1.An assessment of accuracy of half-guided implant surgery using implant surgical guide: A case report

Nowadays, dental implant is one of the widely used treatment options for edentulous patients. Recently, a method of improving the accuracy of implant surgery using an implant surgical guide has been introduced and widely used in order to accurately place the implant in a proper position. Full-guided and half-guided implant surgery can be distinguished according to the level of surgical guide application during the implant surgery. It is true that full-guided implant surgery exhibits higher accuracy, but half-guided implant surgery is often performed in a clinical situation due to the factors such as the circumstances of the operation. A partially edentulous patient who lost teeth due to tooth fracture and periodontal disease was treated using implant and fixed prosthesis. Half-guided implant surgery was performed using an implant surgical guide during implant surgery, the accuracy of implant placement was analyzed.

2.Occlusion design of edentulous implant prosthesis.

The occlusion design of dental implants is related to the growing popularity of dental implantology. This paper discusses the occlusion design of the edentulous implant prosthesis and the relationships between stress change and the alveolar bone and between the occlusal design and implantation complications. The horizontal relationship of condyle, the design of the canine-guided occlusion, and the similarities and differences between the dental implant and the natural teeth on biteforce response are mentioned.

4.Immediate loading of dental implants in partialful edentulous and edentulous jaws.

OBJECTIVETo investigate the clinical feasibility and technical characteristics of immediate loading and to access the short-term clinical results of treatment.
METHODSThis study included 3 completely edentulous patients and 24 patients who were partially edentulous from March 1999 to December 2003. 21 implants were immediately loaded and provided support for mandibular overdenture for 3 fully edentulous patients. 30 implants were placed in 21 partially edentulous patients and implants were immediate loaded in edentulous mandibular providing support for fixed provisional prosthesis within 6 months. Then, metal-ceramic crowns were completed. All patients were followed up by 1, 3, 6, 12 months and the patients were checked every 12 months.
RESULTSA total of 42 implants were loaded immediately. From March 1999 to December 2003, no implants were lost during follow-up (range 3 to 49 months, mean of 28 months). No infections, nerve or sinus damage or other sequelae occurred. No implants exhibited peri-implant radiolucencies. Moreover, immediate loading seems to increase the ossification of the alveolar bone around endosseous implants. Patients were satisfied with the treatment.
CONCLUSIONSThe data and the experience described in this study indicate that immediate loading with restorations using appropriate surgical and restorative techniques can predicate the completely edentulous and partially edentulous mandible in some cases. Further study is needed to determine the long-term result of immediately loaded implants.

5.Clinical application and follow-up evaluation of attachments in partial edentulous jaw prosthodontics.

OBJECTIVETo evaluate the outcomes of attachments prosthesis in patients with complicated intraoral situation.
METHODSThirty-eight patients provided with fixed-removable partial denture were followed up for 1 to 5 years. A total of 51 attachments restoration supported by 139 abutments were included.
RESULTSGood retention and stability of the restoration were achieved; masticatory function and esthetic effects were recovered satisfactorily.
CONCLUSIONSFixed-removable partial denture is an effective method for patients with complicated intraoral situation.

6.Effect of removable partial dentures restoration on oral health-related quality of life of patients with shortened dental arch.

OBJECTIVETo assess the effect of removable partial dentures (PRD) restoration on oral health-related quality of life (OHRQOL) of subjects with shortened dental arches (SDA) using the Chinese version of the Oral Health Impact Profile (OHIP-14).
METHODSConsecutive patients with shortened dental arches were recruited from the Department of Prosthodontics, Foshan Chancheng Hospital of Stomatology. The Chinese version of OHIP-14 was administered to each subject before treatment and after treatment. The subjective outcomes of removable partial dentures therapy on SDA were collected and the pre- and post-treatment scores of the Chinese version of OHIP-14 were compared.
RESULTSAfter treatment, significant decrease in patients' total score of OHRQOL [before: 9 (6, 12) ; after: 4 (2.25, 6)] was detected, as well as physical pain subscale, physical disability subscale and handicap subscale; and the score of OHRQOL in function limitation subscale increased [before: 0(0,0); after: 2(1, 2)].
CONCLUSIONSThe main impacts of shortened dental arch with intact anterior region affecting patients' OHRQOL are physical disability.From a quality-of-life perspective, patients with SDA can perceive benefits from RPD.

7.Clinical application of maxillary endossenous implant with edentulous ridge expansion technique.

