1.Clinical and angiographic observation of renal carcinoma

The authors analyzed 5 cases of renal carcinomas confirmed by operation and pathologically, which wereselected from all the cases of selective angiography performed recently at the department of radiology, Busan National University Hospital. The result obtained were as follows; 1. Among the 5 cases of malignant tumors of thekidney, 4 cases were renal cell carcinomas, and 1 case was transitional cell carcinoma. 2. In the sexdistribution, male to female was 3:2. And all cases were over 51 years of age. 3. All the 5 cases were discoveredat the left kidney; 1 case, in upper pole; 2 cases, in mid-portion; and 2 cases, in lower pole. 4. Clinicalmanifestations were hematuria in all 5 cases; flank pain in 4 cases. But no palpable mass was demonstrated in anycases. 5. The renal angiographic findings; Of 4 cases of renal cell carcinomas, 2 cases revealed markedhypervascularity, 1 case mild hypervascularity with multiple mulbery-like aneruysms, and 1 case avascularity withwell demarcated margin. 1 case of transitional cell carcinoma showed hypovascularity with perivascular pooling anddisplacement of the upper dorsal branch of renal artery.

2.A clinical and radiological observation of shoulder arthrography

The authors analyzed 23 cases of shoulder arthrography performed at Busan National University Hospital and Inje Medical College Paik Hospital for about 2.5 years from March 1981 to Sept. 1983, both clinically and radiologically. The results obtained were as follows; 1. Of the 23 cases, 20 cases (87.0%) were male and 3 cases(13.0%) were female, and the most prevalent age group was second decade. 2. Right shoulder arthrography was performed in 16 cases (69.6%), and left in 7 cases (30.4%). 3. The number of cases which had had certain historyof trauma was 13(56.5%), and remainder of 10 cases (43.5%) had no history of trauma. 4. Frequent symptoms andsigns were pain in shoulder region in 16 cases (69.6%) and limitation of motion in 14 cases (60.9%). 5. On plainfilm findings, 18 cases(78.3%) were normal, and 5 cases (21.7%) were abnormal. 6. On shoulder arthrographicfindings, 16 cases (69.6%) were normal, and 7 cases (30.4%) were abnormal, Those abnormal cases were consisted of2 case (8.7%) of adhesive capsulitis, 2 cases (8.7%) of chronic shoulder dislocation, 1 case(4.3%) of rotator cuff tear, 1 case(4.3%) of loose bodies in joint cavity, and 1 case(4.3%) of rupture of biceps tendon sheath. 7. Among routine views of shoulder arthrography of normal cases, the external rotation view revealed axillary recess &biceps tendon more distinctly, and the internal rotaion view revealed subscapular bursa more clearly. 3.Validation of the Korean Version of the Prodromal Questionnaire-Brief Version in Non-Help-Seeking Individuals

OBJECTIVE:
In this study, we examined the reliability and validity of the Korean version of the Prodromal Questionnaire-Brief (PQ-B), a self-report screening instrument for clinical high risk for psychosis (CHR).
METHODS:
A total of 3,400 middle, high school and college students participated, and 261 subjects with a PQ-B total score ≥3 completed both the PQ-B and the Structured Interview for Prodromal Syndromes (SIPS). Receiver operating characteristic (ROC) curves were used to examine the psychometric properties of the PQ-B.
RESULTS:
A cut-off of 7 for the PQ-B total score and a cut-off of 22 for the PQ-B distress score showed the best balance of sensitivity (72.2%, 77.8%) and specificity (71.4%, 75.0%).
CONCLUSION
The Korean version of the PQ-B showed good reliability and validity for predicting prodromal risk symptoms in the community population.
4.Effect of Reconstruction-Method after a Radical Subtotal Gastrectomy on Gallbladder Motility.

PURPOSE: An increased incidence of gallstones has been widely reported in patients who had undergone a gastrectomy. But, there has been little information about the pathophysiologic mechanism for the occurrence of gallstones after gastric surgery. Many investigators have considered the cause to be decreased gallbladder motility due to vagal denervation. We observed higher increase in the incidence of gallbladder stones in patients who underwent a Billroth II gastrojejunostomy than in those who underwent a Billroth I gastrojejunostomy after radical subtotal gastrectomy. METHODS: We prospectively studied the change in the motility of the gallbladder after a gastrectomy. The gallbladder ejection fraction (EF) was compared pre- and postoperatively by using gallbladder scintigraphy with 2,6-diisopropyl-iminodiacetic acid (DISIDA). RESULTS: Twenty patients underwent a Billroth(B) I anastomosis and twelve patients underwent a B-II anastmosis after the gastrectomy. The means of the EF of the B-I group were 75.9%, 46.4%, 68.1% at the preoperative period, at 1 month and 6 months after the gastrectomy respectively. Those of B-II group were 78.2%, 45.3%, 56.3%, respectively. There was no statistically significant difference of EF between the two groups at postoperative 1 month, but the difference at postoperative 6months was statistically significant. The differences of EF between preoperative period and postoperative 1 month, 6 months were significant in the B-I group andthe B-II group. CONCLUSION: At 6 months postoperative period, there was more recovered gallbladder motility after a gastrectomy with B-I anastomosis than with B-II anastomosis.

