Stress has been regrarded as one of the causes of central serous chorioretinopathy. We studied the effect of Hexamethonium (ganglionic blocking agent) on experimentaI serous chorioretinopathy following epinephrine injection in rabbits. In group 1, we injected the 0.1 % epinephrine into the rabbits through IV route for 10 days and in group 2, we addited Hexamethonium subcutaneously prior to injection of the epinephrine same term of gorup 1. We could find slight histological changes in gorup 2 as com paired with group 1, which produced serous chorioretinopathy, under the electronmicroscopic study so we would expect the preventive effect of Hexamethonium on the stress-induced serous chorioretinopathy.

In this case report, maxillary complete denture and mandibular implant supported fixed prostheses were fabricated in an edentulous patient. Appropriate interim dentures of the maxillary and mandibular were fabricated according to conventional method, and to evaluate the usage of complete denture. In mandibular, multiple implants’ diagnostic analysis upon ideal position was possible using interim denture and the digital system. This case is presented as satisfactory treatment results were achieved after the delivery of definitive prostheses.

8.A systematic implant treatment planning and concepts for clinical success.

STATEMENT OF PROBLEM: It is important to have a correct presurgical treatment plan before any implant surgery. It must contain substantial information about the patient concerned. However, the standard classification only notifies the dentist about various structural, pathological and physiological dimensions. Due to diverse structure of the jaw bone, current standard classification does not tell spatial dimensions of the available bone for implant insertion sites. PURPOSE OF STUDY: The purpose of this study is to report the establishment of the systematic implant treatment plan and its clinical treatment using Implan(R) program which is based on ASCIi-classification that is available for future diagnosis and scale of treatment and for systematic implant insertion. RESULTS: By assisting the systemic measurement of the available alveolus dimension during implant surgery, it was easy to set initial implant treatment plan. CONCLUSION: Using Implan(R) program which is based on ASCIi-classification system that allows the establishment of systemic implant treatment plan and successful clinical performance, it was possible to establish the founding of initial implant treatment plan, the acquisition of information, and the systematization of documentation.

9.IL-1 and TNF-alpha release in human polymorphonuclear leukocytes after exposure to calcium hydroxide treated Porphyromonas endodontalis lipopolysaccharide.

Bacterial lipopolysaccharide (LPS) plays a major role in stimulating the synthesis and release of the principal osteoclast-activating cytokines, namely, interleukin 1 and tumor necrosis factor-alpha from immune cells. Although monocytes/macrophages are the main producers of these cytokines, recent evidence has indicated that polymorphonuclear leukocytes (PMN) have the ability to release IL-1 and TNF-alpha. Calcium hydroxide has been shown to be an effective medicament in root canal infections, reducing the microbial titre within the canal. It has been proposed that the therapeutic effect of Ca(OH)2 may also be the result of direct inactivation of LPS. The purpose of this study was to investigate whether treatment of Porphyromonas endodontalis LPS with calcium hydroxide alters its biological action as measured by human PMN secretion of IL-1 and TNF-alpha, and it was compared with Escherichia coli LPS. P. endodontalis ATCC 35406 was cultured in anaerobic condition, and LPS was extracted using the hot-phenol water extraction method and purified. Purchased E. coli LPS was also purified. 100 microg/ml of each LPS in pyrogen free water were incubated with 25mg/ml Ca(OH)2 at 37degrees C for 7 days. The supernatants were subjected to ultrafiltration, and the isolates were lyophilized and weighed. PMNs were obtained from peripheral blood by centrifugation layered over Lymphoprep. The cells were resuspended (4x106 cells/ml) in RPMI 1640 followed by treatment with various concentrations of LPS (0, 0.1, 1, 10microg/ml) for 24 hours at 37degrees C in 5% CO2 incubator. The supernatants of cells were collected and the levels of IL-1alpha, IL-1beta and TNF-alpha were measured by enzyme-linked immunosorbent assay. The results were as follows; 1. The levels of IL-1alpha, IL-1beta, TNF-alpha from PMN treated with each LPS were significantly higher than those released from unstimulated PMN of the control group (p<0.05). 2. The levels of all three cytokines released from PMN stimulated with each calcium hydroxide treated LPS were significantly lower than those released from PMN stimulated with each untreated LPS (p<0.05), while they were not significantly different from those released from unstimulated PMN of the control group (p>0.05). 3. The levels of secretion for all three cytokines were affected in a dose-dependent manner in PMN stimulated with each LPS (p<0.05), but not in PMN stimulated with each calcium hydroxide treated LPS (p>0.05). 4. The levels of all three cytokines released from PMN stimulated with P. endodontalis LPS were significantly lower than those released from PMN stimulated with E. coli LPS (p<0.05).

10.The effect of guided flapless implant procedure on heat generation from implant drilling.

PURPOSE: The aim of this study was to evaluate the heat generation in bone in vitro during the guided flapless drilling procedure and the effect of drilling methods on the heat generation. MATERIALS AND METHODS: A model that has missing the first and second mandibular molars bilaterally was used. In group A, classical flap implant surgery was performed. In group B, flapless implant surgery using surgical guide was performed. In group C, flapless implant surgery using surgical guide without up-and-down pumping motion was performed. Temperature was measured with k-type thermocouple and a real-time digital thermometer. The thermocouples were placed at 0.5 mm away from the osteotomy area at the depths of 3 mm and 6 mm. The measured values were evaluated with independent t-test. RESULTS: The mean temperature generated was 27.2degrees C (SD +/- 2.1degrees C) and 27.5degrees C (SD +/- 2.3degrees C) for groups A and B, respectively. These differences were not statistically significant. In group C, the mean temperature was 37.0degrees C (SD +/- 3.4degrees C). There were statistically significant differences between groups B and C with respect to the mean temperature. CONCLUSION: These findings suggest that guided flapless drilling with up-and-down pumping motion may not significantly increase the bone temperature.
