1.The Prevalence of Scoliosis in Junior and Senior High School Students, Pusan, Korea

Authors examined 40,000 minifilms(boys and girls each 10,000 in junior and senior high schools) which were taken for survey of pulmonary tuberculosis for junior and senior high school students and checked the prevalence of scoliosis. It was considered lateral curvature of spine more than 10 degrees as nonpostural scoliosis, which were analyzed prevalence rate, degree of curvature, distribution of size of curvature, pattern of curve and direction of curvature. Following results were obtained. 1. The overall prevalence rate was 1.56; male 1.38. and female 1.70%(Junior high school 1.06%, Senior high school 2.05%). 2. Scoliosis secondary due to bony abnormality was found in 5 cases. 3. The most common pattern of curvature was the thoracic curve. 4. In sihgle curve, right to left ratio was 1.7, especially senior high school girls had more right curve and junior high school boys had more left curve. 5. The prevalence of senior high school students was greater than that of junior high school students and the degree of curvature was also severe in senior high school students than in junior high school students.

2.Reversible Dementia With Rapid Recovery to Normal Cognition: A Case Report

We report the case of a 54-year-old female who was diagnosed with clinical dementia and recovered to normal cognition in only 11 days through appropriate intervention. The patient was on a complicated regimen of psychiatric medications for underlying depression and panic disorder. The patient noted increased deterioration in executive function gradual memory impairment starting in early 2020.As a result of the neuroimaging, prominent neurodegeneration and in vivo brain pathology were not observed. During outpatient clinic follow-up, severe obstructive sleep apnea (OSA) was confirmed. Based on the diagnostic results, a clinical impression was made for reversible dementia due to psychiatric drugs and OSA. During hospitalization for 10 days, the patient’s regimen of psychiatric medications with anticholinergic effects was changed, and long-acting benzodiazepines were reduced. The patient also underwent continuous positive airway pressure titration to ameliorate OSA. The patient reported subjective cognitive improvement and a comprehensive neuropsychiatric test performed at discharge later showed a normal range in all cognitive domains, and the patient's activity of daily living evaluated at the outpatient clinic after discharge had recovered.
3.Novel Method of Classification in Knee Osteoarthritis: Machine Learning Application Versus Logistic Regression Model

Objective:
To present new classification methods of knee osteoarthritis (KOA) using machine learning and compare its performance with conventional statistical methods as classification techniques using machine learning have recently been developed.
Methods:
A total of 84 KOA patients and 97 normal participants were recruited. KOA patients were clustered into three groups according to the Kellgren-Lawrence (K-L) grading system. All subjects completed gait trials under the same experimental conditions. Machine learning-based classification using the support vector machine (SVM) classifier was performed to classify KOA patients and the severity of KOA. Logistic regression analysis was also performed to compare the results in classifying KOA patients with machine learning method.
Results:
In the classification between KOA patients and normal subjects, the accuracy of classification was higher in machine learning method than in logistic regression analysis. In the classification of KOA severity, accuracy was enhanced through the feature selection process in the machine learning method. The most significant gait feature for classification was flexion and extension of the knee in the swing phase in the machine learning method.
Conclusion
The machine learning method is thought to be a new approach to complement conventional logistic regression analysis in the classification of KOA patients. It can be clinically used for diagnosis and gait correction of KOA patients.
4.Suppressive Therapy with Levothyroxine in Benignn Solitary Thyroid Nodule.

BACKGROUND: Clinically apparent thyroid nodule is a very common disease in adults. Patients are often treated with thyroxine in order to reduce the size of the nodule, but the efficacy of thyroxine-suppressive therapy with thyroxine remains uncertain. We investigated the efficacy of thyroxine-suppressive therapy and the hormonal characteristics during thyroxine therapy to find out whether such measurement could be used to determine the effectiveness of this therapy in patients with benign solitary thyroid nodule proved by aspiration biopsy. METHODS: In this study, 54 patients were randomly assigned to receive L-thyroxine (Group I, n=24) or placebo (GroupII, n=30) for 1 year. High resolution ultrasonography (10MHz) was used to measure the size of the nodules at six month intervals. Thyroid hormones and thyroglobulin assay and FNA (fine needle aspiration) was done at the same time. The dose of thyroxine was 200ug/day. Patients were followed at 6 month intervals. RESULTS: The results were as follows: 1) 13 (54.1%) out of 24 Group I patients after adequate TSH suppression had a statistically significant reduction of nodule size and before-to-after nodule volume ratios were significantly different between the Group I and Group II patients. 2) In the responders among Group I patients, the before-to-after therapy ratio of the nodule volume was not related to the pretreatment nodule size, thyroid hormones and thyroglobulin levels. CONCLUSION: Thus we concluded that an adequate suppressive dose of L-thyroxine significantly altered the volume of the benign solitary thyroid nodules 12 months later.

