2.A Case of Asymmetric Septal Hypertrophy Combined with Conn's Syndrome.

We report a case of a 44 year old femele with unilateral aldosterone-proudcing adrenal adenoma characterized by hypertension, plasma aldosterone excess, and low plasma renin, commonly but not invariably with hypokalemia. She also had asymmetric septal hypertrophy of left ventricle established with two-dimensional echocardiography. The electrocardiogram showed inverted T wave and prominent U wave with high QRS voltage on precordial leads. In the case of this patient, we are not sure whether asymmetric septal hypertrophy was caused by secondary hypertension and chronic aldosterone excess of primary aldosteronism, or hypertrophic cardiomyopathy per se, so further long=term follow-up is required to determine it. Following the successful unilateral adrenalectomy, however, the systemic pressure fell down to the normal level and electrolyte abnormalities were corrected immediaterly within a few days and the modest regression in septal hypertrophy was noted in one year, suggesting that the promary aldosteronism contributes to the development or porgression of asymmetric septal hepertrophy.

3.Effect of malocclusion or orthodontic treatment on oral health-related quality of life in adults.

OBJECTIVE: The purpose of this study was to evaluate the effect of malocclusion or orthodontic treatment on oral health-related quality of life (OHRQoL) in adults. METHODS: The sample consisted of 860 adults (378 men and 482 women, aged 18-39 years) who were clinically evaluated for malocclusion or orthodontic treatment experience. Participants were divided into 4 groups as follows: normal occlusion, malocclusion, fixed treatment, and retention. OHRQoL was assessed with the short form of the Oral Health Impact Profile (OHIP-14) and Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ). RESULTS: The malocclusion group and the fixed treatment group had significantly higher OHIP-14 scores than the normal occlusion group and the retention group (p < 0.001). The malocclusion group had the highest PIDAQ score, while the normal occlusion group and the retention group had the lowest PIDAQ score (p < 0.001). Women had higher OHIP-14 and PIDAQ scores than men. A significant positive correlation was found between OHIP-14 and PIDAQ scores (p < 0.01). CONCLUSIONS: Malocclusion has a negative impact on OHRQoL, but this could be improved in adults through orthodontic treatment. These OHRQoL questionnaires can provide additional useful information on specific aspects of orthodontic patients' psychological state.

4.The Effects of Nursing Students' Perception of Emerging Infectious Diseases, Ethical Awareness, and Nurse Image on Career Identity in a Pandemic Situation

Purpose:
The purpose of this study is to identify the effect of nursing students' perception of emerging infectious disease, ethical awareness and nurse image on their career identity in a pandemic situation.
Methods:
Data were collected from 247 nursing students within universities of Daejeon, Gimcheon and Yeongdong area from May 18 to June 20, 2020. The data were analyzed using t-test, ANOVA, Pearson's correlation coefficient and regression analysis.
Results:
The subjects of this study had high level of perception of novel infectious disease (3.72±0.43), ethical awareness in pandemic situations (4.06±0.48), and nurse image (4.07±0.49), but low level of career identity (1.99±0.51). Career identify was significantly correlated with perception of novel infectious disease (r=.18, p=.005), ethical awareness in pandemic situation (r=.16, p=.011), and nurse image (r=.32, p<.001). However, excluding grade and residential area among the general characteristics, only the nurse image (β=.35, p<.001) was identified as a factor influencing career identity, and the explanatory power was 13.1%.
Conclusion
To increase the career identity of nursing students, it is necessary to raise professional nurses’ social awareness and develop a positive nurse image.
5.Shear bond strength of ceramic brackets bonded with antimicrobial monomer-containing self-etching primer.

OBJECTIVE: The purpose of this study was to determine (1) the shear bond strength (SBS) of an antimicrobial monomer-containing self-etching primer according to ceramic bracket types and (2) the bracket-adhesive failure mode using an adhesive remnant index (ARI). METHODS: A total of 90 extracted human teeth were randomly divided into 6 groups. Each group consisted of one of two ceramic brackets (monocrystalline, polycrystalline) and one of three primers (Transbond XT primer, Transbond Plus SEP, Clearfil Protect Bond) with each group containing 15 specimens. The SBS was measured, and adhesive residues left on the tooth surface were assessed. RESULTS: The SBS of polycrystalline ceramic bracket groups was significantly higher than that of the monocrystalline ceramic bracket groups (p < 0.001). The SBS of Transbond XT primer groups was significantly higher than those of Transbond Plus SEP groups and Clearfil Protect Bond groups (p < 0.001). All the groups showed bonding failures between the bracket base and adhesive. CONCLUSIONS: The combination of a self-etching primer with a monocrystalline bracket is recommended for clinical use, considering its acceptable SBS and mode of failure.

6.Nutritional Status and Body Composition in Korean Myopathy Patients.

