2.Iliac Vein Compression Syndrome: 4 Case

Iliac vein compression syndrome is a rare clinical condition in which left common iliac vein is anatomically compressed between right common iliac artery and the fifth lumbar vertebrae or pelvic brim. Patients usually have marked edema of the left leg. Although the actual incidence is unknown, this syndrome most affects women in the second to fourth decade of life. We report 4 cases of iliac compression syndrome treated by surgical intervention during the last fifteen-year period. Among them, two women have suffered from chronic intermittent swelling and pain of left leg, and two men had acute or subacute symptoms. The fibrous web in the entrance of left iliac vein into vena cava existed in one case. Including this one, two were treated by transposition of iliac artery or vein with thrombectomy. One was decompressed the arterial compression by mobilization or adhesiolysis of the surrounding vessles with thrombectomy. To the last one in which the distal common iliac vein was compressed by tortous dilated left internal iliac artery, transsection and ligation of the internal iliac artery was performed. All patients have experienced improved left leg condition after operation. except the last one in which suffered from thrombosis of left popliteal vein after the iliac decompression.

3.The Property of p53 Gene in Cell Lines of Squamous Cell Carcinoma.

The mutation of p53 tumor suppressor gene is the most common genetic variation of primary malignant tumors. The occurrence, progression and reaction for medical management of cancers can be different according to the characteristics of p53 gene, even if they are same kinds of malignant tumors. In this study, the property of p53 gene in 4 kinds of squamous cell carcinoma cell lines were investigated by using immunocytochemistry, PCR-SSCP, sequencing and Western blot methods. As a result, p53 mutation detected in 3 kinds of squamous cell carcinoma cell lines. Namely, it is found that T in codon 176 changed to A, and G in codon 281 changed to A in KUMA3 cell lines; CC in codon 241 changed to TT in KUMA4 cell lines; G in codon 266 changed to T in KUMA6 cell lines. In single nucleotide polymorphism of codon 72 of p53 gene, the genetic variations are Arg/Pro heterozygote in KUMA3 and KUMA4 cell lines; Arg/Arg homozygote in KUMA5 cell lines; Pro/Pro homozygote in KUMA6 cell lines. These results will provide useful data for p53 gene researches of various squamous cell carcinomas.

4.Effect of Nurses' Job Stress on Job Satisfaction: Mediating Effect of Head Nurses' Emotional Leadership Perceived by Nurses.

PURPOSE: The purpose of this study was to identify the mediating effect of head nurses' emotional leadership as perceived by nurses in the relationship between nurses' job stress and job satisfaction. METHODS: Study participants were 278 staff nurses currently working at two university hospitals in Busan, Korea. SPSS/WIN 21.0 program was used for data analysis to analyze descriptive statistics, t-test, correlation, and stepwise multiple regression. The significance level was set at p<.05. RESULTS: Nurses' job satisfaction had a significant correlation with job stress (r=-.31, p<.001) and emotional leadership of nurse managers as perceived by nurses (r=.53, p<.001). Also, head nurses' emotional leadership experienced by nurses had a partial mediating effect in the relationship between nurses' job stress and job satisfaction. CONCLUSION: The results of this study show that the nurse managers' emotional leadership has an important influence on nurses' job stress and job satisfaction. Therefore, head nurses' emotional leadership is very important for effective human resource management and the hospital should offer education and training to booster head nurses' leadership by developing emotional intelligence to promote staff job satisfaction.

5.The Results of Hyperfractionated Radiation Therapy Combined with Taxol for Paraaortic Node Recurrence in Cervix Cancer.

PURPOSE: The aim of this study was to investigate treatment results, toxicity and efficacy of hyperfractionated radiation therapy combined with paclitaxel for paraaortic node recurrence in cervix cancer. MATERIALS AND METHODS: Between September 1997 to March 1999, 12 patients with paraaortic node recurrence in cervix cancer who previously received radical or postoperative radiotherapy were treated with hyperfractionated radiation therapy combined with paclitaxel. Of these, 2 patients who irradiated less than 30 Gy were excluded, 10 patients were eligible for this study. Median age was 5 1 years. Initial FlGO stage was 1 stage IB1, 2 stage IIA, 7 stage IIB. For initial treatment, 7 patients received radical radiotherapy and 3 received postoperative radiotherapy. The paraaortic field encompassed the gross recur rent disease with superior margin at T 12, and inferior margin was between L5 and S 1 with gap for previously pelvic radiation field. The radiation field was initially anterior and posterior opposed field followed by both lateral field. The daily dose was 1.2 Gy, twice daily fractions, and total radiotherapy dose was between 50.4 and 60 Gy(median, 58.8 Gy). Concurrent chemotherapy was done with paclitaxel as a radiosensitizer. Dose range was from 20 mg/m to 30 mg/m (median, 25 mg/m'), and cycle of chemotherapy was from 3 to 6 (median, 4.5 cycle). Follow-up period ranged from 3 to 21 months. RESULTS: Interval between initial diagnosis and paraaortic node recurrence was range from 2 to 63 months (median, 8 months). The 1 year overall survival rate and median survival were 75% and 9.5 months, respectively. The 1 year disease free survival rate and median disease free survival were 30% and 3 7 months, respectively. At 1 month after treatment, 4 (40%) achieved a complete response and 6 (63%) experienced a partial response and all patients showed response above the partial response. There was distant metastasis in 6 patients and pelvic node recurrence in 2 patients after paraaortic node irradialion. There was 2 patients with grade 3 to 4 leukopenia and 8 patients with grade 1 to 2 nausea/ vom ting which was usually tolerable with antiemetic drug. There was no chronic complication in abdomen and pelvis during follow up period. CONCLUSION: Hyperfractionated radiation therapy combined with paclitaxel as a radiosensitizer showed high response rate and few complication rate in paraaortic node recurrence in cervix cancer. Therefore, present results suggest that hyperfractionated radiation therapy combined with paclitaxel chemotherapy can be used as optimal treatment modality in this patients.

