Dental Implants have been proved to be successful prosthetic modality in edentulous patients for 10 years. However, there are few reports on the survival of implant according to location in molar regions. The purpose of this study was to evaluate the 4~5 years' cumulative survival rate and the cause of failure of dental implants in different locations for maxillary and mandibular molars. Among the implants placed in molar regions in Gwangju Mir Dental Hospital from Jan. 2001 to Jun. 2002, 473 implants from 166 patients(age range; 26~75) were followed and evaluated retrospectively for the causes of failure. We included 417 implants in 126 periodontally compromised patients, 56 implants in 40 periodontal healthy patients, and 205 maxillary and 268 mandibular molar implants. Implant survival rates by various subject factors, surgical factors, fixture factors, and prosthetic factors at each location were compared using Chi-square test and Kaplan-Meier cumulative survival analysis was done for follow-up(FU) periods. The overall failure rate at 5 years was 10.2%(subject level) and 5.5%(implant level). The overall survival rates of implants during the FU periods were 94.5% with 91.3% in maxillary first molar, 91.1% in maxillary second molar, 99.2% in mandibular first molar and 94.8% in mandibular second molar regions. The survival rates differed significantly between both jaws and among different implant locations(p<0.05), whereas the survival rates of functionally loaded implants were similar in different locations. The survival rates were not different according to gender, age, previous periodontal status, surgery stage, bone graft type, or the prosthetic type. The overall survival rate was low in dental implant of too wide diameter(> or =5.75 mm) and the survival rate was significantly lower for wider implant diameter(p<0.01) in mandibular second molar region. Among 5 surface types(acid etched, SLA, TPS, RBM, and HA), the survival rate of SLA type implant was the highest during the FU periods and the failure rates of HA type implants was significantly high following functional loading. Among 26 failed implants, 20 resulted in early failure of osseointegration or infection prior to functional loading, and 6 were removed because of progressive bone loss or implant fracture. In conclusion, implant survival rates were different in different locations on the posterior jaws, and the fixture diameter and surface type were the significant factor for implant survival in mandibular 2nd molar region. This observation suggests that implant treatment planning might require region-specific manner.

PURPOSE: This study was conducted to evaluate the validity of the ‘ Enterprise Health Promotion Index’, a tool for assessing health promotion activities of worksite developed by the Ministry of Employment and Labor and the Korea Occupational Safety and Health Agency. METHODS: This is designed a methodological study that evaluates the validity of the Enterprise Health Promotion Index. For this study, a questionnaire survey was conducted for the workplace health professional and safety professional, and a total of 297 data were extracted as final data. We developed the informal health promotion needs and activity measurement tools. The correlation coefficient between the score of the enterprise health promotion index and the score of the informal health promotion tool was analyzed for the criterion validity evaluation. RESULTS: The criterion validity of the Enterprise Health Promotion Index and activity index were supported by finding moderate (r=.597) and high (r=.783) correlation coefficient. However, the requirement index has low levels of criterion validity (r=.364). CONCLUSION: Based on these results, we suggest to improve the utility of the corporate health promotion index by developing user manual, active public relations, and providing briefing sessions.

The 5th year KONSAR surveillance in 2001 was based on routine test data at 30 participating hospitals. It was of particular interest to find a trend in the resistances of enterococci to vancomycin, of Enterobacteriaceae to the 3rd generation cephalosporin and fluoroquinolone, and of Pseudomonas aeruginosa and acinetobacters to carbapenem. Resistance rates of Gram-positive cocci were: 70% of Staphylococcus aureus to oxacillin; 88% and 16% of Enterococcus faecium to ampicillin and vancomycin, respectively. Seventy-two percent of pneumococci were nonsusceptible to penicillin. The resistance rates of Enterobacteriaceae were: Escherichia coli, 28% to fluoroquinolone; Klebsiella pneumoniae, 27% to ceftazidime, and 20% to cefoxitin; and Enterobacter cloacae, > or =40% to cefotaxime and ceftazidime. The resistance rates of P. aeruginosa were 21% to ceftazidime, 17% to imipenem, and those of the acinetobacters were > or =61% to ceftazidime, aminoglycosides, fluoroquinolone and cotrimoxazole. Thirty-five percent of non-typhoidal salmonellae were ampicillin resistant, and 66% of Haemophilus influenzae were -lactamase producers. Notable changes over the 1997-2001 period were: increases in vancomycin-resistant E. faecium, and amikacin- and fluoroquinolone-resistant acinetobacters. With the increasing prevalence of resistant bacteria, nationwide surveillance has become more important for optimal patient management, for the control of nosocomial infection, and for the conservation of the newer antimicrobial agents.