5.Morphologic Characteristics of Finger and Palm Prints of the Akha and Lahu in Northern Thailand.

In this study of the physical anthropological characteristics of the Akha and Lahu in northern Thailand, whose customs are similar to the Koreans, the authors examined the finger and palm prints of the Akha (male 107, female 114) and Lahu (male 92, female 101) using qualitative methods, and compared them with those of various ethnic groups including Koreans. The results were as follows : The whorl types (Akha 57.7%, Lahu 58.1%) were the most common finger prints, followed by ulnar loop types (Akha 39.7%, Lahu 38.7%), arch types (Akha 2.6%, Lahu 2.8%), and radial loop types (Akha 1.7%, Lahu 1.2%). Of the palm print types, 9-7-5 (Akha 32.7%, Lahu 21.3%) and 7-5-5(Akha 25.4%, Lahu 21.3%) were most common, followed by 11-O-7 (Akha 2.0%, Lahu 11.3%), 9-O-5 (Akha 1.5%, Lahu 10.0%), 11-9-7 (Akha 6.0%, Lahu 5.7%), 7-5-4 (Akha 7.0%, Lahu 2.2%). The finger and palm print patterns of the Akha and Lahu did not correspond closely to the patterns of those classified as White, Negro, or Oriental, although they were closest to the patterns of the latter, particularly Chinese (rather than Korean or Japanese).

6.Assessment of Left Atrial Function and Remodeling in Patients With Atrial Fibrillation by Performing Strain Echocardiography: A Prospective Study to Assess the Influence of Renin-Angiotensin System Inhibitors on Atrial Fibrillation.

BACKGROUND AND OBJECTIVES: Renin-angiotensin system (RAS) inhibitors are likely to reduce the development of atrial fibrillation (AF) by preventing atrial fibrosis. We attempted to assess the relevance of strain echocardiography for quantitative assessment of the left atrial (LA) status in AF, its modification by RAS inhibitors and changes of biochemical markers during cardiac remodeling in AF. SUBJECTS AND METHODS: Strain echocardiography is performed 2 times (baseline and 12 month) in 60 patients with AF (RAS inhibitors-used group: 30, non-used group: 30). In an apical 4-chamber view, the regional analysis consisted of placing the region of interest cursor at the basal segments of the septal and lateral wall of LA. Mean peak systolic and early diastolic strain/rate are measured with LA end-systolic antero-posterior, longitudinal and transverse dimensions. RESULTS: Six patients of RAS inhibitors-used group (group A, 20%) and three patients of non-used group (group B, 10%) were converted to normal sinus rhythm during the study. LA size, E wave velocity, E/E', strain parameters showed no significant differences between groups at the baseline. There were no significant differences in LA size and E wave velocity between groups at the 12 months, however, peak systolic strain/rate were significantly higher in group A (36.71+/-13.63% and 2.98+/-0.59s(-1), p<0.05, respectively) than group B (27.21+/-10.49% and 2.21+/-0.47s(-1)). In addition, peak early diastolic strain/rate were significantly higher in group A (-1.89+/-3.30% and -2.32 +/-0.77s-1 p<0.05, respectively) than group B (-0.83+/-2.79% and -1.77+/-0.25s(-1)). There were no significant differences in C-reactive protein (CRP) and B-type natriuretic peptide (BNP) at the baseline, but BNP were significantly reduced in group A (822.9+/-798.3 pg/mL, p<0.05) than group B (1481.9+/-209.97 pg/mL) at the 12-month follow-up. CONCLUSION: The increased values of atrial peak systolic and diastolic strain/rate after treatment with RAS inhibitors revealed that passive stretching and shortening of LA wall might improve in some patients with AF even before LA size change possibly because of reduced atrial fibrosis and increased compliance. Our results indicated that strain echocardiography provides clinically useful information of LA function and remodeling and treatment with RAS inhibitors appears to preserve LA reservoir function in AF patients without visible LA structural change.

7.Assessment of Environmental Monitoring Items for Mixed Organic Solvents in Work Place.

