1.Successful Development of Cordyceps bassiana Stromata from Beauveria bassiana.

A specimen of Beauveria bassiana was collected from Yang-yang of Gangwon province, Korea in October 2006. Conidial isolates were prepared from the specimen by the dilution method and inoculated in brown rice medium for fruiting body production. After nearly two months incubation for perithecial stromata developed from single isolates as well as from their combinations. They were determined as Cordyceps bassiana by observing the stromatal characters and their conidial structures. This is the first report of the development of C. bassiana from B. bassiana cultures.

2.14 Cases of Nondysgerminomatous Ovarian Germ Cell Tumor.

A clinicopathologic study was per formed on 14 cases of malignant nondysgerminomatous ovari an germ cell tumors(NDOGCT) treated at the department of obstetrics and gynecology in Korea Cancer Center Hospital between Jan. l987 and,Jun. 1992. The results obtained were as follows ; 1. Median age of the patients was 27.5 years (range, 5 to 54) 2.Clinical symptoms presented were abdminal pain(64.3%), abdominal palpable mass(42.9%), abdominal distension(28.5%) in order of frequency. 3. Histopathologically, 7 cases(50%) were irnmature teratoma, 4 cases(28.6%) were endoder mal sinus tumor, 1 case(7.1%) was embryonal cell carcinoma, 2 cases(14.3%) were mixed germ cell tumor. 4. Bystage, 6 casse(42.9%) were in stage I, 1 case(7.1%) in stage Il, 2 cases(14.3%) in stage III, 3 cases(21.4%) in stage IV, and 2 cases(14.3%) in recurrence. 5, Serum alphafetoprotein was elevated in all patients withendodermal sinus tumor or embryonal carcinoma, and serum CA125 was elevated in about one half of the patients with nondysgerminomatous gerrn cell tumr. 6. l0 cases were managed by salpingo-oophorectomy with poatoperative chemotherapy, 3 cases by hysterectamy and salpingo-oophorectomy with postoperative chemotherapy. 7. Durations of follow up ranged 5 to 66 months, in that period, 5 cases were died, 4 cases were alive with disease and 4 casee wero no evidence of disease. 1 case was loat follow up after operation. All 5 patients were alive who had been treated with REP regirnen. In conelusion, prognosis of malignant NDOGCT is still poor in spite of adjuvant chemotherapy following surgery, and study ineluding appropriate chemotherapeutic regimen may be needed.

4.Plasma Neutrophil Gelatinase-associated Lipocalin is an Early Marker of Acute Kidney Injury.

PURPOSE: Serum creatinine is a late marker of acute kidney injury (AKI). We assessed the diagnostic value of plasma neutrophil gelatinase-associated lipocalin (NGAL) for predicting acute kidney injury in emergency department patients with sepsis. METHODS: This was a prospective observational study of adult sepsis patients. Plasma NGAL levels were measured upon admission to the hospital, and clinical data and serum creatinine were collected daily during the hospital stay. The primary outcome measure was the occurrence of AKI based on criteria from the Acute Kidney Injury Network (AKIN). RESULTS: A total of 178 patients were included, with 13 patients (7.3%) that developed AKI during their hospital stay; 9 and 4 were classified as AKIN stage 1 and 2, respectively. Six patients out of the 13 with AKI died. Mean plasma NGAL levels were 277 ng/mL in patients without AKI and 852 ng/mL in patients with AKI. The area under the receiver operating characteristic curve was 0.883 (95% confidence interval 0.803 to 0.964), the sensitivity was 91.7%, and the specificity was 80.5% for the prediction of AKI (using a cut-off value of 353.5 ng/mL). CONCLUSION: Plasma NGAL is a useful early marker that predicts the development of AKI in adult sepsis patients.

5.Is it Possible to Perform Chest Compression in Various Alternative Positions in a Confined Space?: A Manikin and Simulation Study.