Background: Nursing service is a nonroutine work with an excessive physical load and diverse tasks. This study derived representative common tasks based on the frequently occurring tasks with a high physical load in the nursing workers' daily work and developed indicators to evaluate the work risk by reflecting the characteristics of nonroutine work. Methods: Common tasks were classified through the following stages: literature review, first focus group interview (FGI) with experts, first classification of common tasks, second FGI with hospital health managers, a survey of nursing service workers, and the final classification of common tasks for each task type. To develop an objective risk index for physical load assessment, we investigated the frequency and duration of the derived common tasks via survey. Results: Nursing common tasks were categorized into six task types and 56 subtasks. To evaluate the risks of various tasks in nonroutine works, three frequencies and three working time levels were defined by examining the task frequency and working hours. Exposure time was defined to reflect the characteristics of a nonroutine job. The final risk assessment was the product of the exposure time level and job intensity level. From this, four risk action levels were derived. Conclusion This study has the advantage of solving the problem of focusing on some tasks in evaluating the physical load. It was meaningful in that a new risk assessment index based on exposure time was proposed based on the development of an evaluation scale for frequency and time by reflecting the characteristics of nonroutine work.

PURPOSE: The purpose of this study was to histomorphometrically evaluate the osteoconductivity of a new biphasic calcium phosphate ceramics with fully interconnected microporous structure. MATERIAL AND METHODS: Osseous defects created in the rabbit calvaria were filled with four different bone graft substitutes. Experimental sites were filled with a new fully interconnected microporous biphasic calcium phosphate with(BCP-2) or without(BCP-1) internal macropore of 400micrometer in diameter. MBCP(Biomatlante, France) and Bio-Oss(Geistlich Pharma, Switzerland) were used as controls in this study. Histomorphometric evaluation was performed at 4 and 8 weeks after surgery. RESULT: In histologic evaluation, new bone formation and direct bony contact with the graft particles were observed in all four groups. At 4 weeks, BCP-1(15.5%) and BCP-2(15.5%) groups showed greater amount of newly formed mineralized bone area(NB%) compared to BO(11.4%) and MBCP(10.3%) groups. The amounts of NB% at 8 weeks were greater than those of 4 weeks in all four groups, but there was no statistically significant differences in NB% between the groups. CONCLUSION: These results indicate that new bone substitutes, BCP with interconnected microporous structure and with or without internal macroporous structures, have the osteoconductivity comparable to those of commercially available bone substitutes, MBCP and Bio-Oss.

PURPOSE: This study was performed to examine vascular pain due to gemcitabine and according to clinical factors. METHODS: The survey was performed with 525 cancer patients visiting chemotherapy infusion room in one general hospital. The data were collected via self-reported questionnaire, researchers observation, and reviewing medical records. Data were analyzed descriptive statistics, t-test, Kruskal-Wallis test, Tukey test using ranks, Jonckheere-Terpstra test, and Spearman correlation analysis were used. RESULTS: The mean scores of the intensity and onset time of vascular pain were 3.06+/-2.16 and 8.13+/-13.13 min, and each other were negative correlaton. Degree of intensity of vascular pain had a significant difference on gender (p=.003), age (p=.004), weight (p=.019), Body mass index (BMI) (p=.005), Body surface area (BSA) (p=.030), infusion time (p<.001), dose (p=.027), dose per minute (p<.001), the number of administered gemcitabine (p=.005), combination chemotherapeutic drug (p=.013) and tumor type (p=.002). Degree of onset time of vascular pain had a significant difference on infusion time (p=.044), combination chemotherapy (p=.001) and injection site (p=.001). CONCLUSION: Patients administrated gemcitabine were experiencing various vascular pain, and vascular pain had a difference on clinical factors. Therefore, oncology nurses should be considered significant clinical factors to implement effective interventions to patients administered gemcitabine.

