Osteochondromas are one of the most common benign bone tumors usually involving extraarticular metaphysis of long bone. Solitary intra-articular osteochondroma arising from the elbow joint has rarely been reported. We present a case of 23-year-old female who had pain and limited motion of the left elbow as a result of intraarticular osteochondroma of the distal humerus. Arthroscopic excision of the osteochondroma yielded complete relief of symptoms. Absence of recurrence was confirmed radiographically at two years after surgery. To the best of our knowledge, this is the first report of osteochondroma of the elbow successfully treated arthroscopically.

BACKGROUND: Several practice guidelines for management of dyslipidemia including the National Cholesterol Education Program recommend regular physician follow-up and lipid testing to promote adherence with lipid-modifying drugs. The objective of this study was to analyze the pattern of medical care use among lipid-modifying drug users. METHODS: A retrospective cohort study was conducted with 57,801 new users of lipid-modifying drugs who initiated treatment in 2005. The Korean National Health Insurance Claims Data was used to examine the pattern of medical care use and related factors after treatment initiation for up to two years. Getting the prescription and follow-up lipid testing were considered use of medical care. RESULTS: More than 95% of the patients visited physicians for prescriptions or lipid testings in the first year after initiating drug therapy. In the first two months, 77% had both prescription and follow-up lipid testing, while in the second year, approximately 57% had. Rate for follow-up lipid testing in the first two months was about 78%, but 55% in the second year. Approximately 74% of the patients continued using ambulatory medical care in the second year. In multiple logistic regression analysis, the probability of continual visits in the second year increased in patients who visited one medical institution for prescription and lipid testing within six months. CONCLUSIONS: Early and frequent visits, especially for lipid testings, were associated with continuous medical care use. Patients should be educated regarding the importance of dyslipidemia management in the early stage after treatment initiation.

BACKGROUND: Enterococci exhibit intrinsic resistance or high-level minimum inhibitory concentration (MIC) to beta-lactams than other streptococci. This appears to be due to low affinity of penicillin-binding proteins and rarely production of beta-lactamase, which gives the reason of testing beta-lactamase for blood and cerebrospinal fluid isolates. Ampicillin is more effective than penicillin in vitro, and MIC of ampicillin is generally 1 dilution lower than that of penicillin. The purpose of this study is to detect beta-lactamase producing enterococci an6 to compare MICs of ampicillin and penicillin by Vitek system (bioMerieux, Hazelwood, MO, USA) with those by agar dilution method. METHODS: We collected 110 isolates of Enterococcus faecalis and 51 isolates of E. faecium from clinical specimens in 1998. MICs of antibiotics were determined by agar dilution method and Vitek system. We also performed beta-lactamase test by the Cefinase (Becton Dickinson, USA) for 512 isolates of E. faecalis and 189 isolates of E. faecium collected in 1998. RESULTS: The most common sites of isolates were blood, bile, surgical/traumatic wounds, closed and open pus and urine. MICs of ampicillin were 1 to 2 dilution lower than those of penicillin for E. faecalis (P=0.03). But there were no significant differences in MICs for E. faecium (P=0.19). Five isolates (4 E. faecalis and 1 E. faecium) were susceptible to ampicillin but resistant to penicillin. There were no beta-lactamase producing enterococci among 701 isolates tested. CONCLUSIONS: MIC by Vitek system tends to be 1 to 2 dilution lower than MIC by agar dilution method to beta-lactams, and MIC of ampicillin is 1 to 2 dilution lower than MIC of penicillin, which could result in discrepancy in interpretation of susceptibilty tests. A beta-lactamase test for enterococci is not recommeneded for routine test in Korea.

BACKGROUND: Few studies have reported the results of arthroscopic coracoplasty concomitantly conducted with subscapularis tear. Therefore, this study was conducted to examine and compare the outcomes of arthroscopic subscapularis repair after arthroscopic coracoplasty using either the subacromial approach or rotator interval approach. METHODS: We retrospectively reviewed 51 patients who underwent coracoplasty with subscapularis repair. The patients were grouped according to whether the subacromial approach group (24 patients) or rotator interval approach group (27 patients) was used during coracoplasty. Preoperative and postoperative visual analogue scale scores, American shoulder and elbow surgeons scores, Korean shoulder scores, and range of motion (ROM) were assessed. Assessment of repaired rotator cuff tendon integrity was performed at 1 year after surgery using either magnetic resonance imaging or ultrasonography. RESULTS: At final follow-up, overall functional scores and ROM improved significantly in both groups when compared with preoperative values (p>0.05). The re-tear rates were not significantly different between groups; however, the rotator interval approach group showed a significant increase in ROM compared with that in the subacromial approach group (p<0.05). CONCLUSIONS: Arthroscopic coracoplasty conducted concomitantly with subscapularis repair can provide a satisfactory outcome. There were no significant differences between the two approach groups regarding final functional scores and re-tear rates. However, the rotator interval approach group showed a greater increase in ROM at final follow-up, especially in external rotation.

Angiotensin-converting enzyme (ACE), a dipeptidyl carboxypeptidase, hydrolyzes angiotensin I to the active angiotensin g and also inactivates bradykinin. Activity of ACE was first identified in plasrna by Skeggs et al in 1956. In 1975 Lieberman first reported elevation of serum ACE level in patients with active sarcoidosis and suggested clinical application of the assay to confirm a diagnosis of sarcoidosis and to serve as a guide for therapy. The author studied the serurn ACE levels in 21 syphilitic patients (11 men, 10 women) who showed positive responses in both VDRL and TPHA tests and 20 normal healthy controls (7 men, 13 women) by the spectrophotometric method described by Lieberrnan who modified Cushman and Cheungs method. Comparative studies of ACE levels in syphilitic group with normal control group and relationship among the age, sex and treatment were also conducted. The results were summarized as follows : Ages of the syphilitic patients and normal healthy control group ranged from 18 to 51 years (27 1+-8 98 years) and from 14 to 49 years (28 4+9 08 years) respectively. The mean serum ACE level in syphilitic patients was ll 33+-3 25 p/ml, which was significantly higher than that of normal healthy control group, 6 39+-2 RR p/ml, (p<0001) Of the 21 syphilitic patients, 15(71 4%) had ACE activity higher than the upper limit of normal (mean+1SD). Pityriasis rotunda is characterized by perfectly circular to oval brownish scaly patch on the trunk and extremities. This disease is considered to be a form of acquired ichtkyosis of unknown etiology and develops usually in association with certain systemic diseases, such as tuberculosis, malignant neoplasrn, leprosy, ovarian cyst and uterine myoma etc, We experienced 2 cases of pityriasis rotunda on the abdomen, chest and back associated with liver cirrhosis in 66 year old man 43 year old woman and clinical presentation was made with review of related literatures.
