1.An Immunohistochemical Study on Expression of Proliferating Cell Nuclear Antigen, Transglutaminase, and Glutathione-S -Transferase pi on DMBA induced Oral Carcinogenesis in Hamsters.

The purpose of this study was to evaluate the relationship between proliferation and cell death during oral carcinogenesis. Syrian golden hamsters which were 3 month old and 90-120 gm-weight were used in this study. The 9, 10-dimethyl-1, 2-benzanthracene(DMBA) in a 0.5% solution in mineral oil was painted on the buccal pouch mucosa 3 times per week in the experimental group during 20 weeks. Control group was painted with mineral oil. In each control and experimental group of 6,8, 10, 12, 14, 16, 18,20 weeks, specimen were sectioned for immunohistochemical study with anti-Proliferating Cell Nuclear Antigen(PCNA), anti-transglutaminase transglutaminase and GST-pi were obtained by counting the positive cells to those antibodies. The following results were obtained. 1. Histopathologically, finding of epithelial dysplasia of the 6 and 8 weeks experimental group and carcinoma in situ in the 12 weeks and squamous cell carcinoma in those of the 14 weeks were seen. 2. PCNA positive cells were mainly mild expressed in the basal cell layer of normal oral mucosa, increased moderately, after 6 weeks. In suprabasal cell layer, control group is negative but retained moderately between 6 weeks and 14 weeks, and decreased after 16 weeks. In spinous cell layer, restricted only between 12 weeks and 16 weeks, other period is mild or negative. 3. PCNA index of experimental group revealed the increased peak in 6 weeks and 20 weeks than control group, and retained between 12 weeks and 18 weeks. All experimental group expressed higher PCNA index than control group(p.<0.05). 4. Tranglutaminase expression was localized in outer and suprabasal layers on control group, but after 6 weeks, expression site moved spinous & suprabasal cell layers, and after 8 weeks, expression is spreaded to basal cell layer, and this patters retained to 20 weeks. Transglutaminase expression of experimental group was higher than control group after 8 weeks. 5. The positive staining of detoxifying agent, G1utathione S-Transferase(GST)pi of experimental group was radually increased from 6 weeks. After 10 weeks, all layer of experimental group was seen positive reaction. The strong positive staining in center of tumor and weak positive staining in periphery of tumor were seen at the stage of squamous cell carcimoma in 14 weeks. According to the results, we should suggest that the more increased proliferation of tumor cell, the more increased expression of PCNA, transglutaminase and GST-pi as a detoxifying agent during carcinogenesis by induced DMBA were seen.

4.Sarcomatoid Renal Cell Carcinoma; Special Reference to its Distinction from Carcinosarcoma.

Sarcomatoid renal cell carcinoma is an uncommon tumor that has to be distinguished from renal carcinosarcoma. We have described three cases of sarcomatoid renal cell carcinoma showing different clinical and light microscopic features. An ultrastructural study of the tumor cells from the sarcomatoid area revealed frequent desmosomal junction, confirming the epithelial nature of the neoplasm. All three cases showed an aggressive clinical course and tended to invade adjacent organs or tissues. We believe that an histological and immunohistochemical examination in conjunction with an electron microscopic examination are necessary to diagnose sarcomatoid renal cell carcinoma.

5.Antimicrobial Susceptibilities and Serotypes of Group B Streptococci Isolated from Pregnant Women.

BACKGROUND: Recent data suggest that the colonization rate of group B streptococci(GBS) in pregnant women and the incidence of neonatal infections by GBS is increasing trend in Korea, but the antimicrobial susceptibilities and serotypes in pregnant women have not been reported in Korea. So, we studied to define the antimicrobial susceptibility patterns and frequency of serotypes of GBS in pregnant women. METHODS: The susceptibility and serotyping of 60 GBS isolates from 27 pregnant women and four isolates from their two neonates were tested by an agar dilution method and agglutination test, respectively. The typing sera used in this study were Ia, Ib, II, III, IV, and V. RESULTS: Minimal inhibitory concentration range of 60 GBS from pregnant women were penicillin G 0.015-0.12 microgram/ml, vancomycin 0.5-2 microgram/ml, clindamycin 0.015-4.0 microgram/ml, chloramphenicol 2-4 microgram/ml, erythromycin 0.015-2 microgram/ml, tetracycline 0.5-256 microgram/ml, cephalothin 0.12-0.25 microgram/ml, ceftriaxone 0.03-0.12 microgram/ml, respectively. The resistance rate of GBS were 6.7% to clindamycin, 0% to erythromycin, and 98.3% to tetracycline. Most of GBS serotypes from pregnant women in decreasing order were Ib(48.3%), Ia(24.1%), III(20.7%). CONCLUSION: All GBS strains isolated from pregnant women are highly susceptible to commonly used antimicrobial agents with the exception of tetracycline. The low prevalence of severe neonatal GBS infections in Korea is not due to the absence of serotype III, but probably due to a low genital carriage rate of GBS by pregnant women.

6.Placental Site Trophoblastic Tumor(PSTT) with Vaginal Metastasis: A case report.