BACKGROUND: Heatstroke is one of the most serious heat-related illnesses. However, establishing public policies to prevent heatstroke remains a challenge. This study aimed to investigate the most relevant climate elements and their warning criteria to prevent outdoor heatstroke (OHS).METHODS: We investigated heatstroke patients from five major hospitals in Daegu metropolitan city, Korea, from June 1 to August 31, 2011 to 2016. We also collected the corresponding regional climate data from Korea Meteorological Administration. We analyzed the relationship between the climate elements and OHS occurrence by logistic regression.RESULTS: Of 70 patients who had heatstroke, 45 (64.3%) experienced it while outdoors. Considering all climate elements, only mean heat index (MHI) was related with OHS occurrence (p=0.019). Therefore, the higher the MHI, the higher the risk for OHS (adjusted odds ratio, 1.824; 95% confidence interval, 1.102–3.017). The most suitable cutoff point for MHI by Youden’s index was 30.0°C (sensitivity, 77.4%; specificity, 73.7%).CONCLUSION: Among the climate elements, MHI was significantly associated with OHS occurrence. The optimal MHI cutoff point for OHS prevention was 30.0°C.

Objective: The characteristics and prognosis of out-of-hospital cardiac arrest (OHCA) patients can vary due to a variety of factors, including the time of the day. We tried to identify the characteristics and prognosis of OHCA in a Korean metro city based on the time of the day. Methods: This citywide retrospective observational study was conducted from January 1, 2015, to November 31, 2020, in Daegu, Korea on patients over 18 years of age who were suspected of having a medical etiology of OHCA. We evaluated the characteristics and outcomes of OHCA, according to the time of day, divided into dawn (00:00-05:59), morning (06:00-11:59), afternoon (12:00-17:59), and night (18:00-23:59). The outcome variables were survival to hospital discharge and favorable neurological outcomes. Results: The median age of the total of 4,783 OHCA patients in the study was 72.0 years of which 3,096 (64.7%) were males. The number of patients who survived was 317 (7.8%) and 301 (6.3%) were discharged with favorable neurological outcomes. There were 672 (14.0%) patients admitted at dawn, 1,607 (33.6%) in the morning, 1,379 (28.8%) in the afternoon, and 1,125 (23.5%) at night. After adjusting for the possible confounding variables, compared with the morning group, the survival to hospital discharge was low in the afternoon and the night (adjusted odds ratio [aOR], 0.69; 95% confidence interval [CI], 0.48-0.98 and aOR, 0.48; 95% CI, 0.32-0.74). In addition, favorable neurological outcomes were also low in the afternoon and the night compared with the morning (aOR, 0.59; 95% CI, 0.40-0.85 and aOR, 0.62; 95% CI, 0.41-0.93). Conclusion Diurnal differences in OHCA outcomes were observed. Identification of the diurnal OHCA characteristics will be necessary to devise an appropriate regional emergency medical services strategy.

Objective: This study aimed to analyze types and rates of symptoms according to vaccine type and to identify the characteristics of patients who visited emergency departments (EDs) with adverse reactions after coronavirus disease 2019 (COVID-19) vaccination. Methods: The medical records of 1,020 patients who visited the EDs of five participating tertiary hospitals from February 26, 2021 to July 31, 2021, and reported adverse reactions after vaccination with a COVID-19 vaccine to the Korean Disease Control and Prevention Agency or diagnosed with a disease code U129 at time of ED discharge were retrospectively analyzed. Symptoms, examinations, treatments received at EDs, and hospital outcomes were compared with respect to vaccine type. Results: Of the 1,020 study subjects, 559 received the ChAdOx1-S vaccine, 52 the Ad.26.COV2.S vaccine, 331 the BNT162b2 vaccine, and 77 the mRNA-1273 vaccine. The adenovirus-vectored vaccines (ChAdOx1-S and Ad.26.COV2.S) had the highest myalgia and headache complaint rates during the 24 hours following vaccination. However, the mRNA vaccines (BNT162b2 and mRNA-1273) had the highest chest pain and fever complaint rates at 7 days post-vaccination. Furthermore, 4.7% of the study subjects required hospitalization, and three died during hospitalization. Conclusion Patients who visited EDs with adverse reactions after COVID-19 vaccination had various ages and experienced symptoms that depended on vaccine type. Most patients were discharged following ED symptom management, but caution should be exercised as several patients experienced potentially serious adverse events.
