1.PRESENCE OF VIMENTIN IMMUNOREACTIVE FILAMENTS IN THE CELLS OF MANDIBULAR FOSSA FBROUS LAYER OF RAT WITH AGING.

The purpose of this study was to investigate what the intermediate filaments in the cells of rat mandibular fossa fibrous layer are and any relationships between the presence of these filaments and aging. Mandibular fossae of 4 groups of rats(14-day, 28-day, 55-day and adult groups) were removed on bloc and processed for immunostaining and were subjected to light microscopic examination. The results were as follows : In 14-day group, there were no immunoreactive cells in fibrous layer of mandibular condyle articular surface. But in 28-day group, many immunoreactive cells were seen in fibrous layer, especially central portion of articular surface of mandibular fossa. These cells were fusiform shaped and immunoreactivities were seen in the cytoplasm around the nucleus. In 55-day group many immunoreactive cells were seen in fibrous layer of mandibular fossa. These cells were fusiform shaped and distributed evenly in central portion of this fibrous layer. Immunoreactivities were seen in the cytoplasm around the nucleus. In adult group, the results were similar to 55-day group, Many immunoreactive cells were seen in fibrous layer of mandibular fossa especially central portion. According to these results, vimentin immunoreactive filaments appear with aging and increment of mechanical load associated with incision or mastication.

3.Clinical Study of the Tibial shaft Fracture

A clinical study of the Tibial Shaft fracture was made on 128 patients, who had been treated at the Department of Orthopedic Surgery, College of Medicine Kyung Hee Universlty from the October, 1971 to May, 1979. The results were as follows: 1. The rate of fracture union was accelerated under the age of 20 years but it was slow in aged group. 2. The poor prognosis of fracture union was shown in junction between middle and distal 1/3 than others. 3. The better prognosis of the fracture type was shown in oblique and spiral fracture than in the comminuted and segmental. 4. There was prolonged rate of union in case of associated fibula fracture. 5. The frequency of delayed and nonunion were more prevalent in open fracture. 6. Among the open comminuted tibial shaft fracture with skin and soft tissue loss, the good results was obtained by the Hoffmanns external skeletal fixation method.

4.Popliteal Artery Injury Associated with Severe Knee Trauma

Fracture dislocation or dislocation of the knee can produce a popliteal artery injury that may be difficult to evaluate clinically. Diagnosis of disruption or thrombosis of the popliteal artery is frequently delayed until the opportunity to salvage the extremity is lost. 22 cases of popliteal artery injury were reviewed from July 1978 to December 1983, associated with severe knee trauma at the Orthopaedic department of Kyung Hee University Hospital, of whom average follow-up for about 52 months. The results obtained were as follows; 1. In all 22 cases, 14 cases (63. 6%) were amputated. The amputation rate was correlated with ischemic time after injury. 2. Resection of all injuried vessels with reconstitution of continuity by the use of an interposed saphenous vein graft is often warrented to avoid tension. 3. All 8 patients, which was managed conservatively, were amputated in all cases(100%). In cases of suspicious popliteal artery injury, early aggressive exploration and obvious microvascular reconstruction should be mandatory. 4. Subperiosteal fibulectomy-fasciotomy should be done routinely immediately after vascular injury. 5. Diagnosis of popliteal artery injury was based on the clinical findings but the capillary filling was not contributary. 6. The use of Doppler flowmeter and emergency arteriography was recognizedtobeanexcellent methods in determination of arterial injury.

5.Clinical Evaluation of Outcome of Hypertensive Cerebellar Hemorrhage.

The authors analyzed 28 cases with hypertensive cerebellar hemorrhage, diagnosed by computerized tomography(CT), between 1987 and 1990, in Chonbuk National University Hospital. The authors assessed the relationship of outcome to the CT appearance of the quadrigeminal cistern, which in some cases was obliterated by rostral displacement of the vermis resulting from the cerebellar mass. Obliteration of the quadrigeminal cisterns was classified on the CT scans into three grades:Normal(Grade I), Compressed(Grade II), or Absent(Grade III). There were 7 cases with Grade I, 9 with Grade II, and 12 with Grade II cisterns. Of the 28 cases, 6(85%) of those with Grade I, 7(78%) of those with Grade II, and none of those with Grade III cisterns respectively returned to their previous activities at 6 months or more after onset. A Grade I cisterns predicted a good outcome whether the hematoma was evacuated or not, as long as obstructive hydrocephalus, if present, was relieved early. However, a Grade II cistern was not predictive of a good outcome unless the hematoma was evacuated within 48 hours after onset of the hemorrhage. A Grade III cisterns predicted an unfavorable outcome. Taken together, these results strongly suggest that the CT grade of quadrigeminal cistern obliteration may be an indicator of outcome and may be useful in selecting treatment for patients with cerebellar hemorrhage.

7.Extensor Toe Signs Elicited by Various Methods in Cerebral Palsy Children.

