1.Self-esteem and Social Support as a factor for Suicidal Tendency for College Students in Pusan.

BACKGROUND: Suicide is a self-destructive behavior that can occur throughout life. Recently the suicide rate has gradually increased in Korea. Suicide is the third most common cause of death in adolescents. The most important factOrs closely related to suicide were depression and hopelessness. This study aimed to analyze first, relationships between suicidal ideation, depression, hopelessness, social support and self-esteem, second, the differences between the demographic factors and suicidal ideation, depression, hopelessness, social support and self-esteem. METHODS: This study was surveyed with a questionaire to 220 students of colleges located in Pusan. The instruments were used in this study were Harters(1988) Self-Perception Profile for College Students, Harter and Robinsons(1988) Approval Support Scale for Adolescents, Becks(1961) Beck Depression Inventory, Becks(1974) Beck Hopelessness Scale and Reynolds(1987) Suicidal ideation Questionaire. RESULTS: Self-esteem had a relatively strong positive correlation with social support. Suicidal ideation had a strong positive correlation with depression. Depression had a negative correlation with self-esteem and social support. Suicidal ideation had a negative correlation with self-esteem and social support. CONCLUSIONS: According to this study, the major factors related to suicidal tendency of adolescents were depression, low self-esteem, and low social support. Then the strategy to improve self-esteem and to encourage social support should be implemented for adolescents to prevent suicide.

2.A Case of Purpura fulminans Caused by Streptococcus pneumoniae.

Purpura fulminans is a potentially disabling and life-threatening disorder characterized by acute onset of progressive cutaneous hemorrhage and necrosis on distal extremities, and disseminated intravascular coagulopathy. We experienced a case of purpura fulminans due to Streptococcus pneumoniae. A 42-year-old women presented with skin petechiae, ecchymosis and gangrene on distal extremities with laboratory evidence of DIC. The latex agglutination test with CSF was positive at Streptococcus pneumoniae. To our knowledge, this is the first report of purpura fulminans caused by Streptococcus pneumoniae in Korea.

3.The Effects of a Cognitive Behavior Program on Cognition, Depression, and Activities of Daily Living in Elderly with Cognitive Impairment.

PURPOSE: This study was conducted to investigate the effects of a cognitive behavior program on cognition, depression, and activities of daily living in elderly with Cognitive Impairment. METHOD: The research design was a nonequivalent control group pretest-posttest design. There were 21 subjects in the experimental group and 19 in the control group among 40 senior residents in a Hall for the elderly in the city of S. The subjects scores ranged from 15 to 23 on the MMSE-K(Korean Version of Mini-Mental State Examination) over age 65. The length of time for data collection and intervention was from Jun 26 to September 1, 2006. The cognitive behavior program consisted of 'Facing problem behavior', 'Searching for a coping skill', and 'Training in the coping skill'. It was applied to the experimental group twice a week, fifty minutes per session for six weeks. RESULT: Cognition(t=-4.232, p< .001) and IADL(t=-2.939, p< .01) in the experimental group were significantly higher than those of the control group. Depression in the experimental group was significantly less than the control group(t=3.870, p< .01). However, ADL in the experimental group was not significantly higher than the control group. CONCLUSION: These findings confirmed that a cognitive behavior program contributed to improving cognition and IADL, and to reducing depression in the elderly with Cognitive Impairment.

4.Diagnosis of x-linked ichthyosis and detection of its carriers with southern blot hybidization.

BACKGROUND: The skin changes of X-linked recessive ichthyosis are cnused by the deficiency of the enzyme steroid sulfatase, which usually results from deletions of this gene in Caucasian populations. OBJECTIVE AND MEHTODS: To disgnose X-linked recessive ichthyosis and detect its carrier, we have investigated distinctive gene deletion and measured gene dosage of steroid sulfatase gene by southern blot hybridization in Korean patients with X-linked recessive ichthyosis. RESULTS: Patients from 8 of 9 unrelated families exhibited deletions, if the steroid sulfatase gene. Of 6 families showing a family history compatible with X-linked recessive inheritance, One family exhibited a normal pattern of hybridization. All but one family showed deletion of steroid sulfatase gene. All three patients lacking a fami1y history of the disease exhibited gene deletions. The ratio of the steroid sulfatsse specific band density to the Factor VIII specific band density was measured in 8 obligate carriers using a laser densitometer. The average ratio exhibited by the car riers was less than half that of normal women. Conclusian: These results suggest that the X-linked recessive ichth osis patient and its carrier can also be diagnosed and detected by Southern blot hybridization of steroid sulfatase gene in Korea.

5.A study on osteoblast-like cell responses to surface-modified titanium.

