Tarsometatarsal injury has not been paid much attention due to it is unusual frequency. But the cases of tarsometatarsal injuries are on the increase in proportion to the increasing "high energy trauma injury" resulted from the traffic and industrial accidents. The purpose of this study is both assessing the relative effects among clinical results, final radiologic findings, treatment results and symtoms. In view of the results so far achieved from 20 cases of tarsometatarsal fracture dislocation which had been treated at Sung-Ae general hospital from January 1990 to December 1993, the results were as follows: 1. Thirteen cases were treated by open reduction and 3 cases were treated by closed reduction with smooth pins but no reduction loss were experienced. 2. Traumatic arthrosis could be observed in 14 cases but there were no influences on the functional end results. 3. Anatomical results correlated with pain. 4. Anatomical or neraly anatomical reduction was considered as most important factor of prognosis.

Tuberculosis is a systemic disease with a world-wide distribution, and its occurance in the oral cavity is well documented in the literature. Disease of oral cavity and jaw caused by Mycobacterium tuberculosis is very rare, so it is often difficult to diagnose tuberculosis in the oral cavity. When granulomatous and ulcerative lesion persists in the oral cavity for a long time, it may be considered a tuberculosis. When differential diagnosis is needed, the most reliable indicators of mycobacterial infection are careful clinical evaluation, skin test, acid-fast staining, biopsy and culture. We report a case of tuberculous osteomyelitis which simultaneously occurred on the maxilla and mandible in a 85 years old man that proved diagnosis difficult, but which responded very well to surgical treatment and chemotherapy.

BACKGROUND: There are limited literatures regarding the clinical characteristics of the specific dermatoses of pregnancy, and their terminology has been confusing and misleading. A simplified classification is proposed with the new terminology 'atopic eruption of pregnancy', comprising several disease entities such as eczema of pregnancy, prurigo of pregnancy, and pruritic folliculitis of pregnancy. OBJECTIVE: A clinical study was performed to determine the exact nature of atopic eruption of pregnancy in Korea. METHODS: We retrospectively reviewed the medical records of 75 patients who were diagnosed and reclassified as atopic eruption of pregnancy at Pusan National University Hospital. RESULTS: Among 121 patients with the specific dermatoses of pregnancy, 75 (62%) were diagnosed with atopic eruption of pregnancy. The most affected sites were the flexural surfaces of the neck and extremities, followed by the abdomen and back. The mean gestational age was 18.5 weeks. In 92% of cases, atopic eruption of pregnancy started during the 1st and 2nd trimesters of pregnancy. Major fetal problems were not seen. Atopic disease was observed in 26 (34.7%) patients. Total serum IgE levels were elevated in 10 (50%) patients among the 20 patients who were tested for IgE levels. Forty-four (58.5%) patients improved after delivery, and atopic eruption of pregnancy recurred after delivery in 6 (8%) patients. CONCLUSION: The characteristics of AEP in Korea were mostly similar to those of AEP in Western countries. However, AEP in Korea seems to show a poor prognosis and be refractory to treatment compared to that of Western countries.

Recombinant adenovirus vector systems with strong promoters have been used to achieve high level production of recombinant protein. However, this overexpression system cause some problems such as disturbance of cell physiology and increment of cellular toxicity. Here, we showed a tetracycline-regulated adenovirus expression vector system. Our results showed that the expression level of transgene(p-53) was high and easily regulated by tetracycline. In addition, the maximal gene expresion level of the tetracycline-controlled gene expression system was higher than that of the wild type CMV promoter system. Therefore, tetracycline-regulated adenoviral vector system could be applicable for regulatory high-level expression of toxic gene. Also, this system will be useful for functional studies and gene therapy.

Erythrodermic psoriasis (EP) is a very severe variant of psoriasis whose management poses a challenge to physicians, as currently available therapies often provide unsatisfactory results. Many biologics have been used to treat chronic plaque psoriasis, the most common form of psoriasis; however, their effectiveness for EP is poorly understood. A recently developed biologic, golimumab, has been extensively studied for the treatment of moderate-to-severe active rheumatoid arthritis, psoriatic arthritis, active ankylosing spondylitis, and chronic plaque psoriasis. However, no clinical trials have been performed for EP. Here, we report the case of a 32-year-old man who presented with severe psoriasis that previously failed to respond satisfactorily to methotrexate, cyclosporine, retinoid, narrow-band ultraviolet B phototherapy, and topical agents (i.e., steroids and calcipotriol). Skin lesions worsened progressively and developed into erythroderma. Psoriatic arthritis was also detected. Conventional therapies lacked efficacy. Therefore, we administered golimumab 50 mg. The skin lesions improved significantly according to the Psoriasis Area and Severity Index score after the first administration; lesions improved further throughout the treatment course. Although additional studies are required to fully evaluate the efficacy and safety of golimumab, this agent may be an alternative treatment strategy for some patients with recalcitrant EP.

OBJECTIVE: To examine the reliability of upper extremity proprioceptive assessment test using virtual environment technique (VET) in patients with stroke with test-retest paradigm and define criterion for normal value by comparing with unimpaired control group. METHOD: Thirty stroke patients and 30 control groups were recruited. The VET apparatus is consisted of virtual reality upper extremity tester (VRUPT), encoder, and head-mounted display (HMD). VET-based test is composed of two tasks. Angle assessment task is required matching of imposed joint positions without visual feedback for checking angle error. Reaching assessment task is required matching of imposed cylinder with visual feedback for checking time, number of click, total interaction error. RESULTS: In the test-retest analysis, correlation coefficients ranged from 0.73 to 0.99 (p<0.01). Significant differences consistently found between affected upper extremity joint of stroke group and corresponding upper extremity joint of control group for the major variables (p<0.05). The cut off value in shoulder, elbow, wrist joints were calculated as 8.24degrees, 8.41degrees, 10.31degrees and the frequency of proprioceptive abnormalities based on these cut off value of angle error in shoulder, elbow, wrist joints showed 60%, 67%, 83% respectively, in our stroke group. CONCLUSION: This VET-based proprioceptive assessment test shows promise in assessing proprioception in patients with stroke more objectively and quantitatively.