PURPOSE: Extensor toe signs are neurologic abnormal finding in upper motor neuron impairment, such as spastic diplegia in cerebral palsy. Though classic Babinski sign is widely used, many other methods elicit extensor toe signs are used as substitutes. However, these methods are not tried whether they are clinically valuable or not. So, we carried out these methods to spastic cerebral palsy patients, compared the sensitivities of these extensor toe sign tests and estimated their clinical value. METHODS: From April, 2000 to August, 2001, thirty patients who had been diagnosed spastic cerebral palsy at Chungnam National University Hospital were tested for extensor toe signs at restful supine position. The sensitivity of every extensor toe sign was compared with each other. RESULTS: The sensitivity of Babinski sign is 76%, Gonda-Allen sign 86%, Allen- Cleckley sign 63%, Chaddock sign 63%, Oppenheim sign 50%, Gordon sign 22% and the others less than twenty percent. Although the sensitivity of Gonda-Allen sign is higher than Babinski sign, it is not statistically significant. But these two methods are superior than the others. CONCLUSION: In patients having upper motor neuron impairments, Gonda-Allen sign and classical Babinski sign are more useful and more sensitive than the other tests, which elicit of extensor toe signs.

8.The Results of Survey for Hansen's Disease of the Migrant Worker.

We already achieved the goal of the elimination of leprosy that was recommended by WHO. But the amazing change of our country's situation of the economical & social conditions makes the need of the migrant worker. Especially, the portion of migrant workers form India, Nepal, Indonesia, Philippines, Bangladesh, & Vietnam, the countries of relatively high prevalence rate of Hansen's disease are increasing among them recently. It means the attention about the influx of Hansen's disease form the foreign countries. We perform the survey for Hansen's disease of the migrant worker. Total 603 workers were surveyed. We found only 4 persons, that had the high titer(above 500) of the result of PGL-1 ELISA, and only 3 persons, that had some findings, the suggestive finding of Hansen's disease, of cutaneous & neurological symptoms. In spite of the special study, we did not conform the diagnosis of Hansen's disease among them. During this survey, we did not find the conformed patients of Hansen's disease. But we think that we will be concerned about the influx of Hansen's disease from foreign countries, because of the recently significance change of the epidemiological situations of Hansen's disease in Korea.

9.Evaluation of Cervical Body Configuration from C3 to C7 in Infants and Children.

PURPOSE: To obtain the findings of normal variant types of lower cervical body configuration for the purpose of differention from compression fracture. MATERIALS AND METHODS: We retrospectively analysed simple true lateral radiographs of cervical spine from C3 to C7 in 157 pediatric patients who did not have definitive clinical symptoms of cervical spinal injury. We classified the variations of normal cervical spine into 5 types by their configuration. In case of rounded upper corner or anteriorly wedged type, we measured the height and the width of vertebral body to classify these types, and undertook ANOVA test and multiple range test to determine the correlation between the gross configuration and the measured values. RESULTS: Type 1 was similarly observed at each of C3 to C7 in age of 1-4 group but the frequency was markedly decreased in age of 5-8 group. Type 2a was frequently observed at both age groups, and its incidence increased considerally at C3 and C4 with advancing age. Type 2b was more frequently observed at C3 body and Type 3 was observed only at C3 body in age group 1-4. Type 4 was markedly increased at 5-8 age group. Type 2b and 3 were seen mostly at C3, but sometimes at C4 body. Height of body was statistically more significant than width of body in classification of type 2a, 2b and 3. CONCLUSION: The configuration of lower cervical spine in infants and children changes from immature oval type(type 1) to mature rectagular type(type 4) with increasing age. Among the 4 types, the rounded upper corner type or anterior wedging type of lower cervical spine should be differentiated from compresson fracture. We concluded that the height of cervical body is more significant than the width of body in simple radiographic classification of types 2a, 2b and 3.

10.Characteristics of Pulmonary Nodules in Current Smoker

Lung cancer is the leading cause of death from cancer and the most effective way to reduce lung cancer mortality is early detection and treatment. The low-dose computed tomography (CT) can reduce the chances of lung cancer death, and used as screening test in several countries. Pulmonary nodules are a common finding in smokers, and differential diagnosis from cancer is a particularly important in lung cancer screening in smokers. We tried to find out the characteristics of lung nodules in patients who underwent smoker cancer screening. Data on low-dose lung CT findings and smoking counseling were collected for lung cancer screening patients for smokers conducted from January 2012 to June 2022. Out of a total of 1,320 patients, 1,101 had lung nodules. Lung nodules located in the right upper lobe were the most common at 30.5%, and lung nodules located in the right middle lobe were the least at 11.5%. Solid nodules were 97.3%, and ground-lass shadowing lesion was 2.4%. The average nodule size was 3.33 mm, and the largest was 43 mm. Compared to the previous test, 8.8% of the nodules were changed, and 1.8% of the total were suspected of cancer. The number of cessation patients interviewed was 138 (10.5%), and 98 were referred to smoking cessation clinics. This study shows that the lung nodule reporting rate is higher than overseas lung nodule reporting rate.
