1.A Study on the Minimal Phototoxic Dose (MPD).

In order to measure the MPD with 8-methoxypsoralen, we selected 49 Korean healthy male vlunteers without phototoxic or photosensitive dermatoses. They were divided to 3 groups (Immediate; group 1, 1 hour; group 2, 2 hours', group 3) according to the waiting time, intervals of application of photosensitizer a,nd UVA:irradiation. The reaults were summarized as follows: 1. MPDs of group 1 had no clinical significance. 2, MPDs of group 2 were more than those of group '3 independently of application methods of photosensitizer. 3. In the cases of topical application of 8-MOP, MPDs of group 3 according to reading interval (24 hours, 48 hours and 72 hours after UVA irradiation) were 3.4+/-2.9 J/cm2, 1.9+/-l.5 J/cm' and l. 7+1, 2/cm, respectively. 4 In the cases of oral administration of 8-MOP, MPDs of group 3 according to reading; interval as topical application of 8-MOP were 7.1+/-2.3 J/cm, 4.4+/-1.1 J/ cm2 and 4.2+/-l.2 J/cm2, respectively. 5 MPL)s accarding to the skin types as follows; (waiting time; 2 hours, read- ing interval; 48 hours) a. In the case of topical application of 8-MOP, MPDs of the skin type ]II, 1V and V were 0.9+/-0.5 J/cm2, 1.8+/-1.1 J/cm2 and 3.0+/-l.3 J/cm2 respectively. b. In the case of oral administration of 8-MOP, MPDs of skin type g, W and were 3.5+/-1.1 J/cm2, 4.7+/-1.3 J/cm2 and 6.9+/-l.8 J/cm2, respectively.

2.A Study on the Solar UVA and UVB Doses at Kongju.

An appreciation of daily and annual changes of ultraviolet radiation is important in order to optimize the conditions for climatic therapy of skin diseases or in order to minimize the harmful effects of the sun. Ultraviolet A(355nm) and ultrviolet B(290nm) doses were measured at Kongju from April to September, 1985, on two different weather conditions-clear and relatively clear. The followings were the summary of the findings. 1. On clear weather, UVA dose was peak at noon of the day and in June of the year. 2. On relatively clear weather, UVA dose was peak at 2 p.m. of the day and in July of the year. 3. UVB dose was peak at noon of July regardless of weather conditions. 4. In relatively clear weather the UVA and UVB doses decreased from those of clear weather by 29%, and 36% respectively. 5, The relative difference between the doses of UVA and UVB depended on the degree of solar zenith angle. The greater the solar zenith angle, the greater the difference. 6. In order to treat a patient with solar UVA radiation, therefore, it is suggested to have the patient expose himself to the sun around 10 a, m. and 3 p.m. to minimize the harmful effects of solar UVB. However, when one wants to treat with solar UVB radiation, it would be better to expose to the sun around noon.

3.A Case of Pityriasis Lichenoides et Varioliformis Acuta Treated with Photochemotherapy.

A lg year-old female patient with pityriasis lichenoides et varioliforrnis acuta (PLEVA) was been treated with PUVA, At first, she was treated with tetracycline for 8 weeks with improvement somewhat. But 2 months later, her skin lesions aggravated slowly. So, we tried to treated her with PUVA and then she was cleared completely. The initial dose of UVA was 5 J/cm The UVA dosage increased by 10g of the previous exposure and increased to the maximum dose, 15 J/cm The patient exposed to UVA R times per week. 12 times after the PUVA therapy, the skin lesions begin to improve and completely improved at 21 times of Pl.JVA therapy. She was followed up every week and there was no side effect except hyperpigrnentation till 24 months. The total dose during treatment is 210 J/cm.

4.Three cases of benign symmetric lipomatosis.

Benign symmetric lipomat,osis is a rare disease of undetermined cause characterized by symmetric deposits of adipose tissue o:n the neck, upper trunk and abdomen. We report three cases of kienign symmetric lipomatosis, one of which is associated with alcoholic liver disease and the patient complained of a diffieulties in neck motion, All of them are about sixty year old men with history of frequent alcohol consumption. In histopathologic findings, the reticular dermis was completcly replaced by mature fat cells and there was a slight increase in vascular and fibrous elements.

5.Malignant Lymphoma of Mucosa-associated Lymphoid Tissue Arising in the Conjunctiva.

We report two cases of malignant lymphoma of mucosa-associated lymphoid tissue arising in the conjunctiva. Case 1, an 18-year-old girl, showed her left lower conjunctival swelling for four months and Case 2, a 29-year-old man, showed similar lesions on both his eyes for one year. Histologically, both cases revealed monomorphous dense lymphoid infiltrates invading the overlying conjunctival epithelium with diffuse patterns. The infiltrates were composed of small lymphocytes admixed with centrocyte-like cells within the conjunctival epithelium and stroma in association with B-cell lineage. No relapse was noted during the period of about 1½ years follow-up in case 1 treated by local excision only and in the follow-up period of 2 months in case 2 treated by local excision and subsequent chemotherapy.

