An appreciation of the radiation changes in the brain adjacent to radiated glioma is of considerable importance from both clinical and pathological standpoint because the alteration in the brain parenchyma may result in a picture that mimics recurrence of the neoplasm. A 35 years old woman was admitted because of deterioration of consciousness which has started about 20 days ago. Past history revealed removal of left frontal brain tumor 4 years earlier and the diagnosis was grade II astroma. Postoperative radiation was done at that time. Sine then, she has been doing well. Computerized tomogram showed an ill defined huge low density mass at right frontal and left posterior parietal areas, which was irregularly enhanced in postcontrast study. Unlikely to the usual brain tumors, there was no mass effect on adjacent structures. Histologically acellular necrotic zone was alternating with hypercellular zone and most striking changes were vascular and glial reaction. Most of the vessels showed acellular hyliane thickening of their wall and some vessels were almost completely occluded. There was marked glial proliferation with considerable cellular and nuclear irregularities. Gemistocytic astrocytes were frequently seen and some were multinucleated.

As increasing concern for the fetal aspects of pregnancy, the availability of normal standard of placenta becomes more and more important. because it is very difficult to test the functional capacity of placenta. And for this reason anatnmic studies to determine the normal relationship between the placenta and newborn infant have come to be a considerable importance. A study of morphometric features of 4,000 consecutive placentas delivered at the Seoul National University Hospital during a period of 3 years, from April 1979 to May 1982, in relation to gastational age and weight of newborn was carried out. The findings could be summarized as follows: 1) Among 4,000 consecutive deliveries there were 3,771 liveborns 76 deadborns, and 153 cases of abortion, respectively. 2) The average weight and dimensions of term placenta were 511 gm and 19.94 x17.09 x1.89 cm, respectively. The weight , size and volume of placenta were progressively increased from 20 weeks of gestation till 41 weeks. 3) Morphometric features of placenta such as placenta weight, dimension, fetoplacental ratio and placental co-efficient showed a more significant correlation with birht weight than with gestation age. 4) The average length of umbilical cord was 39.80 cm, with the range of 6 cm to 124 cm. 5) The congenital anomalies of newborn was encountered in 74 cases (1.85%) among 4,000 consecutive deliveries. Anomalies involving musculoskeletal system was the most common oen. 6) The abnormal gross findings among 4,000 consecutive placentas included 6 cases of single umbilical arteries, 4 cases of chorangioma, 545 cases of infarction and 748 cases of calcification.

Objective: Helicopter emergency medical services (HEMS) have been used for prompt and appropriate transport of trauma patients. South Korea operates a dual HEMS system consisting of the Doctor-HEMS and the EMS-HEMS. This study compared the relevance of evaluation and treatment during transport using the two HEMS systems. Methods: This was a retrospective, single-center cohort study, and 180 trauma patients transported via HEMS from July 2013 to June 2021 were enrolled in the analysis. We compared the appropriateness of evaluation and management between the Doctor-HEMS and the EMS-HEMS using a newly developed checklist based on Advanced Trauma Life Support protocol. Results: In terms of the performance score, the EMS-HEMS was higher with respect to basic management at the scene, especially cervical immobilization and warming (P<0.001). On the other hand, the Doctor-HEMS showed a higher score with respect to monitoring and procedures during transport (P<0.001). The total performance score was higher in the Doctor-HEMS group than in the EMS-HEMS group (P=0.022). Conclusion The Doctor-HEMS generally showed better performance in the transport of trauma patients, although it showed relatively lower performance with respect to basic care at the scene. The EMS-HEMS displayed a lower performance in monitoring and management during air transport. It is necessary to develop a common checklist for both the Doctor-HEMS and the EMS-HEMS. Through continuous efforts and feedback, the transportation of trauma patients can be improved.

To determine the ventricular looping pattern in relation to cardiac laterality, we studied rat embryos treated with retinoic acid (RA). A total of 243 Wistar rat embryos from an in vivo treated group (a single dose of 20-40 mg/kg all-trans RA administered to pregnant rats on day 6.5 to 9.5) and 29 control embryos were examined on day 13 of gestation. Twenty-nine embryos from the in-vitro treated group (treated by all-trans RA at 2 x 10(-7) M for 6 hr on day 9.0 or 9.5 during the entire embryo culture for 72 hr) and seven control embryos were examined on day 12 of gestation. Abnormalities in cardiac laterality and ventricular looping were found in the in-vivo groups treated on day 8.5 and 8.75 and in the in-vitro group on day 9.0. Among 25 animals with abnormal laterality, right isomerism was the most common feature (22 cases), while the type of ventricular looping varied. Cases with normal laterality had a low incidence of abnormal looping (1.4%). In rat embryos treated with all-trans RA, normal cardiac looping was expected when cardiac laterality was normal. But in cases with abnormal laterality, the type of abnormal ventricular looping was unexpected.

Conjoined twins show varying degree of conjoining in either facing or side-by-side fashion. Cephalothoracopagus janiceps is a prototype of facing anomaly in which the two bodies demonstrated a cross symmetry to the midline, that is axial symmetry. Interfacial and intersternal lines crossed at a right angle and no abnormality of situs was associated. Dicephalus dipus dibrachius is a case of side-by-side union, in which the bodies facing nearly the same direction were symmetrical to the middle sagittal plane. Abnormal situs of one was always associated. Other types of conjoined twins as thoracopagus lie between the two extremes of facing and side-by-side union. The three dimensional architectures of the organs in each type would be explained using cross sectional figures of skull, thorax and pelvis. Although the facing twins share the internal organs without fusion, the organs in the side-by-side component are fused with modification of the situs. We postulate sixteen pairs of situs and four manners of division for the explanation of the midline organs and the presence of a dominant co-twin. The splenic locations in a given cardiopulmonary situs are evaluated for the appraisal and applicability of these hypotheses.

BACKGROUND AND OBJECTIVES: The incidence of retropharyngeal abscess has been decreased with use of antibiotics, but it still causes critical complications such as airway obstruction, aspiration pneumonia, mediastinitis, or sepsis. For this reason, early diagnosis and proper management of retropharyngeal abscess should be undertaken as soon as possibile. The treatment includes maintaining airway, performing surgical drainage, and administering antibiotics; but there are no definite guidelines for treating patients with retropharyngeal abscess, nor a golden rule for the correct surgical approach. SUBJECTS AND METHOD: A retrospective analysis of patients with retropharyngeal abscess who were treated at the Chungbuk University Hospital from 1993 to 2003 was performed. We analized general symptoms, signs, causing factors, and causing bacteria. We also analized the treatment outcome and selected approaches from the surgically treated patients. RESULTS: The general symptoms of retropharyngeal abscess were sore throat, fever, dyspnea, with the most common cause being the upper airway infection. Bacteriologically, hemolytic streptococcus was the most common bacteria among aerobes and bacteroides was the most common bacteria among anaerobes. However, in most cases, both aerobe and anaerobes were cultured at the same time. Among 18 cases, 6 cases were treated conservatively with antibiotics and surgical interventions were performed in 12 cases, which included 11 cases of transoral and 1 case of transcervical approach. CONCLUSION: Retropharyngeal abscess can be treated with either conservative or surgical treatment according to initial abscess size. The abscess size which is greater than 3 cm is prefered to be treated surgically, and transoral approach is a useful way of dealing with less invasive procedure.