6.Tuberculosis of the Puboischium: Report of Five Cases

Tuberculosis of the puboischium is an uncommon entity. The two constant features in tubereulosis of the pubis are limping and abscess formation. This disesse should be differentiated from other diseases such as osteitis pubis, suppurative osteomyelitis, postpartum synphysiolysis, and adolescent osteochondritis. Kirker advocated that wide excision of the lesion for the treatment of this disease is the best method. However, Nicholson(1953) reported that the lesion has a good prognosis snd responds well to simple curettage. Since the first reports by Thilesen(1855) and by Heinnies(1888), there were paucity of reports. Also in Korea, there have been no report about this disease. The five patients who were treated at the Ksng-Nsm St. Marys Hospitsl between 1982 and 1987 were reported. The results were as follows ; 1. Four patients were boys and one was a female. 2. All cases had clinically abscess, tenderness at the lesion, and limp. 3. Lesions in four cases responded well to simple curettage and18 months of chemotherapy. However, in one case fusion of the symphysis using a plate was performed because of tuberculosis-induced instability of the sacroiliac joint. 4. In a child, destroyed symphysis pubis was well restituted after treatment. Through the cases it is concluded that simple curettage and anti-tuberculous chemotherapy are good treatment modality for tuberculosis of the puboischium.

7.Multiple diagnostic approaches to palpable breast mass

The combination of the various diagnostic methods of palpable breast mass has improved the diagnosticaccuracy. From Sep.1983 to Aug. 1985 pathologically proven 85 patients with palpable breast masses examined withX-ray mammography, ultrasonography, penumomammography and aspiration cytology at Korea Cancer Center Hospital wereanalyzed. The diagnostic accuracies of each methods were 77.6% of mammogram, 74.1% of ultrasonomgram, 90.5% ofpenumommaogram and 92.4% of aspiration cytology. Pneumommamograms was accomplished without difficulty orcomplication and depicted more clearly delineated mass with various pathognomonic findings; air-ductal pattern infibroadenoma(90.4%) and cystosarcoma phylloides(100%), air-halo in fibrocystic disease(14.2%), fibroadenoma(100%),cystosarcoma phylloides (100%), air-cystogram in cystic type of fibrocystic disease(100%) and vaculoar pattern orirregular air collection without retained peripheral gas in carcinoma.

10.Physical Fitness, Leisure Time Physical Activity, and Serum Lipid Levels in Middle-Aged Male Workers .

This is a cross-sectional study to evaluate the relationships between physical fitness, leisure time physical activity, and serum lipid levels in middle-aged male workers. Physical fitness was measured by a step test score, and leisure time physical activity was self-reported on a questionnaire. Serum total cholesterol was negatively related to physical fitness(r=-0.27), and positively to obesity index(r=0.27). But leisure time physical activity was related to total cholesterol negatively(r.-0.20) only in subjects whose total cholesterol levels were above 170mg/dl. High density lipoprotein(HDL) cholesterol was positively related to physical faintness(r=0.15), negatively to obesity index(r=-0.22), and positively to weekly alcohol consumption(r=0.14). Total cholesterol/HDL cholesterol ratio was related to physical fitness(r=-0.23), obesity index(r=0.32), total cigarette index (r=0.13), weekly alcohol consumption(r=-0.13), and vegetable preference(r=0.13) physical fitness was also related to leisure time physical activity(r=0.19) and obesity index(r=-0.18). In multiple linear regression models, physical fitness(beta= -0.23) and obesity index(beta=0.18) were significantly associated with total cholesterol, obesity index(beta=-0.25) with HDL cholesterol, and obesity index(beta=0.30), physical fitness(beta= -0.16) and vegetable preference (beta=0.14) with total cholesterol HDL cholesterol ratio. In conclusion, as physical fitness has a stronger relationship with serum lipid levels than leisure time physical activity, and the association between physical fitness and leisure time physical activity is modest, physical fitness should be added as an important variable in addition to activity in future epidemiologic studies.
