Isolated heaptic tuberculous granuloma with no coexistent tuberculosis elsewhere in the body is extremely rare. We report a case of pathologically proven tuberculous granuloma in the liver followed with both IV bolus and portal CT scans. The lesion on preenhanced CT scan showed undefinable isodensity. After IV bolus injection, it showed poor enhancement with central low-density and surrounding hyperdenity due to compensatory hypervascularity of the left lobe of liver in early phase. It showed peripheral rim enhancement in late phase and in delayed phase showed relatively homogeneous but slightly decreased contract enhancement. On portal CT scan, it showed a hypodense portal defect similar to other hepatic mass lesions. During follow-up studies, it was a slowly growing mass which was more easily detectable by prtal CT scans than bolus CT scans.

BACKGROUND: The gaitaids are helpful for the patients of gait disturbance, but there was no study about the quantitative assessment of the effectiveness of gaitaids of several different types. But proper use of gaitaids is important for the disabled who is suffered from falls with or without trauma. The appropriate prescription of gaitaids is coming from the objective assessment for gaitaids. METHODS: We evaluated the 'balance index(BI)', which is originally suggested assessment scale by author, for the five different gaitaids :mono-cane, quad-cane, forearm crutch, axillary crutch, walker We assessed the difference of B.1 with or without using each gaitaids examined by 50 healthy adults and 20 stroke patients. RESULTS: The BI in the normal adult was 34.5+/-2.7, and 21.0+/-6.9 in the stroke patients. The balance index o( walker user was 37.6+/-1.4, which was the highest score. The BI of quad-cane was 30.1 +/-4.8, which was the next highest score and the BI of mono-cane, forearm crutch, axillary crutch were 25.5+/-5.6, 25.5+/-5.7, 25.5+/-5.4, which were lower than that of quad-cane. CONCLUSION: It can be suggested that the walker is the most effective for the support balance on gait and the quad-cane is more effective than mono-cane or crutches for balance support.

This investigation was performed to evaluate Doppler mitral valve flow velocity profiles in hypertensive heart disease for the detection of the left ventricular diastolic dysfunction which preceded the systolic dysfunction. Doppler derived parameters included; the duration of the early diastolic flow velocity peak(DF duration), the downward slope of the early diastolic flow velocity peak (EF slope) and the ratio between the height of the early and the late diastolic flow velocity peak (E-E'/A-A'). Doppler flow velocity profiles were measured in 19 normotensive subjects and 28 hypertensive patients. We found decreased EF slope in hypertensive patients (3.6+/-1.4./sec) in comparison with normotensive subjects (5.2+/-2.4m/sec) (p<0.01). In hypertensive patients, E-E'/A-A' value was significantly lower (0.8+/-0.2) than that of normotensive subjects (1.6+/-0.4)(p<0.001). DF duration was similar in both groups. In conclusion, we found that Doppler mitral valve flow velocity profiles, EF slope and E-E'/A-A', were decreased in hypertensive patients, which may be due to the left ventricular diastolic dysfunction.

OBJECT: In order to compare several Obstetric characteristics and outmmes of teenage pregnancy. METHODS: The 10 year clinical records of 11,583 women undergoing Cesarean section and delivery were reviewed in the department of Obstetrics and Gynecology, Incheon Christian Hospital from July 1, 1989 to June 30, 1999. There were seventy-eight pregnancies under age 20 and we compared them with two hundred pregnancies aged 20-29 who were randomly chosen during the same periods as the control group. RESULT: The following results were obtained. The proportion of birth by teenagers increased from 0.5% of all birth in 1989 to 2.61% in 1999. The most of cases were in high teen group, age 14- 19.The majority(50.5%) were in the age of 19. Seventy two cases(92.3%) had no previous pregnant history. Six cases had one previous pregnant history. The frequencies of prenatal care were significantly lower than that of the control group(p<0.05). The mean gestational period was 36+4weeks, and showed significant difference between the study group and the control group(39+3 weeks). The incidence of preterm labor was significantly different between the two proups(p<0.05). But the incidence of postterm delivery were not significantly different between the two groups. The mean weight gain of pregnant teenagers revealed 10.64kg, significantly less profound than the control group. The matemal weight gain of control group revealed 12.95kg, respectively. The mean birth weight(2.54kg) of infant was significantly less profound than the control group. The incidence of birth of small for gestational age(birth weight 1,500-2,499gm ; 14.5%) was significantly increased among study group than the control group(1%). The rate of abdominal delivery(21.8%) were not significantly decreased than the control group(47%). The incidence of cephalopelvic disproportion and malpresentation for cesarean section were somewhat higher than the control group, and were statistically significant(p<0.05). During the antepartun period, anemia, pyelonephritis, and preterm labor occurred more frequently in the teenage pregnancy than the control group, but not statistically significant. Among postpartum complications, incidence of anemia(51.3%) increased significantly. The perinatal mortality, intrauterine growth retardation, congenital anomaly occurred more frequently than the control group. CONCLUSION: These results show that pregnancy of teenage will be comeout high risk and complications. And then we will be more concern and study about teenage pregnancy.

Background: Endoscopic forehead lifting is one of the most common procedures in the field of upper facial surgery. The upper third of the face determines the facial expression and plays a key role in the appearance of facial youth. After the forehead, a high hairline is one of the most important features of the upper third of the face contributing to age identification. The combined evaluation of these two features should be a basic premise of upper facial surgery. Methods: The authors present a surgical sequence in which endoscopic forehead lifting and lowering of the high hairline by means of a scalp flap advancement are carried out during the same operation. The incision line is located along the hairline. After the scalp and forehead flap are moved, they are fixed using the bone tunnel fixation method. Results: In total, 194 patients were treated with endoscopic forehead lifting and simultaneous hairline lowering between August 2018 and July 2020. On average, the patients’ hairlines were 18 mm lower and their eyebrows were 5 mm higher. No patients had serious complications. Conclusions Endoscopic forehead lifting and simultaneous hairline lowering surgery make it possible to address the entire upper third of the face in a single facial rejuvenation operation.
