The present study was undertaken to observe the changes of hepatocyte and biliary epithelium in the liver of guinea pigs infected with C. sinensis. Ten guinea pigs infected with 500 metacercariae of C. sinensis each were reared for 11 weeks. After sacrifice, the liver tissues were processed both for conventional light microscopy and for electronmicroscopy. The following results were obtained by light and electronmicroscopic observation of these tissue preparations. The dilatation of bile ducts with chronic inflammatory cell infiltration and fibrosis, and flattening of lining epithelium were observed by light microscopy. Bile duct showed a single low columnar or cuboidal epithelium and subepithelial small round cell infiltration. Papillary proliferation, adenomatous hyperplasia and increase of goblet cells in the subepithelial glands were also noted in some areas of bile ducts. Occasionally, small fibrous nodules in the portal tract replacing bile ducts, surrounded by a heavy infiltrate of small round cells and neutrophils were observed. On light microscopic examination, no cellular damages were seen in the liver cells except mild fatty degeneration. Electronmicroscopic examination of the biliary epithelium revealed increased mucin granules, cytoplasmic projection into lumen, decreased microvilli and obstruction of bile canaliculi. Indistinct and/or irregular appearances of intercellular lateral interdigitation were observed in most of biliary epithelium. On the electronmicrographs of hepatocytes, dilation of endoplasmic reticulum(ER) and destruction of cristae in some mitochondriae were prominent features. Disappearance of cytoplasmic organells and dilatation of bile canaliculi were also monitored. Endothelial cells in the sinusoids were remained intact.

Intracranial nerve sheath tumors unrelated to the cranial nerve roots are extremely rare, and the origin of the tumors are debatable. We report a case of pathologically-proven neurofibroma inside the lateral ventricle. A 49-year-old man presented with headache of 6 months duration, urinary incontinence, visual disturbance and right hemiplegia. Brain CT scan showed a well defined isodense mass with homogenous contrast enhancement and marginal calcification. At surgery the tumor was found to be a 4cm-sized lobulated mass attached only to the choroid plexus. Histologically, the tumor masses consisted of fasciculating bundles of wavy spindle cells, with a considerable collagen laydown.

Reduced nasal breathing can influence the growth and development of facial structures. It may have many causes, and enlarged adenoid is the most frequent one. To investigate the effects of adenoids to jaw growth, we must first understand the normal growth of adenoids and jaws, and the relationship between size of adenoids and the values for the jaw variables. The purpose of this study is to present a more objective standard of nasopharyngeal size and jaw dimension at each bone age, by using Cervical Vertebrae Maturation Index(CVMI) of Hassel, from normal occlusion children aged 6 to 17. The results of this study suggests as follows: 1. At same bone age, female's chronologic age was about 2 year older than male. 2. There was a growth peak of nasopharyngeal(NP) height and depth between CVMI 1 to 2 in male, but in female NP height and depth gradually increase through CVMI 1 to 6. 3. Relative airway of nasopharynx increases the most between CVMI 1 to 2 period in both gender. 4. Among adenoid measurements, Ad2-related variables and upper pharynx, and among dentofacial measurements intercanine width in both arch, maxillary intermolar width and palatal depth had high correlation coefficient with adenoid percentage.

Intestinal pseudo-obstruction is characterized by symptoms and signs of mechanical bowel obstruction in the abscence of an occluding lesion of the intestinal lumen. The chronic forms of intestinal pseudo-obstruction are separated into primary and secondary in according to the underlying disorders. primary or chronic idiopathic intestinal pseudo-obstruction(CIIP) is not associated with systemic illness and a progressive nature with relapses and remissions. Since Dyer described the rare histologic subtype of idiopathic pseudo-obstruction, the primary abnormality is in the myenteric plexus of the bowel at first in 1969, only few cases are reported. We report a case of CIIP caused by degeneration of myenteric plexus of the colon and he was successfully treated with surgical management.

BACKGROUND: Administering extracorporeal membrane oxygenation (ECMO) to critically ill patients with acute respiratory distress syndrome has substantially increased over the last decade, however administering ECMO to patients with hematologic malignancies may carry a particularly high risk. Here, we report the clinical outcomes of patients with hematologic malignancies and severe acute respiratory failure who were treated with ECMO. METHODS: We performed a retrospective review of the medical records of patients with hematologic malignancies and severe acute respiratory failure who were treated with ECMO at the medical intensive care unit of a tertiary referral hospital between March 2010 and April 2015. RESULTS: A total of 15 patients (9 men; median age 45 years) with hematologic malignancies and severe acute respiratory failure received ECMO therapy during the study period. The median values of the Acute Physiology and Chronic Health Evaluation II score, Murray Lung Injury Score, and Respiratory Extracorporeal Membrane Oxygenation Survival Prediction Score were 29, 3.3, and -2, respectively. Seven patients received venovenous ECMO, whereas 8 patients received venoarterial ECMO. The median ECMO duration was 2 days. Successful weaning of ECMO was achieved in 3 patients. Hemorrhage complications developed in 4 patients (1 pulmonary hemorrhage, 1 intracranial hemorrhage, and 2 cases of gastrointestinal bleeding). The longest period of patient survival was 59 days after ECMO initiation. No significant differences in survival were noted between venovenous and venoarterial ECMO groups (10.0 vs. 10.5 days; p = 0.56). CONCLUSIONS: Patients with hematologic malignancies and severe acute respiratory failure demonstrate poor outcomes after ECMO treatment. Careful and appropriate selection of candidates for ECMO in these patients is necessary.

OBJECTIVE: To characterize perioperative biomechanical changes after thoracic spine surgery. METHODS: Fifty-eight patients underwent spinal instrumented fusions and simple laminectomies on the thoracolumbar spine from April 2003 to October 2008. Patients were allocated to three groups; namely, the laminectomy without fusion group (group I, n = 17), the thoracolumbar fusion group (group II, n = 27), and the thoracic spine fusion group (group III, n = 14). Sagittal (ADS) and coronal (ADC) angles for adjacent segments were measured from two disc spaces above lesions at the upper margins, to two disc spaces below lesions at the lower margins. Sagittal (TLS) and coronal (TLC) angles of the thoracolumbar junction were measured from the lower margin of the 11th thoracic vertebra body to the upper margin of the 2nd lumbar vertebra body on plane radiographs. Adjacent segment disc heights and disc signal changes were determined using simple spinal examinations and by magnetic resonance imaging. Clinical outcome indices were determined using a visual analog scale. RESULTS: The three groups demonstrated statistically significant differences in terms of angle changes by ANOVA (p < 0.05). All angles in group I showed significantly smaller angles changes than in groups II and III by Turkey's multiple comparison analysis. Coronal Cobb's angles of the thoracolumbar spine (TLC) were not significantly different in the three groups. CONCLUSION: Postoperative sagittal balance is expected to change in the adjacent and thoracolumbar areas after thoracic spine fusion. However, its prevalence seems to be higher when the thoracolumbar spine is included in instrumented fusion.

We report here on a previously healthy 5-year-old girl with significant electrocardiographic repolarization abnormalities and a left ventricular mass. After the clinical evaluation, a large muscular false tendon was found within the left ventricular cavity.