In myopathy patients, fat mass increases as the disease progresses, while lean body mass decreases. The present study aimed to investigate the overall nutritional status of Korean myopathy patients through surveys of diet and dietary habits, bioelectrical impedance analysis (BIA), and biochemistry tests, as well as the examination of related factors, for the purpose of using such findings as a basis for improving the nutritional status in myopathy patients. The energy intake of all participants was found to be insufficient at only 44.5% of Dietary Reference Intakes for Koreans 2010 (KDRIs 2010), whereas protein intake was sufficient at 89.8% of KDRIs 2010. Dietary fiber intake was found to be 58.4% of sufficient dietary fiber intake for adults according to KDRIs 2010. Calcium intake was found to be 55.0% and magnesium was 14.9% of the recommended calcium and magnesium intake for adults according to KDRIs 2010. With respect to quality of life (QOL), overall increase in QOL domain score showed significant positive correlations with vegetable fat intake (p < 0.05), vegetable protein intake (p< 0.05), and dietary fiber intake (p < 0.05). With respect to BIA, the mean phage angle of all participants was found to be 2.49 +/- 0.93degrees, which was below the cutoff value. As a study that examined nutrient analysis and dietary habits of myopathy patients in Korea, the present study is meaningful in providing the basic data for future studies that aim to present dietary guidelines for patients suffering from myopathy.

Burkholderia cepacia is an aerobic gram-negative rod that causes healthcare-associated infections through contaminated disinfectants, fluids, and medical equipment. This bacterium shows resistance to various antibiotics. For this reason, available therapeutic antimicrobial agents are limited, and the optimal antimicrobial therapeutic regimen was still not well-determined in the case of infective spondylitis. We experienced a case of B. cepacia bacteremia with infective spondylitis and paraspinal abscesses which did not respond to carbapenem monotherapy, but was did respond to ceftazidime and sulfamethoxzaole/trimethoprim combination therapy.

8.Atypical Anterior Spinal Artery Infarction due to Left Vertebral Artery Occlusion Presenting with Bilateral Hand Weakness.

BACKGROUND: Infarct of the anterior spinal artery is the most common subtype of spinal cord infarct, and is characterized by bilateral motor deficits with spinothalamic sensory deficits. We experienced a case with atypical anterior-spinal-artery infarct that presented with bilateral hand weakness but without sensory deficits. CASE REPORT: A 29-year-old man presented with sudden neck pain and bilateral weakness of the hands. Magnetic resonance imaging (MRI) of the brain did not reveal any lesion. His motor symptoms improved rapidly except for mild weakness in his left wrist and fingers. Magnetic resonance angiography showed proximal occlusion of the left vertebral artery; a spine MRI revealed left cervical cord infarction. CONCLUSIONS: Bilateral or unilateral hand weakness can be the sole symptom of a cervical cord infarct.

9.The Serum Lipid Level is Associated with Intimal Thickness of the Carotid Artery for Patients with Coronary Atherosclerosis.

BACKGROUND AND OBJECTIVES: It's not clear whether the serum lipid level is associated with the individual carotid arterial wall thickness for patients suffering with coronary atherosclerosis, although hypercholesterolemia is associated with an increased carotid IMT. We sought to evaluate the association between the serum lipid level and the individual carotid arterial wall thickness (intimal thickness (IT) and medial thickness (MT)) as well as the carotid intima-media thickness (IMT) for patients with coronary atherosclerosis. SUBJECTS AND METHODS: The carotid arterial wall thickness was measured using high-resolution B-mode ultrasound in 139 consecutive patients (58+/-11 years old, 75 males) with coronary atherosclerosis by performing coronary angiography. RESULTS: Measurement of the individual arterial wall thickness was possible in 126 patients (90.6%) out of all the study subjects. The carotid IMT was correlated with the total cholesterol (r=0.207, p=0.015) and low-density lipoprotein (LDL) cholesterol (r=0.237, p=0.006). The carotid IT was correlated with the total cholesterol (r=0.210, p=0.020), triglyceride (r=0.212, p=0.018), and LDL-cholesterol (r=0.246, p=0.006), whereas the MT did not show any significant correlation with the serum lipid level. Multivariate analysis disclosed that the serum LDL cholesterol level was associated with the carotid IMT and IT for the patients with coronary atherosclerosis, but it was not correlated with the MT. CONCLUSION: This study suggests that the serum LDL cholesterol level is more closely associated with the carotid IT than the IMT for patients with coronary atherosclerosis, and each carotid arterial wall has a different response to the serum lipid level.

10.Primary Mucosa-Associated Lymphoid Tissue Lymphoma of the Breast: A Case Report.

A primary mucosa-associated lymphoid tissue lymphoma (MALT lymphoma) of the breast is extremely rare. We report a case of a MALT lymphoma of the breast that presented as a palpable left breast mass in a 37-year-old woman. A physical examination revealed a large firm, and fixed mass in the left inner breast. Mammograms showed a large, isodense mass in the lower inner quadrant of the left breast and an enlarged lymph node in the axilla. A sonogram demonstrated a 5 cm sized, oval, circumscribed, and heterogeneously hypoechoic mass with posterior acoustic enhancement. A surgical biopsy was performed, and the pathology revealed a MALT lymphoma.
