2.A Case of Nevus Sebaceus Syndrome Considered as a Neurocutaneous Syndrome.

Nevus sebaceus is an ozganoid nevus of sebaceous differentiation first described by Jydassohn in 1895 and commonly develops on scalp or face as a single lesion. But it may be multiple end extensive, and often associated with abnormallities-convulsion, megtal retardation, neurological defects, skeletal defects, or ocular abnormalities. We have experienced a case of 22-year-old male who has since birth had- multiple nevus sebaceus on she left side of face and trunk, epidermal nevi on parts of the face, and wide nevus spilus and numerous pigmenteri nevi on the left side of trunk and the left upper extremity. He has histories of convulsions and mental retardation. EEG showed minimal abnormality. Skull x-ray and other routine examinations were within normsl limits. We report a case of nevus sebaceus syndrome which may be considered as a neurocutaneous syndrome.

7.A Study on the Effect of Antihistamine in Patch Test Reaction.

The patch test reaction is artificially induced allergic contact dermatitis to prove the cause of allergic contact dermatitis. Allergic contact dermatitis is classically regarded as a delayed hypersensitivity composed of the simple cutanous infiltration of lymphocytes and macrophages. Recently several authors observed new morphologic findings in allergic contact dermatitis including infiltration and piecemeal degranulation of basophils, degranulation and replication of fixed tissue mast, cells, increased vascular permeability leading to dermal and epidermal edema, microvascular alterations affecting endothelial cells and pericytes, and activation of the clotting system, with deposition of fibrin in a characeristic inravascular pattern in the reticular dermis. And they proposed new hypotbesis that the vaoactive amines released by basophils and mast cells have the crucial role in eliciting allergic contact dermatitis. We studied the effect of antihistamine on allergic contact dermatitis caused by dinitrochlorobenzene(DNCB). with phenirarnine maleate,(Avil) 136.5mg/d in 15 young adult male persons. Clinically the degree of the skin reaction is decreased in 9 of 15 persons after tie ingestion of pheniramine maleate. and histopathologically the epidermal and dermal changes are ameliorated in accordance with the clinic':1 improvement. Our results indicate that the histamine also participates in the pathogenesis of allergic contact dermatitis.

8.Chest radiographic findings of scrub typhus: An analysis of 160 cases occurred in Ulsan area.

Scrub typhus (Tsutsugamushi disease)is an acute febrile systemic illness caused by Rickettsia tsutsugamushi that is transmitted to humans by the bite of larval-stage trombiculid mites (chiggers). The authors analyzed chest radiographic findings of scrub typhus in 160 patients in Ulsan area. One hundred and eight (67.5%) of160 patients showed abnormal findings which included lung lesions in 108 patients (67.5%), cardiomegaly in 37 patients (23.1%), lymphadenopathy in 25 patients (15.6%) and pleural effusion in 11 patients (6.9%). Among the lung lesions, interstitial patterns were seen in 107 patients (66.9%), mostly fine or medium reticulonodular, and air-space patterns in 14 patients(8.8%) and combined interstitial and air-space patterns in 13 patients (8.1%). Sixty-four patients(40%) had combined chest radiographic findings. The typical chest radiographic findings of scrub typhus would be helpful in evaluation of the causes of acute febrile illness that occur during late fall in the endemic area.

9.A Clinical Observation of Intussusception in Infants and Children: Comparing the results of barium enema with that of controlled air insufflation.

Intussusception which is the mot frequent cause of intestinal obstrucsion in childhood, is the invagination of a segment of the gastrointestinal tract into an adjacent segment. Since 1927, barium enema under fluoscopic guidance has been used widely as a therapeutic method. In 1953, Fiorito and Cuestas reported the use of controlled insufflation of air for diagnosis and treatment of intussusception. This method has several advantages than barium enema for diagnosis and treatment, with its simplisity, fast diffusion of air, clear-cut visualization of intussusception, no contraindications, and no interference with the normal physiology of bowel. We compared the results of treatment with barium enema in 117 cases with that of controlled air insu-fflation in 50 cases from January, 1976 to Octover, 1981. The results were summarized as follows 1) Age and sex distribution of the overall cases: 82% of the patients were under 1 year of age(the peak incidence was 4~8 months of age) with the predominence of male(2.2:1). 2) Growth percentile comparing with Korean standard data: 67% of the patient were above 50 porcentile. 3) Seasonal incidence: slight prevalence was noted during spring, but not significant. 4) Etiologic factors: 97% of the patient were idiopathic and only 3% of the cases had organic causes. 5) Cardinal symptoms and signs: abdominal pain and irritability 82%, vomiting 76%, bloody mucous stool 64%, and abdominal mass 50.3%. 6) The most common type of intussusception is ileo-colic. 7) Reduction rate: 68.4% for barium enema and 92% for controlled air insufflation(p<0.01). 8) Recurrence rate: 12.5% for barium enema and 8% for controlled air insufflation. 9) Recurrence interval of overall cases is the most frequently from one month to 6 months after reduction. 10) Perforation: One case with barium enema and 2 cases with controlled air insufflation. 11) Duration and pressure requiring to reduce by controlled air insufflation: 84.8% of cases reduced within 30 min. and the pressure required to reduce was about 61 mmHg to 100 mmHg in 73.9% of cases.

10.Mitral Atresia Associated with Corrected Transposition of Great Arteries.

A case of congenital absence of right atriovntricular (A-V) connection of heart is reported. Patient was 81 day old male infant with chief complaints of dyspnea and irritability. Clinical evaluation revealed increased pulmonary vascularity on chest x-ray, absence of right A-V connection, regurgitation of left A-V valve, right-sided ventricular hypoplasia and L-TGA. Clinical assessment of the case was tricupsid atresia with LTGA and mitral regurgitation. Pulmonary artery banlding and artial septectomy was performed. Parenteral alimentation through the femoral veins resulted in obstruction of IVC and subsequent multiorgan failure. The autopsy findings of the heart were corrected transposition of great arteries with associated anomallies of right-sided mitral atresia, Ebstein's malformation of left-sided tricuspid valve, ventricular septal defect, muscular hypertrophy of right-sided left ventricle with luminal obliteration. Long segment of IVC was completely occluded due to occlusive thrombi with fungal infection. Bilateral kidneys were infarcted and azygos vein was markedly engorged. Microscopic examination revealed candidal colonization in the inferior vena cava and septic embolzation in brain.