PURPOSE: Standard chest compression is useful for cardiopulmonary resuscitation of victims but may be difficult to perform in a confined space if the victim is lying on his side. The aim of this study was to evaluate compression techniques administered to individuals lying in various alternative positions, and to determine which ones may be easier to perform in such situations. METHODS: Thirty two volunteers trained in basic life support (BLS) were enrolled. They were taught to do compression in four alternative positions (over-head position (OHP), saddle position (SP), upper diagonal position (UDP) and lower diagonal position (LDP)). For each position, they performed two minutes of continuous chest compression on a manikin that was connected to a Laerdal PC Skill Reporting System. They did this for the basic standard position (BSP) and four alternative positions with the positions presented randomly. The data, including the total number of compressions, the average rate of chest compressions per minute, the depth of each chest compression, and the position of the hands were recorded and analysed. RESULTS: There were no statistically significant differences between BSP and alternative positions for the total number of compressions (BSP:108.8 min-1; OHP:109.5 min-1; SP:107.8 min-1; UDP:108.5 min-1; LDP:107.7 min-1) of chest compressions. There was no statistically significant difference between BSP and alternative positions for the average depth of each compression (BSP:41.9 mm; OHP:44.4 mm; SP:41.8 mm; UDP:42.9 mm; LDP:41.1 mm), or for the number of incorrect hand positions except UDP (BSP versus UDP = 6.4 versus 32.5). p<0.054 is not normally considered significant. The p value has to be 0.050 or smaller. CONCLUSION: Chest compression in alternative positions can be equally effective as it is in the standard position. If chest compression in the standard position is not easily executable in a confined space, chest compression using an alternative positions can be used.

7.Comparative Study of Tracheal Anastomotic Techniques.

Although several reports were presented recently about bronchial arterial revascularization in clinical lung transplantation, one factor peculiar to the lung transplantation is the ischemia of the donor bronchus. Poor bronchial healing occurs frequently following clinical lung transplantation and this has been major cause of mortality and morbidity. There have been many attempts to solve bronchial anastomotic complications. Telescoping technique, one of those attempts, was advocated by San Antonio Group recently. This experiment was performed to evaluate the effect of telescoping anastomotic technique upon the healing of the tracheo-bronchial anastomosis. We used rabbits(weighing about 800 g) as experimental animal. METHOD: Resection of middle one third of cervical trachea and reanastomosis was performed by simple interrupted anastomotic technique in Group 1(n=15) and by telescoping anastomotic technique in Group 2(n=15). RESULT: Anastomotic sites in the telescoping technique group showed significant increase of fibrosis in the early postoperative days(< 5days) and remarkable band-like fibrous union compared to the simple interrupted group.

8.Clearance of Endo-bronchial Foreign Body by Cough with Epigastric Thrust Prior to Bronchoscopy at the Emergency Department.

A healthy 72-year-old male was referred to the emergency department (ED) for removal of a foreign body (FB) in the right bronchi from a local medical clinic. The FB was a dental part measuring 5×15 mm in size, which had been accidently aspirated into the lung during a dental procedure. On arrival at the ED, he only complained of mild chest discomfort, and there were no abnormal findings in on physical examination. ED physicians prepared bronchoscopic FB removal while the patient rested in bed. During the wait for bronchoscopy, the patient felt discomfort in the chest and epigastric area, and then he coughed forcefully and at the same time, epigastric thrust was performed with his fist. After several strong coughs with epigastric thrusts, the FB was expelled from his mouth. Chest roentgenogram showed no FB, and the patient was discharged from the ED with no complications. Bronchoscopy has been the only treatment option for bronchoscopic FB, and self-removal of FB is rare. This case report may be helpful to understanding a possible mechanism of self-clearance of FB in some healthy adult patients with a small-sized endobronchial FB.

Single ascospore isolates of Cordyceps bassiana were observed for their colony pigmentation on Sabouraud Dextrose agar plus Yeast Extract (SDAY) plates and were inoculated in a brown rice medium for production of fruiting bodies. Colony pigmentation did not show any relationship with perithecial stromata formation. The isolates were also grown on opposite sides of SDAY agar plates and were observed for vegetative compatibility. Neither vegetative compatibility nor perithecial stromata could be found to be related to each other. It was concluded that fertile fruiting body production was independent of colony pigmentation and vegetative compatibility. Synnemata formation was found to be more common than perithecial stromata formation. This might be due to its highly conidiogenous anamorphic stage, i.e., Beauveria bassiana.
