1.The pathogenic mechanisms of motor weakness following aneurysmal subarachnoid hemorrhage: A review

Motor weakness is one of the neurological complication that can occur after aneurysmal subarachnoid
hemorrhage (SAH); incidence of motor weakness of 14~29% has been reported. Detailed information
on the pathogenic mechanism of motor weakness is essential for brain rehabilitation because it enables
estimation of the severity of injury, establishment of scientific rehabilitative strategies, and prediction
of motor outcomes by clinicians. However, the exact pathogenic mechanisms of motor weakness
following aneurysmal SAH have not been clearly elucidated. In this article, 14 previous studies on
pathogenic mechanisms in patients with aneurysmal SAH were reviewed according to the location of
the lesion (cerebral cortex, brainstem, spinal cord, and peripheral nerve). The following pathogenic
mechanisms have been suggested: vasospasm, cerebral ischemia, hydrocephalus, compression of
cerebral cortex, neural injury, spinal cord infarction, and radiculo-neuropathy. Considering the high
incidence of aneurysmal SAH and motor weakness following aneurysmal SAH, we believe that the
pathogenic mechanisms of motor weakness have been relatively understudied. More effort should be
taken to investigate this important topic.

2.The Facet Tropism of Lumbar in Young Adults.

STUDY DESIGN: The purpose of this study was to determine whether facet joint orientation and/or tropism influences the prevalence of disc degeneration in MRI in young patients. OBJECTIVE: 41 patients with disc degeneration disease including HNP in MRI were assesed on facet joint angles and tropism between 18 and 30 years old. SUMMARY OF BACKGROUD DATA: The patients who had back pain had association between disc degeneration and facet tropism in adults. But some authors have suggested that facet tropism has no clinical relevance. Physiologic aging and premature disc degeneration induce a gradual loss of water and proteoglycan from nucleus pulposus. METHODS: We reviewed the 41 MRI of lumbar spine with disc degeneration and measured facet joint angles on the mid-disc cut, which was parallel to inferior vertebral end-plate of superior vertebra. In simple X-ray, we reviewed relation between the transitional vertebrae and disc degeneration. It was analyzed statistically the facet joint angles and facet tropism in each levels. RESULTS: The mean facet angles was more increased in upper vertebral facet joints but statistically nonspecific. The facet tropism was not significant in all lumbar levels and the effects of transitional vertebrae on disc degeneration were ignorable and unclear. CONCLUSIONS: No statistical relationship was found among the magnitude of the angle and facet tropism and the presence of disc degeneration at any levels in young patients.

3.Comparison of NM23-H1/NDP Kinase and CD44v Expression in Human Skin Cancer : An Immunohistochemical Study.

Two isotypes of nm23 gene have been reported as multifunctional genes as well as CD44 gene. In tumor, both of genes, one isotype of human nm23, nm23-H1 and splice variants of surface glycoprotein CD44[CD44 v8-10], are correlated with tumor growth and metatastic potential[Keim et al., 1992 ; Dall et al., 1995]. However, the correlation of expression between these genes in tumor was not reported. In this immunohistochemical study on skin cancers, basal cell carcinoma, squamous cell carcinoma, and malignant melanoma, we intended to clarify the differences of expression on the basis of origins of skin tumors, basal cell, prickle cell, melanin producing cell, and compare the alterations of expressions between two genes in each tumor, respectively. The conclusion of this comparison is that relative parallel alteration in expressions between nm23-H1/NDP kinase and CD44 v8-10 was observed in basal cell carcinoma and malignant melanoma with inverse relation in differentiation. In squamous carcinoma, the expressions of two genes were much associated with differentiation. On the periphery of tumor, very low level of nm23-H1 protein and high level of CD44 v8-10 protein were detected.

4.A Case Dermal Melanocyte Hamartoma.

A 26-year-old man presented with a bluish speckled patch on the dorsum of the left hand. It had apperaed at birth as a bluish spot. Since adolescence the lesion extended and at the time of visiting hospital it covered the most part of the dorsum of the left hand and even some palmar aspect. The histopathological and ultrastructural examination revealed numerous dermal melanocytes, mainly in the upper dermis. Clinically and pathologically, the patient was diagnosed as having dermal melanocyte hamartoma, a distinct type of dermal melanocytosis.

5.Carcinosarcoma in Recurrent Lesion of Serous Cystadenocarcinoma of the Ovary: A case report.

Neoplasms showing both carcinomatous and sarcomatous features are well established, and have been reported in practically every organ where carcinoma can occur. But the difference in terminology and difficulties in histopathologic interpretation have hampered adequate understanding of these neoplasms. We report a case of carcinosarcoma in the recurrent form of serous cystadenocarcinoma of the ovary. The patient was a 64-year-old female who underwent total hysterectomy and bilateral salpingoophorectomy three years ago, due to perforated papillary serous cysta denocarcinoma of the right ovary. Recurrent pelvic masses contained both carcinomatous and sarcomatous components. Morphological transition between carcinomatous and sarcomatous components, and epithelial characteristics in the sarcomatous component by immunohistochemistry were recognized. We postulate the histopathogenetic mechanism based on the phenotypic conversion of carcinoma into sarcoma in the carcinosarcoma.

6.Ovarian Mucinous Cystadenocarcinoma with Mural Nodules of Anaplastic Carcinoma: A case report.

A case of an ovarian mucinous cystadenocarcinoma with mural nodules of anaplastic carcinoma is reported. The patient was a 46-year-old female with a right ovarian cystic mass and underwent a total abdominal hysterectomy with bilateral salpingo-oophorectomy. The patient died of the disease in five months. Macroscopically, the right ovarian cyst wall contained numerous well-demarcated mural nodules. Histologically, the cysts were mucinous cystadenocarcinoma, and the nodules were composed of sarcoma-like spindle and polygonal cells with atypia and numerous mitoses. Spindle cells in the mural nodule showed epithelial characteristics by electron microscopy and immunohistochemistry. This case supports an epithelial origin of the mural nodule of anaplastic carcinoma in ovarian mucinous tumors.

10.A Clinical Study on Pathologic Fractures

Pathologic fracture occurs in a bone with preexisting stuctual weakness. The author reviewed 30 cases of pathologic fractures in which admission record and X-ray finding was well preserved among 53 cases admitted patients in the department of Orthopaedic Surgery, Seoul National Universty Hospital for 10 years from 1967. The results were as follows: 1.Age & Sex incidence showed even distribution. 2. The most frequent involved site was femur (40%) 3. The underlying etiology was as follows: 2 chronic osteomyelitis 1 homophilia 7 benign tumor 20 malignant tumor 4. metastatic tumor was the most frequent cause in 30 cases. 5. Treatment was as follows: Conservative treatment 4 cases Surgery 21 cases.
