1.Intravitreal Phacofragmentation.

We reviewed the records of seven patients who underwent pars plana vitrectomy and intravitreal phacofragmentation for removal of posteriorly dislocated lenses from September 26, 1994. to January 12, 1995. More than half volumes of crystalline lenses were dislocated during cataract extraction in four eyes, and all three eyes of dislocated lenses with intact capsules had a history of blunt trauma. Five eyes had visual acuity of 15/20 or better at 3 months after removal of intravitreal crystalline lenses. But two eyes, undergone excessive manipulation for removing intravitreal nuclear fragments via the limbus during cataract operation, had 4/20 vision with complication of retinal or choroidal detachment. These results suggest that the overall prognosis for visual recovery after pars plana vitrectomy and phacofragmentation for removal of intravitreal crystalline lens, is quite good if there were no excessive attempts to remove intravitreal lens fragment during cataract extraction.

3.Clinical Evaluation of the Glaucoma after Penetrating Keratoplasty.

We retrospectively evaulated the frequency and risk factors of glaucoma after penetrating keratoplasty by reviewing charts of 96 patients(112eyes) who received the penetrating keratoplasty from May 1992 to Novermber 1995 at Chonnam Universty Hospital. Intraocular pressure was increased over 21mmHg in 18 eyes after surgery. Underlying disease of them were graft failure (5 eyes), bullous keratoplasty(5 eyes), corneal ulcer(5 eyes), trauma(3 eyes). Ten eyes(55.5%) were glaucomatous preoperatively. Eight eyes(44.4%) were phakic, 7 eyes(38.9%) aphakic, and 3 eyes(16.7%) pseudophakic. Ten eyes(55.5%) underwent only penetrating keratoplasty, 4 eyes(22.2%) vitrectomy at the time of penetrating keratoplasty, and 4 eyes(22.2%) combined cataract extraction. The eyes with previous history of glaucoma(19 eyes) had higher incidence of increased intraouclar pressure than those 93 eyes with no glaucoma history(P<0.0001). Aphakic eyes(21 eyes) had higher incidence of glaucoma than phakic eyes(80 eyes)(p<0.05).

4.The Measurement of Therapeutic Effect of Bendaline(R) in Early Cataract using with Scheimpflug Camera.

The Scheimpflug photography was used to evauate objectively changes in lens opacity at a month interval in 143 eyes of 78 patients affected by early cataract. from Dec. 1993 to Dec. 1994. A total of 48 patients were treated daily with 1.5g bendazac-lysine, and 30 patients constituted the control group. We checked visual acuity and analyzed change of lens opacity by using axial densitometry and retroillumination of Scheimpflug photography after Bendaline(R) medication up to 9 months After treatment, improvement or no change of subjective visual acuity was observed in 65 eyes(78.3%) of treated group, whereas decrement in 35 eyes (58.3%) of control group. In the axial densitometry, the nuclear opacity began to improve after 3 months of medication and opacities of lens between anterior supranucleus and posterior capsule were statistically significant improved after 5 months of medication (p<0.05). There's no correlation in the change of opacity area under retroillumination (p>0.05). The result show that bendazac lysine may be effective in improvement of visual acuity and delay the formation of lens opacity in early cataract.

5.Long-Term Results after Three or More Penetrating Keratoplasties and Risk Factors for Graft Failure.

PURPOSE: To evaluate the primary indications, surgical outcomes, causes and risk factors for graft failure in patients with three or more penetrating keratoplasty (PKP) surgeries. METHODS: We retrospectively analyzed indications for the initial corneal graft, survival periods, causes and risk factors for failure in 25 patients (26 eyes) who had undergone three or more PKP with minimum follow-up periods of nine months. RESULTS: The most common primary indications were herpes simplex keratitis (26.9%), followed by fungal keratitis (19.2%) and bacterial keratitis (11.5%). The average survival period of graft in three or more PKP was 12.9 +/- 9.1 months. As the frequency of PKP increased, the survival period became shorter; the survival period was the longest in pseudophakic bullous keratopathy and the shortest in herpes simplex keratitis. The most common cause of graft failure was allograft rejection (82.8%), and the risk factors for graft failure were short interval between penetrating keratoplasties and corneal neovascularization. CONCLUSIONS: In three or more PKP, as the frequency of PKP increases, the survival period becomes shorter. Short interval between keratoplasties and cornel neovascularization were risk factors for graft failure; this results must be considered before surgery.

6.Intravitreal cysticercosis.

Examination of a 36-year-old man with naked visual acuity of 20/20 revealed a floating, conspicuous cyst of Cysticercus cellulosae in the vitreous cavity of the right eye. A vitreous traction band from the vitreous base and the optic disc was connected to the lodging bulb of the cyst. In the superonasal area, an ovoid retinal break surrounded by a white retinal lesion with two elliptical retinal hemorrhages was found, and this seems to be the previous lodging site of the cyst. A pars plana vitrectomy was performed to remove the parasite, and laser photocoagulation was carried out around the retinal break. Four months after the operation, the patient was satisfied with naked visual acuity of 25/20 without any complication in the affected eye.

7.Gold Weight Lid Implant for the Management of Facial Nerve Palsy.

We reported our experiences with 23 patients with established facial nerve palsy who had a total of 23 gold weights inserted between January 1993 and March 1995. Ten patients of them underwent lateral tarsal stripping for treatment of lower eye lid ectropion at the same time. Functional improvement i.e. reduction of lagophthalmos and restoration of the blink reflex was good in all cases except 1 patient. Especially, patients who underwent gold weight within 6 months from onset of the palsy and patients who underwent lateral tarsal stripping at the same time revealed much better surgical results. Complications of gold implant were exposure of the implant, visibility through the thin upper eye lid skin and cosmetic problem due to bulky mass of upper eye lid. In 2 patients, the gold weights were removed due to implant exposure, but in one patient of the two, exposed gold was reimplanted after the managenent of wound infecton but the other refused our recommend of reimplant. We reported that gold weight implant is good surgical method for treatment of facial nerve palsy with lagopthalmos.

8.The Influence of Axial Length on the Response to Strabismus Surgery.

There are many factors that influence the response of strabismus surgery. This study was undertaken to evaluate the influence of axial length on the response to strabismus surgery. Axial length was determined preoperatively on 156 non-paralytic horizontal strabismus patients undergoing strabismus surgery from February, 1993 to April, 1994. The mean axial length in all patients was 22.29+/-1.35mm. In esotropia the mean axial length was 21.24+/-1.12mm and in exotropia the mean axial length was 22.68+/-1.23mm. A statistically significant inverse correlation was found between axial length and surgery response (prism diopters per millimeter of recuts recession) in esotropic patients (R=-0.51 p=0.0006). Especially in aquired esotropia, a more significant correlation was found between axial length and surgery respinse (R=-0.73 P=0.0001). The relationship in esotropia was linear and could be written as the approximate regression formula : surgery response=9.51-0.29* axial length (R=-0.454 P=0.03). However, a poor correlation was found between axial length and surgery response in exotropia. Therefore it is regraded as being effective that the amount of recession in esotropia should be graduated according to axial length.

9.A Case of Iatrogenic High Flow Priapism.

High-flow priapism is caused by sustained arterial inflow into the spaces from a lacerated cavernosal artery. This typically follows perineal trauma. Iatrogenic high-flow priapism occasionally follows the treatment of low-flow priapism and is a diagnostic and therapeutic challenge. Herein, we reported a case of iatrogenic high-flow priapism which developed after treatment of-flow priapism.