PSTT has been established as a separate entity distinct from choriocarcinoma since 1983, because of its different morphological features, biologic behavior and response to chemotherapy. Most cases of PSTT have indolent clinical courses, but there have been several reports showing malignant behavior resulted in death of the patient. We report a case of PSTT showing vaginal metastatis. In biopsy specimen, it was difficult to make differential diagnosis from squamous cell carcinoma of uterine cervix due to uniform and mononuclear cytologic characteristics. Immunohistochemical stains for hPL and hCG were not typical for either PSTT or choriocarcinoma. We suggest that poorly differentiated PSTT may synthesize the lesser amount of hPL than typical PSTT and it may behave more aggressively.

7.Self-efficacy and Compliance in Patients with Chronic Heart Failure: The Effect of a Self-management Program using Decision Tree.

PURPOSE: The purpose of this study was to identify effects of a self-management program on self-efficacy and compliance in patients with CHF. Hypothesis: 1) Patients with CHF who are provided with a self-management program will show higher self-efficacy scores than a control group. 2) Patients who are provided with a self-management program will show higher compliance scores than a control group. METHOD: This study was designed as a nonequivalent non-synchronized pre-posttest control group. There were eight patients in the experimental group, and twelve in the control group. According to NYHA classification, all patients belonged under the classesl to lV. Data were collected using the instruments developed by the researchers. Data were analyzed using descriptive statistics and Mann Whitney U test. RESULT: There were significant differences in self-efficacy scores and compliance scores between the experimental and control group. CONCLUSION: By utilizing the program, patients were able to monitor their symptoms routinely, comply with therapeutic regimen, and feel better able to positively influence their disease. Therefore, better compliance means fewer readmissions of patients with CHF.

8.Radiological features of lower respiratory infection by respiratory syncytial virus in infants and young children.

Respiratory syncytial virus is the most common cause of lower respiratory infection (bronchiolitis and pneumonia) of infancy and early childhood. We analyzed clinical and radiological features of 76 patients with lower respiratory infections by respiratory syncytial virus, which were diagnosed by indirect immunofluorescent test or culture of nasal aspirate in Hep-2 cell monolayer, during the period of January-December, 1991. There were peaks of incidences in March-May and November-December, accounting for 87% of eases. Sixty-two cases (82%) were under 1 year of age. Fifty cases(66%) had underlying diseases. Major radiographical findings were overaeration (83%), parahilar peribronchial infiltrates(67%), segmental or subsegmental atelectasis(32%), and segmental or lobar consolidation(16%). In 15 cases(20%), overaeration was the only radiological findings. There was no evidence of pleural effusion or hilar lymph node enlargement in all cases. By considerig clinical features(symptoms, age. Underlying diseases, epidemic seasons) in addition to the radiological findings, radiologists would be familiar with lower respiratory infection by respiratory syncytial virus. Air space consolidation, which is generally though to represent bacterial pneumonia, is also observed not infrequently in respiratory syncytial virus infections.

9.A Case of Prenatal Diagnosis of Congenital Fetal Goiter in Hyperthyroidism Mother.

We report a case of prenatal diagnosis of fetal congenital goiter at 31weeks of gestation by ultrasonogram and fetal hypothyroidism confirmed at birth as thyroid function test by umbilical cord blood sampling. Maternal Graves' disease and the drugs used to treat hyperthyroidism in pregnant women can affect the fetus, causing hyperthyroidism or hypothyroidism and goiter. Fetal hypothyroidism may be caused by transplacental passage of either maternal thyrotropic-binding inhibitory immunoglobulin(TBII) antibodies or maternal treatment with propylthiouracil(PTU). Untreated fetal hypothyroidism may result in mental retardation, perceptual-motor, visual-spatial, and language developmental problems. In this article fetal thyroid function was not assessed by cordocentesis, but fetal congenital goiter was detected ultrasound. Ultrasound should be used to detect fetal goiter from 20 weeks onward. Fetal goiter should resolve when maternal PTU treatment is decreased. We have diagnosed fetal hypothyroidism in utero by ultrasonography. Significance of in utero management of fetal hypothyroidism is discussed.

10.Species Identification of Coagulase Negative Staphylococci by Polymerase Chain Reaction-Restriction Fragment Length Polymorphism of Heat Shock Protein 60 Gene.

BACKGROUND: An accurate and rapid method for specise identification of coagulase negative staphylococci(CNS) has been increasingly necessary for the clinical significance and planning the management of patients with staphylococcal infections. Recently, it has been reported that there is a highly conserved area on their 60KDa heat shock protein(HSP60) gene sequences between the interspecies of CNS and it can be amplified by a set of universal degenerate primer. This led us our attention to focus on whether the PCR-based RFLP method using Mse / restriction enzyme could be a useful tool for the species identification of CNS. METHODS: In the present study, we performed PCR-based RFLP analysis using a set of degenerate primers covering HSP60 and Mse / restriction enzyme on the reference strains and 25 clinical isolates(10 of S. epidermidis, 10 of S. haemolyticus, 4 of S. lugdunensis and 1 of S. warneri) which were previously identified by the API-STAPH, Vitek GPI card and/or with conventional biochemical test. RESULT: All the seven reference strains revealed that each strain has a distinct electrophoresed band patterns with combination of different number (up to 8) and size of fragments. And these distinct band patterns showed remarkable concordance with the seven reference strains and 25 clinical isolates. CONCLUSION: These result strongly suggest that the PCR-RFLP method using degenerate primers covering the HSP60 gene and Mse / digestion enzyme offer a convenient and accurate tool for species-specific identification of CNS.
