To build and to manipulate clinical data in one of the important works in the hospital. In order to accurately append the data and to quickly find and display the informations as the user need, we developed a software program running on the personal computer. Our system largely consists of four parts; registration of the clinical and departmental data, retrieving tool of articles in the medical journal, collection of special data for clinical survey and display system of the various reports. In our experiences, we consider the key factors for systemic management of clinical rearch data base as the follows; work analysis for data processing, design of data base, coding and classification of basic data and technique of registeration. Of these standarized coding system of the orthopedic diseases appeare to be of it most importance.

4.Multiple Primary Malignant Neoplasms: A case of double primary malignant cancer.

This is a case report of double primary malignant cancer occurred aynchronously in the stomach and lymphoid tissue, We report this case with review of literatures about the criteria, age distribution, predisposing factor, inidence, immunity and susceptibility of the primary malignant neoplasms. This case was a 59-year-old man who had Hodgkins disease and tubular adenocarcinoma, of the stomach. The diagnoais was verified histologically, Although multiple primary neoplaas are rare, the possibility of that must be conaidered seriously, And its hereditary predisposition and other predisposing factor muat be researched with enthuaiasm.

5.A Survey on the Long-term Hospitalized, Chronically Mentally-ill Patients in Mental Health Facilities.

OBJECTIVES: In an attempt to identify the demographic characteristics and the factors influencing long-term hospitalization of chronically mentally-ill patients, a survey on the chronically mentally-ill patients who had been staying longer than 1 year in mental health facilities was conducted. METHODS: Checklist including some demographic data that were regarded as independent variables was applied to 3,477 subjects through administration system of Pusan Metropolitan City. Statistical analyse were done using distribution-free, non-parametric tests. RESULTS: 1)Subjects who had been staying longer than 1 year were 70.7% of the total inpatients of mental health facilities, and median duration of their hospitalization was 5.5 years. 2)In mental hospitals and asylums, the ratio of chronically mentally-ill subjects to the total inpatients in each facilitiy was 46.3% and 95.1%, respectively. 3)All of the 10 independent variables had an influence on the hospitalization duration. CONCLUSIONS: These results indicated that many indirect, extramedical factors had an influence the long-term hospitalization of chronically mentally-ill patients, and it is likely to reflect the underdevelopment of mental health services in Korea.

6.Conjoined Nerve Root Anomaly Associated with Lumbar Disc Herniation: A Case Report.

The condition of anomalous nerve root is rare. The authors are reporting a case of conjoined nerve root anomaly associated with lumbar disc herniation in a 36-year-old male who was transferred from another hospital. A standard hemilaminectomy with a discectomy had been performed under the disgnosis of the L4-5 lumbar disc herniation at the other hospital. However symptoms were not relieved. During a second surgery at our hospital, a L5-S1 conjoined nerve root was discovered after removal of the extradural hematoma at the previous laminectomy site, a medial facetctomy with a foraminotomy were carried out and the conjoined root was decompressed completely. The patient returned to work 2 months postoperatively with complete relief of symptoms.

7.A Case of Total Anomalous Pulmonary Venous Confluence in Adult Diagnosed by Echocardiography.

Total anomalous pulmonary venous confluence(TAPC), a rare cyanotic congenital anomaly of the heart especially in adult, is now diagnosed by echocardiography instead of cardiac catheterization with high sensitivity and specificity. The key points are identification of pulmonary veins, pulmonary venous confluence draining anomalously to the heart. RVDVO on M-mode, abnormal echo free space posterior to left atrium on 2D-echo are basic characteristics of TAPVC. Peak velocity and flow profile measurement on the doppler provide additional informations : obstruction and anomalous vessels. A case of 23 year old male with supracardiac type TAPVC diagnosed by echocardiography was corrected successfully by surgery.

8.Smoking Status and Serum NSE Level, as Prognostic Factors in Adenocarcinoma of Lung.

BACKGROUND: The incidence of lung adenocarcinoma, which is more prevalent in women and nonsmokers, is increasing. The aim of this study was to determine the prognostic factors of an adenocarcinoma of the lung. MATERIAL AND METHOD: The clinical information of patients diagnosed with an adenocarcinoma of the lung at the Kosin University Gospel Hospital from January 1994 to July 2004 was reviewed retrospectively. The survival time of these patients was analyzed by the patient's age, gender, performance status, weight loss, smoking history, location of the primary tumor, clinical stage, serologic tumor markers, and treatment modality. RESULTS: For all 422 patients with an adenocarcinoma of the lung, 247 (58.5%) were male, and their mean age was 59.8 years the. The majority of patients were smokers (58.3%), and the tumors were located in the periphery (59.7%). In the smokers, the tumor was located more in the central airway compared to the non-smokers (42.8% vs. 31.9%, p=0.12). The overall median survival time was 390 days (95% CI;304-436 days). Univariate survival analysis revealed that an older age (>or=65 years old), male, weight loss, smoker, central type, advanced clinical stage, elevated serum carcinoembryonic antigen (CEA, >5 ng/ml) and neuron specific enolase (NSE, >15 ng/ml), and the supportive care only were significantly poor prognostic factors. The median survival time was shorter in the smokers than nonsmokers (289 days vs. 533 days, p<0.001). In addition, it was also shorter in the elevated NSE group than in the normal range group (207 days vs. 469 days, p<0.001). Multivariate analysis showed that age, clinical stage, serum NSE, smoking status, and treatment modality were independent predictors of survival (hazard ratios: 1.68, 1.94, 1.92, 2.39 and 1.57, respectively). CONCLUSIONS: Smoking is an important prognostic factor in an adenocarcinoma of the lung, but not gender. This suggests that the better prognosis of women is more related with the lower rate of smoking. In addition, the elevated serum NSE is also an important prognostic in an adenocarcinoma of the lung.

9.Photodynamic Therapy in Early Lung Cancer: A Report of Two Cases.

Photodynamic therapy (PDT) that is based on the science of photochemistry has been recognized as a lung sparing local therapeutic modality that can achieve remarkable responses. It is an alternative treatment for early stage lung cancer patients who have poor lung function or multiple sites of cancer. Recently we treated a 70-year-old man who presented with squamous carcinoma in situ at a previous pneumonectomy site, and a 64-year-old man with a newly developed secondary superficial lung cancer, with PDT. There were no complications related to the procedure. Both patients had poor lung function due to prior lung cancer surgery. Clinical and histological complete remissions were achieved without any evidence of recurrence during 30 months of follow-up in both patients.

10.Spontaneous Lumbar Epidural Hematoma: Case Report.

The authers report a case of spontaneous lumbar epidural hematoma in a 16-year-old boy who developed lumbago, paraparesis and gait disturbance over a six-day period. Metrizamide myelography and CT demonstrated the extradural mass lesion at the L3-4 level. MRI revealed a hyperintense mass on T1-weighted and T2-weighted sequences and it enabled to make diagnosis of epidural hematoma. The patient had full postsurgical recovery. We concludes MRI is the most helpful diagnosis modality for spinal epidural hematoma.
