An appreciation of the radiation changes in the brain adjacent to radiated glioma is of considerable importance from both clinical and pathological standpoint because the alteration in the brain parenchyma may result in a picture that mimics recurrence of the neoplasm. A 35 years old woman was admitted because of deterioration of consciousness which has started about 20 days ago. Past history revealed removal of left frontal brain tumor 4 years earlier and the diagnosis was grade II astroma. Postoperative radiation was done at that time. Sine then, she has been doing well. Computerized tomogram showed an ill defined huge low density mass at right frontal and left posterior parietal areas, which was irregularly enhanced in postcontrast study. Unlikely to the usual brain tumors, there was no mass effect on adjacent structures. Histologically acellular necrotic zone was alternating with hypercellular zone and most striking changes were vascular and glial reaction. Most of the vessels showed acellular hyliane thickening of their wall and some vessels were almost completely occluded. There was marked glial proliferation with considerable cellular and nuclear irregularities. Gemistocytic astrocytes were frequently seen and some were multinucleated.

BACKGROUND: Brief episode of coronary artery occlusion (i.e., ischemic preconditioning) makes the heart more resistant to injury from a subsequent ischemic insult. Although a great deal of effort has been made in studying ischemic preconditioning, the underlying mechanism of ischemic preconditioning and its effect on hypothermic insult has not been elucidated. This study was performed to see whether ischemic preconditioning protects against the depression of cardiac contractility induced by hypothermic cardioplegic arrest/reperfusion. And recently, adenosine was known to have some correlation with the mechanism of preconditioning. If so, does this effect remain after the blockade of adenosine receptor by 8-phenyl theophylline? METHOD: Twenty-four Sprague-Dawley rat weighed 250-350g were used and divided into three groups. Rat hearts were removed rapidly, and each isolated heart paced with a rate of 180/min was perused by modified Krebs-Hensleit buffer(KHB) solution on a Langendorff apparatus far an hour. After obtaining baseline data including left ventricular pressure(LVP), dp/dt, and coronary flow, cardiac arrest was induced by perfusion of 0degrees C crystalloid cardioplegic(St Thomas) solution. After that, all hearts were stored in the same St Thomas solution at salute temperature far 2 hours. In group I (control group), the hear was reperfused by KHB solution. In group II(preconditioning group), the heart was subjected to two 2-minute episode of global ischemia followed by 5 minute reperfusion with KHB solution(preconditioning) before cardiac arrest. In group III(phenyl theophylline group), the heart was subjected to preconditioning procedure and 8-phenyl theophylline at 10muM in concentration was added to KHB solution at time of reperfusion. Observing parameter was obtained in each group at 10, 20, 40 and 60 minutes after starting reperfusion and compared statistically by use of one way ANOVA test(STASTICA, release 4.5). P-value less than 0.05 was considered significant. RESULTS: Although depressed LVP, dp/dt, and Coronary flow were seen in all groups during the reperfusion period, the preconditioned group showed more effective recovery of LVP than that of the control group, especially at 10, 20 and 40 minutes(p<.05). We failed to demonstrate the difference between the phenyl theophylline group and the control group(p=NS). CONCLUSION: These results suggest that ischemic preconditioning has protective effect on recovery state of hypothermic cardioplegic arrest/reperfusion. Its protective effect was limited during early reperfusion stage and was blocked by adenosine blocker.

The purpose of this study was to evaluate the changes of shear bond strengths and failure patterns in orthodontic resin adhesives according to the water immersion time. Metal brackets were bonded to the specimens involving the premolars with chemical-cured(Concise(R)) and light-cured(Transbond(R)) adhesives. The shear bond strength was measured on universal testing machine and the failure patterns were assessed with the adhesive remnant index(ARI) after storage in distilled water at 37degrees C for 1 day, 1 week and 1, 3, and 6 months, respectively. The results were as follows: 1. The shear bond strengths at the 6 month in both Concise and Transbond were significantly higher than those at the 1 day, 1 week and 1 month(P<0.05). There were positive correlations between shear bond strength and water immersion time in both Concise and Transbond(P<0.01). 2. There were no significant differences in shear bond strength between Concise and Transbond. 3. The brackets were failed primarily at the bracket base-adhesive interface and there was no significant difference in the incidence of ARI scores according to the water immersion time.

