2.A Clinical Study on the Diaphyseal Fractures Treated with Compression Plate

Most orthopedic surgeons were aimed at the rigid internal fixation on the surgical treatment of the fracture. And the compression plate was developed and regarded as an excellent rigid internal fixation device by its advantages of accurate anatomical reduction, early joint mobilization and shortening of the duration of the fracture healing, which are chief aim of A.S.I.F. method. During the period of January 1970 to December 1979, 67 cases of diaphyseal fractures of the long bones were treated with compression plate according to A.S.I.F. method at the Department of Orthopedic Surgery, College of Medicine, Seoul National University and results were obtained as follows: 1. The age of patient was ranged from 15 to 77, and the highest incidence was in the 3rd decade male group (31.4%). 2. Old fracture cases were slightly more than fresh cases, and the fresh femur fracture was the commonest. 3. The periods of the postop immobilization of fresh fractures were 3.7 weeks in humerun fracture, 6.3 weeks in forearm fractures, 8.7 weeks in femur fracture and 7.0 week in tibia fracture, and in old fractures about 4 weeks was longer than fresh fractures. 4. The bony union was accomplished in all cases between 8 week and 24 week. 5. The complication were 4 superficial infections, 4 deep infections, 1 delayed union and 9 joint stiffnesses. 6. Compression plate fixation is considered as an excellent method for the treatment of the fractures of the long bones.

3.A Case of Primary Adenocarcinoma of the Female Urethra.

We report a case of primary adenocarcinoma of the urethra in 62 years old female. Primary carcinoma of the female urethra is an uncommon malignancy that accounts for less than 0.02 per cent of all cancers occurring in women. Adenocarcinoma is rare in the literature. Whereas squamous cell carcinoma and transitional cell carcinoma, which comprise the majority of urethral malignancies, arise form the squamous or epithelial cells lining the urethra, adenocarcinoma arises form the periurethral glands.

4.Electromechsnical Dissociation during Exsanguinating Hemorrhage on Rabbits .

Cardiac electromechanical dissociation is a cause of cardiac arrest and this is characterized by regularly recurring QRS complexes and T waves- P waves may even be present. without an obtainable blood pressure and pulse. Such findings in man are almost invariably associated with a fatal outcome although one can observe this phenomenon in potentially treatable disorders. Recognition of these disorders on the basis of certain clinical and electrocardiographic findings may be of clinical importance. This experimental study on rabbits was performed to observe electromechanical dissociation (E.M.D.) and to recognize its clinical importance, in exsanguinating hemorrhage with the cutting of the right femoral artery. The results were as follows: 1) The incidence of E.M.D. was 100 percent. 2) In the all cases except two cardiac rhythm showed sinus rhythm but in two cases it was difficult to distinguish it from nodal rhythm. 3) Heart rate at the time of E.M.D showed a significant increase of 13,8 percent as compared with the control. 4) The voltage of the P wave and QRS complex at the time of E.M.D. showed a significant decreases of the former, 34.7%, and the latter, 30%, as compared with the control. and the QRS complex at the time of E.M.D showed severe axis deviation. 5) In seven cases in all, the voltage of the T wave at the time of E.M.D, showed a significant decrease of 61. 3 percent (one case showed T wave inversion) and the others showed. conversely a significant increase of 271 percent. 6) Regarding change of interval, the QRS interval showed no changes and P-R and Q-T intervals showed shortening of 14, 7% and 2.4%. 7) We consider that devices for heart sound, pulse or blood pressure monitoring should be: combined with ECG monitoring.

5.Arachnoid Cyst in Sylvian Fissure Presented with Bitemporal Hemianopsia.

The anterior middle fossa is the most common location of benign intracranial arachnoid cysts. In the adult, headache, temporal bulging, and mild proptosis are the usual presenting complaints, although seizures and contralateral weakness have been described. Bitemporal hemianopsia associated with this lesion has not been noted previously. Herein we describe the patient with bitemporal hemianopsia associated with sylvian fissure arachnoid cyst. Cystoperitoneal shunt was beneficial. The etiology, histology, and suggested therapy of other patient with arachnooid cyst are also discussed.

6.Doxycycline - Induced Esophageal Ulcers.

Esophageal ulcers induced by doxycycline is a rare complication. These patients usually complain of sudden onset of symptoms, ie acute substernal or chest pain and odynophagia without prior hietory of esophageal syraptoms. On esophagoscopic examination, there are upper or midesophageal ulcers, which heal after diseontinuation of the drug within 2 weeks. A history of ingestion of the doxycycline,with liquid jost before bedtime can be elicited. The exact eause of the xaucosal ulceration is not clear, but a direct irritant effeet on esophageal mucosa seems most likely. We report 5 cases of esophageal uleeration secondary to the ingestion of doxycydine. Esophagoscopy revealed esophageal ulcers in all patients and the patients hecame asymptomatic following stopping of tbe drugs and taking antacids.

7.Useful Life of Soda Lime in the Circle Absorber.