OBJECTIVETo evaluate the application and the effect of edentulous ridge expansion(ERE) technique in maxillary endossenous implant placement.
METHODS49 patients with maxillary alveolar ridge atrophy received edentulous ridge expansion using condenser. In order to be similar to natural root, dental implants were selected and placed to tooth missed sites according to the requirements of aesthetics, function and dimension.
RESULTS49 patients with atrophied alveolar ridge received 86 implants. The labio-lingual width augmented from 3.3 to 5.4 mm and the alveolar ridge height from 2 to 7 mm 6 months after operation. The implants osseintergrated tightly with alveolar bone and second-step prosthesis was performed 6 months after implant placement.
CONCLUSIONThe edentulous ridge expansion technique can meet the requirements of aesthetics and function and is applicable to endossenous implant placement in maxilla. The method is simple and valuable to clinical application.

9.Technical complications rates and plaque control of fixed dental prostheses in patients treated for periodontal disease.

OBJECTIVETo compare the incidence of technical complications of implant-supported fixed dental prostheses in Chinese patients with a history of moderate or severe periodontitis and periodontally healthy patients(PHP) and analyze the effects of interproximal papillae patterns on food impaction and efficacy of plaque control.
METHODSA total of 103 partially edentulous patients treated with implant-supported fixed dental prostheses between December 2009 and December 2012 for a minimum 1-year follow-up period were recruited from Department of Periodontology, Peking University, School and Hospital of Stomatology. Based on the initial periodontal examination, the participants were divided into three groups: 30 PHP, 36 moderate periodontally compromised patients(mPCP) and 37 severe periodontally compromised patients(sPCP). Implant survival/loss, technical complications, plaque index, papilla index, food impaction and degree of proximal contact tightness of each patient were assessed around the implants at follow-up. According to the implant papilla index, the implants were divided into two groups: the "filling" group with the mesial and distal aspects with papilla index=3 and the "no filling" group with at least one aspect with papilla index<3. Data on implant survival, technical complications were analyzed. Comparisons of the incidence of technical complications were performed between the patients with different periodontal conditions with chi-square or Fisher's exact test. The influences of the interproximal papillae loss on food impaction and efficacy of plaque control were estimated with chi-square and Mann-Whitney U tests.
RESULTSThe total implant survival rate was 100%(162/162) for all three groups. Technical complications were as following: veneer fractures(1.9%, 3/162), abutment screw loosening(1.9%, 3/162), prosthetic screw loosening(3.1%, 5/162) and decementation(3.1%, 5/162) in all subjects. No implant/screw fracture was noted. The incidence of technical complications in sPCP, mPCP and PHP did not yield statistically significant differences(P>0.05). The proportion of the implant with the mesial and distal papilla index=3 in the sPCP was less than that in the PHP and mPCP. The interproximal papillae loss did not appear to affect the food impaction and the plaque index in all three groups(P>0.05). However, for the PHP, the accumulation of plaque at buccal aspect was more in the "no filling" group compared with the "filling" group (implant plaque index[M(Q)]: 1[1] vs 0[0]), and for the sPCP, the accumulation of plaque at lingual aspect was more in the "filling" group compared with the "no filling" group(implant plaque index[M(Q)]: 1[1] vs 0[1], (P<0.05).
CONCLUSIONSThe patients with a history of severe periodontitis did not exhibit more technical problems compared with the periodontally healthy patients. The interproximal papillae loss did not show a negative impact on the plaque control and food impaction. However, for the sPCP, changing the morphology and the position of the interproximal contact point to reduce the interdental black triangle may lead to accumulation of plaque at lingual aspect. More attention should be placed on the morphology design of prosthesis, but not the papillae filling up the interproximal space.

10.Clinical and radiographic outcomes of delayed implant placements after maxillary sinus floor augmentation with different grafting materials.

OBJECTIVETo evaluate the long-term clinical outcome of delayed implant placements after maxillary sinus floor augmentation with autologous bone or Bio-Oss grafting.
METHODSEighteen patients underwent maxillary sinus floor augmentation and delayed implant placements from January, 2002 to December, 2008. Bone grafting and sinus floor augmentation were performed in 21 sides of maxilla and 46 implants were placed 6 - 8 months later. Residual bone height was less than 4 mm. The cases were divided to 2 groups and different materials (autologous bone + Bio-Oss and Bio-Oss alone) were grafted relatively. The bone resorption was assessed by panoramic X-ray and the stability of the implant was reviewed postoperatively.
RESULTSThe average follow-up time was 54 months. Only one implant was lost and the implant survival rate was 98%. X-ray showed that the bone resorption was observed in both groups. The absorption ratio of autologous bone + Bio-Oss group was 18.65% and that of the Bio-oss group was 1.93%. The difference was significantly different.
CONCLUSIONSThe result of maxillary sinus floor augmentation with bone grafting was predictable. More bone absorption occurred in the Bio-Oss than in autologous bone + Bio-Oss.
