1.Genomic species identification of Acinetobacter calcoaceticus - Acinetobacter baumannii complex strains by amplified ribosomal DNA restriction analysis (ARDRA).

Members of the genus Acinetobacter are recognized as newer pathogens of the nosocomial infection with an increasing frequency in recent years. Strains that belonged to A. calcoaceticus-A. baumannii complex (genomic species 1, 2, 3, and 13TU) were major groups associated with nosocomial infection. Phenotypic identification was unreliable and laborious method to classify Acinetobacter strains into 19 genomic species. Rapid and reliable identification of clinical isolates is essential to diagnosis and epidemiology of Acinetobacter. We investigated the suitability of amplified ribosomal DNA restriction analysis (ARDRA) to identify genomic species of 131 Acinetobacter isolates. The 16S rRNA genes (ribosomal DNA) were enzymatically amplified and the amplified PCR products were restricted independently with the enzymes, AluI, CfoI, and MboI. Genomic species of Acinetobacter was classified by the combinations of restriction patterns. The analysis was showed that restriction profiles were characteristic for each genomic species. One hundred fourteen isolates were identified as A. baumannii, twelve were identified as genomic species 13TU, and one was identified as genomic species 3. Four isolates were found to be unknown organisms. All of the isolates which were identified to A. baumannii by phenotypic tests were completely discriminated into A. baumannii and genomic species 13TU by ARDRA. This study demonstrates that ARDRA is a rapid and simple techniques for the identification of Acinetobacter species according to the genomic species.

2.A Case of Postpoliomyelitis Muscular Atrophy.

A relationship between preceding acute paralytic poliomyelitis and the later development of motor neuron disease has only occasionally been suggested since it was first postulated by Charcot in 1875. The authors recently experienced a 20-year-old male who was considered to have postpoliomyelitis muscular atrophy. We report this case in view of its rarity and necessity of differential diagnosis from other neuromuscular disorders. Clinical presentation included slowly progressive muscle wasting of left thigh for 4 years, mild weakness of left arm and both thigh, intermittent fasciculation, and previous history of acute paralytic poliomyelitis. Electromyographic findings showed fibrillation potentials, positive sharp waves, fasciculations, giant motor unit potentials and reduced interference patterns. Muscle biopsy revealed scattered small angulated fibers, individual myofiber degeneration and mild inflammatory cell infiltration.

3.A Case of Severe Air Leak Syndrome Treated with High - Frequency Oscillatory Ventilation ( HFOV ).

In air leak syndrome, a significant portion of the volume delivered during a positive pressure breath can be lost through the leak. HFOV can achieve adequate ventilation at lower peak and/or mean intrapulmonary pressure than conventional mechanical ventilation (CMV) and has been an effective treatment of already established air leak syndrome. We report a 1-day-old male infant with severe respiratory failure from pneumothorax and pneumomediastinum, who was refractory to CMV with chest tube drainage. HFOV was applied to this patient for 114 hours, and improvement of oxygenation and ventilation as well as significant reduction of pneumothorax followed.

5.Comparing Ruminative and Distracting Responses and Emotion Regulation Difficulties in Early Community Adolescents With and Without Self-Harm

Objectives:
This study aimed to compare the demographic characteristics, responses to negative emotions, and difficulties in emotion regulation between self-harming adolescents and control individuals aged 12–14 years from the community.
Methods:
Data were collected from adolescents in Chungcheong Province, South Korea, between September 2021 and November 2022.Demographic characteristics and responses to the Depressed Mood Questionnaire and Difficulties in Emotion Regulation Scale-16 (DERS-16) were compared between the self-harm and control groups.
Results:
The self-harm group exhibited a higher prevalence of child abuse (odds ratio [OR]=4.787, 95% confidence interval [CI]=1.591– 14.409, p=0.005) and school bullying victimization (OR=4.495, 95% CI=2.353–8.588, p<0.001) than those in the control group. The selfharm group displayed higher levels of rumination (t=7.88, p<0.001) and reduced distraction responses (reverse score t=2.25, p=0.025) than those of the control group. Additionally, the self-harm group scored higher on all subscales and the total DERS-16 score (t=7.61, p<0.001).
Conclusion
Interventions for self-harming adolescents should address child abuse and bullying victimization. Prevention programs for self-harming adolescents should focus on reducing rumination responses, increasing distractive responses, and addressing difficulties in emotion regulation using dialectical behavior therapy-skill training.

Gynecomastia is a proliferation of the glandular component of the male breast. Gynecomastia is the most common variant condition of the male breast and results from the effect of an altered estrogen-androgen balance on breast tissue or from the increased sensitivity of breast tissue to a normal estrogen level. We experienced a case of macrogynecomastia in a 16-year-old boy who had a normal male phenotype and was treated by reduction mammoplasty with breast tissue removal, which was effective and produced good cosmetic and psychologic results.

8.Integron Carriage in Multiresistant Acinetobacter baumannii Strains.

One hundred and eight Acinetobacter baumannii strains isolated from three university hospitals in Korea were investigated for the presence of integrons and its correlation with multiple antibiotic resistance. A. baumannii strains were classified into 23 different groups based on the biochemical tests, RAPD patterns, and antibiotic susceptibility. Many strains isolated from same hospital showed identical epidemiological markers: 75% of isolates from Gwangju, 53% from Seoul, and 39% from Cheonan appeared identical. Integrase genes were detected in 79 (73%) A. baumannii isolates. The intI1 and intI2 genes were detected in 73 (68%) and 10 (9%) isolates, respectively. Class 3 integrons were not detected. Integrase genes were detected in A. baumannii isolates from Gwangju and Seoul, but not in the strains from Cheonan. Based on the epidemiological grouping, most of A. baumannii strains affecting four or more patients carried integrons, while only one strain among the sporadic strains carried a class 2 integron. Integron-carrying A. baumannii strains were resistant to more antibiotics compared to the integron-negative strains. This result suggests that integron-carrying A. baumannii strains are more apt to spread nosocomially and to show multiple antibiotic resistances.

10.Use of Viscoelastic Substance in Longstanding Recurrent Hyphema Patient.

Rebleeding in traumatic hyphema may cause several complications in the affected eye, and specific treatment of recurrent hyphema is still not estabilished and only conservative medical treatments are used except for surgical evacuation of the clot in case of persistent high intraocular pressure. We treated longstanding recurrent hyphema patient unresponsive to conventional medical treatment, with viscoeleastic substance which was injucted into anterior chamber. A large dose(0.2CC) of viscoelastic substance sufficient to directly compress the anterior structures(Iris, ciliary body) was injected and subsequently it increased the intraocular pressur gradually. We assume that these tamponade effect by the viscoelastic substance and later intraocular pressure rise was responsible for the therapeutic effect. Thus, viscoelastic injection into anterior chamber in longstanding recu.rrent hyphema patient may be a useful treatment.