During general anesthesia, removal of exhaled carbon dioxide by rebreathing appliances is provided by chemical absorption and this carbon dioxide absorption during anesthesia would at first glance seem to be a simple problem. The problem, however, is far more complicated and much attention must be given to details of rebreathing appliances. Incomplete removal of carbon dioxide in circle absorber systems, which are commonly used today must be carefully monitored to prevent hypercapnea caused by accumulation of carbon dioxide in the breathing circuits. Although circle absorbers have been developed through clinical trials anesthesiologists have no reliable indication of the end point of useful life of the absorbent. This study was undertaken to investigate the useful life of the soda lime used in two type of canisters (type A: canister of anesthesia machine. Quantiflex, USA. 12.9cmX16.6cm, tbye B: canister of anesthesia machine, International 2. U.K., 13.1cmX16.9cm). the effect of flow rate on this useful life and the relations between indicator change and the endpoint of useful life of the soda lime. The control group used a as total flow rate of fresh gases at 2.0 liters per minute and the experimental group used a total flow rate of 4.0 liters per minute. The carbon dioxide concentration of 0.1% at the canister exit was employed as the end point of useful life of the absorbent and the exit carbon dioxide concentration were monitored continuously by intra-red carbon dioxide monitor apparatus(Datex Normocap CD-102, Pinland). Also the carbon dioxide concentration in the respiratory circuit of exhaling side was examined. The results were as follows: 1) In type A, the average useful life of soda lime was 1,885 minutes in the control group and 2,514 minutes in the experimental group. In type B, the average useful life of soda lime was 1,587 minutes in the control group and 1,980 minutes in the experimental group. 2) At the end point of useful life of the absorbent the level of the color change of the indicator of the absorbent in all types was above the three-fourth height of two chambered canister. 3) At the end point, end tidal CO2 concentration was 3.5 vol% in the control group 2.88 vol% in the experimental group. The data obtained from this observation indicated that the increase of total flow rate of gas and the type A canister prolonged the useful life of soda lime and that the double chambered canister could be used safely until the color change of the indicator reached the level of three fourth the height of the canister.

8.Anesthesia for Thymectomy in Myasthenis Gravis - A case report.

Anestheiologists are called upon to undertake, or to assist in the management of severe respiratory and circulatory emergencies that may occur in myasthenics or to administer anesthesia for thymectomy or other surgical procedures to be performed on myasthenic subjects. Consequently the well-trained anesthesiologists must be sufficinetly familiar with the diagnosis and treatment of myasthenia gravis to carry on the therapy. A case of anesthesia for thymectomy in myasthenia gravis was experience. During operation, 0.1% succinylcholine in dripp method was given for adequate muscle relaxation and no respiratory probelms were encountered postoperatively.

9.A case report of the Peripheral cementifying fibroma arising from periodontal ligament of mandibular psoterior tooth.

Peripheral cementifying fibroma is histomorphologically similar to the central cementifying fibroma, but this lesion is unique to the gingival mucosa and is only rarely seen in edentulous area. In most instances, it arise from interdental papilla, with 80% occurring anterior to molar region. This lesion appears to involve the maxilla and mandible equally, demonstrates the highest occurrence rate during the second decade of life, and affects female more frequently than male, with 4.3:1 to 3:2. Peripheral cementifying fibroma presents as firm, pedunculated or sessile mass with an intact or ulcerated surface depending on the presence of trauma. Microscopically, it reveals a highly cellular mass of connective tissue containing numerous plump, proliferating fibrolasts and several forms of calcification. Effective treatment requires deep excision, which includes the ligament and periosteum, followed by a thorough root scaling of the adjacent teeth. A reveiw of the literature and a report of a case of peripheral cementifying fibroma are presented.

10.Molecular Cloning of the Major Immunogen of Orientia tsutsugamushi Thai Strains and Development Passive Hemagglutination Test.

The 56-kilodalton protein genes of Orientia tsutsugamushi TA678, TA686, TA716, TA763 strains were amplified by PCR. The amplified products were sequenced and cloned into pIH821 vector. The recombinant proteins were expressed in Escherichia coli as fusion proteins with maltose binding protein. The recombinant proteins were purified and used for the sensitization of sheep RBCs and the reactivity of the recombinant 56-kDa proteins of Orientia tsutsugamushi TA 678, TA686, TA716 strains were analyzed with 40 sera from scrub typhus patients in Korea, 40 sera from scrub typhus in Thailand, Malaysia and Philippines. The 56-kDa protein coding DNA sequence of Orientia tsutsugamushi TA678, TA686, TA716 show 70 to 88% homology with other known strains and four variable regions are also observed. 39 of 40 sera from scrub typhus patients in Korea showed the strongest reactivity to the recombinant protein of Boryong strain and one sera showed the strongest reactivity to the recombinant protein of Gilliam strain. 14 of 40 sera from patients in Thailand, Malaysia and Philippines showed the strongest reactivity to the recombinant protein of TA686 strain and 12 sera showed the strongest reactivity to the recombinant protein of TA716 strain. No serum from patients in Thailand, Malaysia and Philippines showed the strongest reactivity to the recombinant protein of Boryong strain.
