidities can be the clue for the diagnosis and treatment of systemic conditions and a need for dermatological expert opinion is increased. OBJECTIVE: To analyze the patterns and to quantify the dermatologic consultations referred by the department of obstetrics and gynecology (OBGY). METHODS: We retrospectively studied the data from 479 cases referred by OBGY for a dermatologic consultation during a three-year period in a tertiary hospital. RESULTS: The most common skin disorder was pregnancy-related dermatoses (29.77%) in obstetric inpatients, while cutaneous infection (33.33%) was the most frequent in gynecologic inpatients. The distribution of the skin disorders in consulted OBGY inpatients was consistent with the previous reports. However, there were some differences, such as high proportion of infectious disease. Infectious skin diseases were significantly more frequent (p<0.001) in the chemotherapy group of gynecologic inpatients compared to that of the non-chemotherapy group, viral infection being the most common. In addition, superficial fungal infection accounted for the same proportion as the viral infection (37.50% each) among obstetrical inpatients. Maculopapular or urticarial eruption (51.52%) was the most common type of chemotherapy-related adverse drug reaction. CONCLUSION: This study revealed the distribution of skin disorders in patients referred by OBGY for dermatologic consultation. Furthermore, it could be helpful as an educational material to assist cooperation between dermatology and OBGY.