Ambient air monitoring(environmental monitoring) and biological monitoring of mixed organic solvents exposure have been used in exposure assessment of these compounds in the work environment. Environmental monitoring is more useful when assessing low level exposure to these compounds. The specific objectives of this study were to measure air organic solvent levels in the work environment, to test the agreement of items measured in this study with items which have been reported to the administrative agency, and finally to evaluate the appropriateness of environmental monitoring items. The study subjects were 13 small companies in which less than 300 employees were working and presumably exposed to mixed organic solvents(e.g. automotive repair and painting, 6; textile dyeing, 2; printing, 2; electronics, etc, 3). Personal air samplings were performed by the flow-lite sampling pump(MSA) and the 600mg charcoal tube with air velocity of 100ml/min for qualitative analysis and 2l/min for quantitative analysis. Samples were analysed by gas chromatography/mass spectrometry(Hewlett-Packard 5890/5970). High levels of toluene, n-hexane, and MIBK were detected in most study companies and low levels of ethylbenzene, xylene, n-butylacetate were also detected. When these measured items were compared with those of reported items, only 6 of 13 companies showed relatively good agreement between them. Two companies failed to report items exceeding the limit value. In conclusion, qualitative analysis of ambient air organic solvents seemed to be as important as quantitative analysis of these compounds and a periodic evaluation of monitoring items reported to the administrative agency should be required.

Adrenoleukodystrophy is an inborn error of metabolism characterized by adrenal insufficiency and progressive demyelmation of brain white matter and peripheral nerves. Authors experienced three cases of adrenoleukodystrophy in a 7 year old boy, a 17 and a 210 year old males that were diagnosed by increased plasma content of very long chain fatty acid(VLCFA). The clinical symptoms include seizure, visual impairment, and hemiparesis. Two cases showed typical radiological findings and sural nerve biopsy was performed in one.

9.Linkage Disequilibrium Analysis of Quantitative Trait Locus Associated with Lipid Profiles.

BACKGROUND AND OBJECTIVES : The common methods of genetic association analysis are sensitive to population stratification, which may easily lead to a spurious association result. We used a regression approach based for linkage disequilibrium to perform a high resolution genetic association analysis. SUBJECTS AND METHODS : We applied a regression approach that can increase the resolution of quantitative traits that are related with cardiovascular diseases. The population data was composed of 543 males and 876 females without cardiovascular diseases, and it was obtained from a cardiovascular genome center. We used information about linkage disequilibrium between the marker and trait locus, and we added the covariates to model their effects. RESULTS : We found that this regression approach has the merit of analyzing genetic association based on linkage disequilibrium. In the analysis of the male group, the total cholesterol was significantly in linkage disequilibrium with CETP3 (p=0.002), and triglyceride was significantly in linkage disequilibrium with ACE8 (p=0.037), APOA1-1 (p=0.031), APOA5-1 (p=0.001), APOA5-2 (p=0.001) and LIPC4 (p=0.022). HDL-cholesterol was significantly in linkage disequilibrium with ACE7 (p=0.002), ACE8 (p=0.008), ACE10 (p=0.003), APOA5-2 (p=0.022), and MTP1 (p=0.001). In the female group, total cholesterol was significantly associated with APOA5-1 (p=0.020), APOA5-2 (p=0.001), and LIPC1 (p=0.016), and triglyceride was significantly associated with APOA5-1 (p=0.009), APOA5-2 (p=0.001), and CETP5 (p=0.049). LDL-cholesterol was significantly associated with APOA5-2 (p=0.004), and HDL-cholesterol was significantly associated with LIPC1 (p=0.004). CONCLUSION : We used a regression-based method to perform high resolution linkage disequilibrium analysis of a quantitative trait locus that's associated with lipid profiles. This method of using a single marker, as applied in this paper, was well suited for analysis of genetic association. Because of the simplicity, the method can also be easily performed by routine statistical analysis software.

10.The Therapeutic Effects of the Q-Switched Nd:YAG Laser on Pigmented Lesions.

The Q-switched lasers which was introduced under the concept of selective photothermolysis. Q-switched Nd:YAG laser, which targets dark pigments in a longer wave length (1064 nm) that has less absorption by melanin, can effectively treat deep tattoos with less pigmentary alterations. We report our experience over the past 3 years in treating 565 patients with tattoos and cutaneous pigmented lesions using the Q-switched Nd:YAG laser. Patients were exposed with a pulse duration of 10 ns and fluences of 6, 7 J/cm2(1064 nm) or 12, 14 J/cm2(532 nm), in exposure spots of 2 mm or 3 mm, at intervals of 4 weeks. Q-switched Nd:YAG laser was effective in removing deep pigmented lesion with 1064 nm, and colored inks with 532 nm. Seventy-three percent of amateur black pigmented lesion were > 75% clear after four to ten treatment. Ninty-eight percent of professional black pigmented lesion were > 75% clear after two on eyebrow, six to eight on extremity and trunk. No significant side effects, including pigmentary changes or scarring, were noted.