STATEMENT OF PROBLEM: The success of implants depends on intimate and direct contact of implant material on bone tissue and on functional relationship with soft tissue contact. Creation and maintenance of osseointegration depend on the understanding of the tissue's healing, repairing, and remodeling capacity and these capacities rely on cellular behavior. Altering the surface properties can modify cellular responses such as cell adhesion, cell motility, bone deposition. Therefore, various implant surface treatment methods are being developed for the improved bone cell responses. PURPOSE: The purpose of this study was to evaluate the responses of osteoblast-like cells to surface- modified titanium. MATERIALS AND METHODS: The experiment was composed of four groups. Group 1 represented the electropolished surface. Group 2 surfaces were machined surface. Group 3 and Group 4 were anodized surfaces. Group 3 had low roughness and Group 4 had high roughness. Physicochemical properties and microstructures of the d iscs were examined and the responses of osteoblast-like cells to the discs were investigated. The microtopography was observed by SEM. The roughness was measured by three-dimension roughness measuring system. The microstructure was analyzed by XRD, AES. To evaluate cell responses to modified titanium surfaces, osteoblasts isolated from calvaria of neonatal rat were cultured. Cell count, morphology, total protein measurement and alkaline phosphatase activities of the cultures were examined. RESULTS AND CONCLUSION: The results were as follows 1. The four groups showed specific microtopography respectively. Anodized group showed grain structure with micropores. 2. Surface roughness values were, from the lowest to the highest, electropolished group, machined group, low roughness anodized group, and high roughness anodized group. 3. Highly roughened anodized group was found to have increased surface oxide thickness and surface crystallinity. 4. The morphology of cells, flattened or spherical, were different from ach other. In the electropolished group and machined group, the cells were almost flattened. In two anodized groups, some cells were spherical and other cells were flattened. And the 14 day culture cells of all of the groups were nearly flattened due to confluency. 5. The number of attached cells was highest in low roughness anodized group. And the machined group had significantly lower cell count than any other groups(P<.05). 6. Total protein contents showed no difference among groups. 7. The level of alkaline phosphatase activities was higher in the anodized groups than electropolished and machined groups(P<.05).

6.Two Cases of Pachyonychia Congenita.

We report herein two cases of pachyonychia congenita in 19-month-old and 7-year-old girls. In addition to severely deformed nails, both had white plaques on the tongue and keratotic papules on the elbows and knees. In the 19-month-old girl, her father showed deformity of the nails and severe hyperkeratosis of the palms and soles, The family history was normal in other case.

Microvascular tissue transfers have facilitated primary closure of various complex defects after radical ablation of head and neck cancers. From Oct 1991 to Feb 1992, we used forearm free flap in two patients and delto-pectoral flap in one patient who had preoperative irradiation for pharyngoesophageal reconstruction. The stricture and fistula formation were most troublesome complication in forearm free flap, so we designed as lazy S shape in distal flap margin to prevent circular contraction and longitudinal margin was deepithelialized (5 mm) and sutured double layer to withstand fistular formation and this can be considered useful in place of a free jejuna transfer.

8.Self-esteem, Parent-adolescent Communication, Friend's Sexual Attitude and Sexual Permissiveness in College Students.

PURPOSE: This study was done to identify factors that affect sexual permissiveness in college students. METHODS: A descriptive design was used with 380 college students who completed a questionnaire about self-esteem, parents-adolescent communication, friend's sexual attitude and sexual permissiveness. RESULTS: The sexual permissiveness showed significant differences for the variable: grade (F=0.51, p=.002), gender (t=7.28, p<.001), age (F=14.72, p<.001), religion (t=4.97, p<.001), residence type (F=4.75, p=.009), number of call with parents (F=3.56, p=.030), number of viewing pornographic video (F=36.05, p<.001), number of viewing pornographic magazine (F= 47.90, p<.001), status of dating (t=3.62, p<.001), number of sexual intercourse (F=14.51, p<.001), and experience of sexual intercourse (t=13.00, p<.001). There was a positive correlation between the friend's sexual attitude and sexual permissiveness (r=.64, p<.001). The variables such as friend's sexual attitude, experience of sexual intercourse, the number of watching pornographic video, educational system, religion, and the number of calls with parent explained 52% of the variance in sexual permissiveness. CONCLUSION: The results of this study indicate that it is necessary to develop sexual education program for college students considering their level of sexual permissiveness and their friend's sexual attitude.

9.Evaluation of family function in the inpatient of rehabilitation medicine department witn impairment of activity of daily living.

BACKGROUND: When one of family members must be treated, the others are required to get various forms of changes including modification of their proper roles in the family. Particularily when he or she can not perform his or her activities of daily living(ADL) owing to functional impairment by himself or herself, there seems to be greater dependence upon his or her family. Accordingly, we tried to give help in the treatment of patients through the comparative analysis concerning functional impairment in both groups on the assumption that there is difference of family function between two family groups which have a patient in family, or not. METHODS: This study was carried out to 83 inpatients in Sun Hospital in Dae Jeon and local rehabilitation clinics in Iksan from 1. July. 1995 to 31. Jun. 1996. We evaluated activites of daily living by using modified Barthel Index(MBI) and classified them into dependent group with 60 points or less(6roup I ), and independent group with more than 60 points(Group II). We also used Family APGAR score and FACES III to evaluate their family functions. RESULTS: MBI scores to evaluate ADL were 42.9+/-15.6 in Group I and 82.1+/-17.9 in Group II. Family APGAR scores were 6.93+/-2.52 in Group I and 7.24+/-17.9 in Group II but there was no significance between two groups. Types of family according to Family APGAR score were highly functional, morderate dysfunctional, and severly dysfunctional one in order of frequency in both groups, but there was no significance in their frequencies in both groups. Types of Family in view of cohesion was disengaged, separated, connected, and enmeshed one in order of frequency in Group I, and separated, disengaged, connected, and enmeshed one in order of frequency in Group II but there was no significance between two groups. Types of family in view of adaptability were rigid, chaotic structured,and flexible one in order of frequency in Group I, and were flexible, structured, rigid, and chaotic one in orders in Group II, which showed significance between two groups. Extremal types of family structure were large in numbers in Group I, but it didnt show significance. CONCLUSIONS: Assuming that there were large numbers of rigid and chaotic family in Group I, it is considered that tne family which has a patient with severely impaired function seems to have weaker adaptability to their family stresses than otherwise. Therefore, it is desirable that physicians who take charge of such patients provide continuous and comprehensive medical care for them including their family with greater concerns and through analysis and assessment of their family functions.