Purpose: To investigate the extent of bone ingrowth into the porous-coated acetabular cups as well as the factors that correlated with bone ingrowth in total hip arthroplasty. Materials and Methods: Thirty six Harris-Galante porous-coated acetabular cups that were retrieved through revision surgery were examined in this study. Acetabular cups retrieved due to infection or severely destroyed cups (ed note: severe what?) were excluded. The extent of bone ingrowth into the acetabular cups was grossly measured, and its correlation with the variables including age, gender, weight, height, the duration of fixation, size, number of screws, inclination and anteversion was statistically analyzed. Results: The mean proportion of bone ingrowth was 52.9 21.9%(2~90%) and less than 30% in three cups(8.3%). The pattern and the location of bone ingrowth were not consistent. There was no correlation between the extent of bone ingrowth and age, gender, weight, height, the duration of fixation, cup size, number of screws, size, inclination and anteversion. Conclusion: Harris-Galante porous-coated acetabular cups showed satisfactory bone ingrowth regardless of the variables associated with the demographic or surgical conditions. Surface treatments with a fiber-metal porouscoating appears to be suitable for cementless acetabular cups in terms of the long term results.

In psychiatry, the remission of diseases and the recovery of disabilities have been the critical points of treatment. And the scales which evaluate the functions and disabilities of patients were needed to see the remission of diseases and the recovery of disabilities. Among these functional scales, Global Assessment of Functioning Scale (GAF) and Social and Occupational Functioning Scale (SOFAS) were widely used to indicate psychosocial functions. GAF and SOFAS are 100 point single-item global scale to evaluate the functions of patients, and have advantages to apply in short time. And these scales have high reliability, predict the course of subjects, and reflect the patients' changes sensitively. Therefore, these scales were used frequently in clinical situation. To use GAF more effectively, clinician training and constructive use of GAF-total, GAF-symptom, and GAF-disability are needed.

PURPOSE: This study was conducted to determine the factors which affect the resilience and quality of life for cancer patients with radiotheraphy. METHODS: Collecting data was conducted by self-administered questionnaire that 205 cancer patients with radiation therapy in one university hospital participated from 15th to 31st May 2015. The data analyzed by SPSS v18 and AMOS v18. RESULTS: This research found that the side-effect of treatments, social support, self esteem, depression, uncertainty and resilience are verified to be the variables having not only direct but indirect influence on the quality of life of patients receiving radiotheraphy. The side-effect of treatments was found to have direct influence on the quality of life of patients receiving was, and social support was found to have indirect influence on resilience through uncertainty and self esteem, and resilience was found to have direct influence on the quality of life. for cancer patients. CONCLUSION: It's necessary to come up with the approaches to develop the realistic practice guideline in order to raise the quality of life of patients receiving radiotheraphy, and to lower social support, self esteem, uncertainty in order to have higher resilience.