7.Traction Induced Vertical Displacement of Odontoid due to Type III Odontoid Fracture with Unrecognized Ligamentous Injury: A Case Report.

Dens fractures are a common traumatic cervical spine injury. Among them, a type III fracture is the second common fracture. Although there are several treatment options, it has been accepted that type III fracture is usually healed by non-surgical method. After adequate reduction with traction, subsequent external immobilization has been associated with successful union rates. However, in the review of literatures, there are some cases with neurological deterioration after application of skull traction. So, the authors report a case of type III dens fracture with initially unrecognized ligamentous injury in which vertical dissociation and quadriplegia occurred after only five-pound Gardner-Well tongs traction. And also, the authors raise awareness of this potentially injury.

8.Preliminary Experiences of the Combined Midline-Splitting French Door Laminoplasty with Polyether Ether Ketone (PEEK) Plate for Cervical Spondylosis and OPLL.

OBJECTIVE: The purpose of this study was to evaluate the safety and efficacy of cervical midline-splitting French-door laminoplasty with a polyether ether ketone (PEEK) plate. The authors retrospectively analyzed the results of patients with cervical laminoplasty miniplate (MAXPACER(R)) without bone grafts in multilevel cervical stenosis. METHODS: Fifteen patients (13 males and 2 females, mean age 50.0 years (range 35-72)) with multilevel cervical stenosis (ossification of the posterior longitudinal ligament and cervical spondylotic myelopathy) underwent a combined surgery of midline-splitting French-door laminoplasty with or without mini plate. All 15 patients were followed for at least 12 months (mean follow-up 13.3 months) after surgery, and a retrospective review of the clinical, radiological and surgical data was conducted. RESULTS: The radiographic results showed a significant increase over the postoperative period in anterior-posterior diameter (9.4+/-2.2 cm to 16.2+/-1.1 cm), open angles in cervical lamina (46.5+/-16.0degrees to 77.2+/-13.1degrees), and sectional volume of cervical central canal (100.5+/-0.7 cm2 to 146.5+/-4.9 cm2) (p<0.001). The sagittal alignment of the cervical spine was well preserved (31.7+/-10.0degrees to 31.2+/-7.6degrees, p=0.877) during the follow-up period. The clinical results were successful, and there were no significant intraoperative complications except for screw displacement in two cases. The mini plate constructs did not fail during the 12 month follow-up period, and the decompression was maintained. CONCLUSION: Despite the small cohort and short follow-up duration, the present study demonstrated that combined cervical expansive laminoplasty using the mini plate is an effective treatment for multilevel cervical stenosis.

9.A case of renal cell carcinoma with mediastinal lymph node metastasis diagnosed by EBUS-TBNA.

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a new method of sampling mediastinal lymph nodes to aid in diagnosing lymphadenopathy and in staging metastatic cancers. This paper describes a case of renal cell carcinoma with mediastinal and hilar lymph nodes metastasis that was diagnosed using EBUS-TBNA. A 17-year-old woman who had mediastinal lymphadenopathy and a right renal mass underwent imaging studies. The results of a first EBUS-TBNA suggested malignancy, but the type of tumor and exact site of the primary tumor were uncertain. Therefore, we repeated EBUS-TBNA with a lower pressure on the vacuum syringe. We successfully diagnosed the mediastinal lymph node metastasis from renal cell carcinoma.

10.A Case of Leiomyoblastoma of Stomach.

In 1960 Martin and his associates described another type of tumor of the stomach derived from the smooth muscle, which they named myoid tumor in a study of six patients. In 1962 Stout applied the term "lelomyoblastoma" to this interesting entity in a report of a collected series of 69 patients. Tumor cells are round or polygonal epithelioid cells, which contain perinuclear vacuole or clear zone. A 58 year-old female was admitted to this hospital because of epigastric discomfort for three months. At Upper gastrointestinal series and gastrofiberscope, a round hemispherical submucosal tumor was noted at posterior wall near the lesser curvature of gastric antrum. After subtotal gastrectomy, the submucosal tumor was diagnosed as leiomyoblastoma. Because leiomyoblastoma are rare and easily misdiagnosed as leiomyoma or leiomyosarcoma, careful histologic study is recommended.