6.Placental Site Trophoblastic Tumor(PSTT) with Vaginal Metastasis: A case report.

PSTT has been established as a separate entity distinct from choriocarcinoma since 1983, because of its different morphological features, biologic behavior and response to chemotherapy. Most cases of PSTT have indolent clinical courses, but there have been several reports showing malignant behavior resulted in death of the patient. We report a case of PSTT showing vaginal metastatis. In biopsy specimen, it was difficult to make differential diagnosis from squamous cell carcinoma of uterine cervix due to uniform and mononuclear cytologic characteristics. Immunohistochemical stains for hPL and hCG were not typical for either PSTT or choriocarcinoma. We suggest that poorly differentiated PSTT may synthesize the lesser amount of hPL than typical PSTT and it may behave more aggressively.

7.Role of Leukemia Inhibitory Factor in the Effect of Co-Culture on Preimplantation Embryo Develpement.

OBJECTIVE: To assess the effect of recombinant human leukemia inhibitory factor on in vitro development of 1-cell ICR mouse embryo. MATERIALS AND METHOD: ICR mice were superovulated with PMSG/hCG and 1-cell stage mouse embryos were recruited. 1-cell mouse embryo were cocultured on human oviductal cells in a CO2 incubator (coculture group) and were cultured on 0.4% BSA+HTF media (control group). And anti-hLIF Ab was added the cocultured group in a different concentration (1pg, 10pg, 100pg, 1ng) and developmental rate was compaired to the control group, and rhLIF was added to the preincubated 0.4% BSA+HTF media in a different concentration (2000U, 1000U, 100U, 10U) and its developmental rate was compaired to group which was cultured on 0.4% BSA+HTF media only. RESULT: 1. The cleavage rate of 2-cell mouse embryo co-cultured with human tubal epithelial cell was significantly higher than that of cultured with media alone (HTF with 0.4% BSA) (p<0.05). 2. When LIF antibody was added to the medium with human tubal epitherlial cell, the mouse embryo could not cleave more than 2-cell in 1 ng of LIF antibody, and less than 1 ng, the cleavage rate was lower than cultured without LIF antibody group(p<0.05). 3. Two cell blocked ICR mouse embryos were developed into four cells under LIF(p<0.05), but no further development was observed. CONCLUSIONS: These results shows that LIF enhances the development of preimplantation embryo, and when rhLIF is applicated in vitro, it has positive effects on the development of early mouse embryo and can help overcoming the two-cell block.

Early gastric cancer(EGC) is defined as carcinoma limited to the mucosa or submucosa, regardless of whether metastasis to lymph nodes have occurred, and the frequency of lymph node metastasis varies from 7 to 18%. The incidence of early gastric cancer has been increasing recently, probably with advance in the diagnostic procedure. Multiple gastric cancer, now cosidered to be a sort of multiple primary cancer by Moertels classification, is a special type of cancer in which two or more tumor lesions arise independently from the stomach. Multiple carcinoma was found in about 8.3% of 500 early gastric cancer cases at the National Cancer Center Hospital in Japan. In 77% of these, two lesions roexisted in the stomach. Coexistence of three lesions were found in 20% and more than four lesions in 3%. We report a case of multiple early gastric cancer of different histologic types in which two adenoma coexisted in the stomach.

10.Expression of Transglutaminase K in Several Skin Diseases.

To understand the role of Transglutaminase K(TGase K) in skin diseases, the expression of TGase K protein was studied in hyperkeratotic inflammatory skin diseases, and in several different kinds of benign and malignant skin tumors. Immunohistochemistry was used to detect the TGase K protein with a new anti-human TGase K polyclonal antibody. This antibody showed the same immuno- histochemical staining pattern as in previously published immunofluorescence data with the exception that the immunohistochemical stain showed a slightly weaker stain in the granular layer. However, the staining intensity was decreased in psoriasis, which is opposite for results obtained with the widely used TGase K monoclonal antibody B.C1. In other hyperkeratotic inflammatory skin diseases, like pityriasis rubra pilaria, lichen planus, and pityriasis rosea, the staining patterns were similar while the staining intensity was decreased, compared to normal epidermis. Squamous cell carcinoma showed a diffuse staining pattern in tumor cells with a small intensity increase in well differentiated tumor cells. However, in actinic keratosis and Bowen's disease, which are thought to be precursors of squamous cell carcinoma, the staining intensity was weak. The tumor cells of basal cell carcinoma, malignant melanoma, and seborrheic keratosis showed a weak staining intensity.