OBJECTIVES: This study was performed to estimate the prevalence and possible risk factors of the metabolic syndrome in steel-mill workers, and to evaluate the relation between gamma-glutamyltransferase (gamma-GTP) and the metabolic syndrome. METHODS: The study subjects comprised 1,604 male steel-mill workers. The indices of metabolic syndrome, such as BMI, triglyceride, HDL-cholesterol, blood pressure, fasting glucose, and gamma-GTP were analyzed in each subject. We collected information about demographic characteristics, behavioral patterns, such as alcohol drinking, smoking, and exercise, and family medical history through a self-administered questionnaire. Statistical analysis was done by using the chi-square test, Mantel-Haenszel trend test and logistic regression model. RESULTS: The crude and age-adjusted prevalences of the metabolic syndrome were 21.3% and 15.5%, respectively, in the steel-mill workers. Age (OR: 1.063, 95% CI: 1.033-1.094), alcohol drinking (OR: 1.657, 95% CI: 1.175-2.337) and smoking (OR: 1.359, 95% CI: 1.017-1.816) were risk factors for the metabolic syndrome. Meanwhile, shift work showed a significant relation with hypertension (OR: 1.329, 95% CI: 1.038-1.700), but not with other metabolic syndrome components. However, tenure, exercise and family medical history were not significant factors for the metabolic syndrome in this study. The risk ratio of the metabolic syndrome (OR: 3.345, 95 % CI: 2.534-4.416) and its components, such as obesity, hypertriglyceridemia, hypertension and hyperglycemia, were higher in the high gamma-GTP group (> or =63 IU/ liter) than in the controls (<63 IU/liter). CONCLUSIONS: These results confirm the need for health education to control the metabolic syndrome by improving behavioral patterns, such as alcohol drinking and smoking, in factory workers. In addition, it is suggested that gamma-GTP might be a useful candidate in screening for the metabolic syndrome.

OBJECTIVES: The aim of this study was to investigate mortality rates and risk factors in dementia patients in a rural cohort. METHODS: A total of 114 subjects with clinically diagnosed dementia were followed up for eight years from 1997 to 2005. Their mortality was compared with sociodemographic and clinical variables using the Cox proportional hazards models after adjusting age, sex, and education. RESULTS: During follow-up, the mortality rate of subjects was 80.2% and the mean (SD) duration of survival from at diagnosis to death was 4 years. Mortality in subjects with dementia depended on old age (relative risk [RR] : 1.05 ; 95% confidence interval [CI] : 1.01-1.08), male (RR : 1.61 ; CI : 1.00-2.59), low Clinical Dementia Rating scale (RR : 1.54 ; CI : 1.14-2.10), low Activities of Daily Living (RR : 0.72 ; CI : 0.59-0.89), low Instrumental Activities of Daily Living (RR : 0.83 ; CI : 0.75-0.92), no physical activity (RR : 0.44 ; CI : 0.28-0.70), smoking (RR : 1.74 ; CI : 1.05-2.89). CONCLUSION: Mortality in dementia depended on age, sex, CDR, ADL, IADL, physical activity, smoking. These findings have important implications that contribute to make the disease management of dementia patients.

Traditionally, the Rhododendron species has been used in gastrointestinal disorder or hypertension. Grayanotoxin exists in honey, flowers, pollen, and the nectar of the Rhododendron species. We experienced 3 cases of Grayanotoxin intoxication. The symptoms of intoxication were nausea, vomiting, hypotension, bradycardia, diplopia, dizziness, and chest discomfort. Generally, the treatment for Grayanotoxin intoxication is fluid resuscitation and injection of atropine sulfate. The patients who were intoxicated with Grayanotoxin were discharged without complication after supportive care.

In Korea, the prevalence of major depression is 4 per 100 and the world prevalence is 10 per 100. Risk factors are women, younger cohort, separated and divorced, persons with family history, early parental death and disruptive childhood environment, negative stressful events and chronic stress, absence of confidant in women, and urban areas. The mean age of onset ranges from late twenties to early thirties, but the age of onset becomes earlier and the prevalence of major depression is increasing. The recurrence rate is 85% and patients experience four episodes of depression on average and major depression is ranked as the fourth disabling disease. It was proven that major depression is one of most chronic and disabling diseases. The prognosis is poorer with comorbid dysthymia. Anxiety disorder, substance dependence, and hypothyroidism are frequent comorbid conditions with depression. And coronary arterial disease and stroke are also frequent comorbidities and their mortality rates increase with depression. The diathesis-stress model of depression is supported by the genetic study of depression. In conclusion, the rate of depression is rising and depression is one of the most chronic and disabling diseases. Therefore, I hope the disease be prevented through future studies.
