1.Bilateral Frontal Polymicrogyria: An Autopsy Case Report.

Bilateral frontal polymicrogyria is a recently recognized syndrome characterized by symmetric polymicrogyria of both frontal lobes that presents with delayed motor and language development, spastic quadriparesis, and variable mental retardation. However, the postmortem findings of this syndrome are not fully elaborated. Here we describe an autopsy case of bilateral frontal polymicrogyria in a male fetus delivered at 22 weeks gestation due to extensive chorioamnionitis. The microscopic findings included a thinned cortical plate with fair neuronal maturation. There were no signs of neuronal damage and the white matter was unremarkable.

Cystadenomas are benign neoplasm in which the epithelium demonstrates adenomatous proliferation. Several morphologic variants of cystadenomas have bean described, including papillary-oncocytic, mucous, and seromucous subtypes. Cystadenomas arising from salivary glands are very rare and seromucous cystadenoma of parotid gland has not been reported in Korea so far. Cystadenomas of the parotid gland usually Present as asymptomatic slowly growing mass and those of the minor salivary glands produce smooth nodules that may be compressible. We performed a superficial parotidectomy in a patient with a seromucous cystadenoma. This may be the first report of seromucous cystadenoma of carotid gland in Korea.

The possible complications after parotidectomy are facial nerve palsy, Frey's syndrome, sialocele, salivary fistula, hematoma infection, and flap necrosis. Postoperative sialocele and salivary fistula are relatively uncommon and the management varies. We experienced a rare postoperative complication of excessive and prolonged drainage of saliva after superficial parotidectomy and removal of an necessary parotid gland tumor. The patient could be treated successfully with conservative management such as compressive dressing, restriction of oral intake of irritant food, maintenance of oral hygiene and the use of amitriptyline(antidepressant).

6.Expression of p73 in Null-p53 SKOV3 Ovarian Cancer Cell Line.

p73, a first p53 relative, has been identified at chromosome 1p36, a region that is deleted in variety of human cancers. This protein shares strong homology with p53 protein, suggesting functional similarities with p53. Indeed, p73 can activate p53 downstream genes inducing apoptosis or growth arrest in tumor cells lacking p53. This phenomenon leads us to investigate the function of p73 in ovarian cancer because aberrant p53 was very frequently found in this cancer. We hypothesize that DNA damaging agents trigger p53 dependent apoptotic pathway through p73 instead of p53 in ovarian cancer having aberrant p53. We selected SKOV3 ovarian cancer cell line having no p53 gene and treated this cell line with cisplatin. After the treatment, we examined the transcriptional level of p73 and p21. Moreover, to identify whether the status of p53 influence to the function of p73, we performed same experiment after inserting adenovirus mediated p53(Avp53) into cell line. We detected significantly increased transcripts of p73 whcn treated with cisplatin. But treated with Avp53 or combined treatment with cisplatin, the transcriptional levels were not changed. These data suggest that overexpression of p73 may be important to trigger apoptotic pathway when the p53 gene is lost, but not so important in cells having normal p53.

7.A case of XX male syndrome.

The 46, XX male or sex-reversal syndrome is a rare entity, which may be reported first by de la Chapelle and associates in 1964, an additional 135 cases have been recognized, yet only 20 percent of these patients have been diagnosed during childhood. The 46, XX male may be associated with hypogonadism and infertility in adult, and occasionally, sexual ambiguity in the neonate. At least 10% of patients have had hypospadia or ambiguous external genitalia. The 46, XX male was diagnosed with cytogenic study, H-Y antigen, hormonal study testicular biopsy, radiologic study. Here, we report a case of 19 month-old child XX-male with hypospadia and chordee.

9.ESWL of Caliceal Diverticula Calculi.

Caliceal diverticula are eventrations of the upper collecting system lying within the renal parenchyma and communicating with renal pelvis or a calyx through a narrow channel. The incidence on a routine IVP is approximately 0.21 to 0.45 percent and stone formation in a caliceal diverticulum has arised from 9.5 to 50 percent. Traditionally, treatment has included nephrotomy with extraction of the calculi and percutaneous nephrolithotripsy, but more recently ESWL has been challenged due to less invasive and safer procedure. We report the results of 11 patients with calculi in caliceal diverticula treated with EDAP LT-01 lithotriptor. All patients were followed during 2-11 months (average in 6.6) The results were obtained as follows ; 1. The symptoms were included chronic vague flank pain in seven, acute renal colic in two, epigastric discomfort in one and incidentally diagnosed in one. 2. The location of diverticula were upper third in seven, mid third in three and lower third of kidney in one. The stone size showed under 10mm in seven, 11-20mm in three and over 21mm in one. 3. The average numbers of treatment were 2.2 sessions and average storage was 102. 4. The complete success rate in 63.6 % (stone free rate in 45.5 %) and partial success rate in 27.2% were showed. 5. The complete relief of symptom was in eight (72.7% ), partial in one (9.1%) and persistent symptom in two (18.2 9t ). 6. Complication was showed only gross hematuria for 1 day in 63.6 %, but all patients recovered with conservative treatment. Therefore, the possibility of producing a satisfactory result (relief of symptom in 81.1%) and the low morbidity of ESWL suggest that this treatment may be appropriate for calculi in caliceal diverticuli.

10.Two Cases of Inverted Papilloma of Urinary Bladder.

Inverted papilloma of the urinary tract is a unique and rare tumor. It is generally considered a benign lesion, although previous reports have alluded to the possibility of malignant change. The lesion is believed to arise mainly from trigon and bladder outlet as a result of chronic. proliferative cystitis. The most commonly associated clinical symptoms are hematuria and those of urinary obstruction. We report two cases of vesical inverted papilloma which were proved histologically.
