3.Effect of Nurses' Job Stress on Job Satisfaction: Mediating Effect of Head Nurses' Emotional Leadership Perceived by Nurses.

PURPOSE: The purpose of this study was to identify the mediating effect of head nurses' emotional leadership as perceived by nurses in the relationship between nurses' job stress and job satisfaction. METHODS: Study participants were 278 staff nurses currently working at two university hospitals in Busan, Korea. SPSS/WIN 21.0 program was used for data analysis to analyze descriptive statistics, t-test, correlation, and stepwise multiple regression. The significance level was set at p<.05. RESULTS: Nurses' job satisfaction had a significant correlation with job stress (r=-.31, p<.001) and emotional leadership of nurse managers as perceived by nurses (r=.53, p<.001). Also, head nurses' emotional leadership experienced by nurses had a partial mediating effect in the relationship between nurses' job stress and job satisfaction. CONCLUSION: The results of this study show that the nurse managers' emotional leadership has an important influence on nurses' job stress and job satisfaction. Therefore, head nurses' emotional leadership is very important for effective human resource management and the hospital should offer education and training to booster head nurses' leadership by developing emotional intelligence to promote staff job satisfaction.

4.Multiple diagnostic approaches to palpable breast mass

The combination of the various diagnostic methods of palpable breast mass has improved the diagnosticaccuracy. From Sep.1983 to Aug. 1985 pathologically proven 85 patients with palpable breast masses examined withX-ray mammography, ultrasonography, penumomammography and aspiration cytology at Korea Cancer Center Hospital wereanalyzed. The diagnostic accuracies of each methods were 77.6% of mammogram, 74.1% of ultrasonomgram, 90.5% ofpenumommaogram and 92.4% of aspiration cytology. Pneumommamograms was accomplished without difficulty orcomplication and depicted more clearly delineated mass with various pathognomonic findings; air-ductal pattern infibroadenoma(90.4%) and cystosarcoma phylloides(100%), air-halo in fibrocystic disease(14.2%), fibroadenoma(100%),cystosarcoma phylloides (100%), air-cystogram in cystic type of fibrocystic disease(100%) and vaculoar pattern orirregular air collection without retained peripheral gas in carcinoma.

5.The Prognostic Factor Analysis and Effect of Adjuvant M-VAC Chemotherapy on Advanced Transitional Cell Carcinoma of the Upper Urinary Tract.

PURPOSE: We evaluated the prognostic factors affecting the patients' survival and analyzed the effect of the adjuvant M-VAC chemotherapy after nephroureterectomy for patients with advanced (T3, T4) transitional cell carcinoma (TCC) of the upper urinary tract (UUT) with compared to surgery only group. MATERIALS AND METHODS: Of 33 patients diagnosed with upper urinary tract transitional cell carcinoma at our institution from 1988 to 1999, 20 patients with advanced transitional cell carcinoma of the renal pelvis and ureter underwent nephroureterectomy and follow-up. 5 patinets were treated nephroureterectomy only, and 15 patients were treated with nephroureterectomy plus adjuvant M-VAC chemotherapy. Prognostic factors such as age, bladder invasion, IVP finding, tumor stage, tumor grade, and tumor multiplicity were investigated. RESULTS: The mean age was 61 years old (38-75). The tumor stage (T3; 10 cases, T4; 10 cases) was not of benefit in predicting survival. Other prognostic factor such as bladder invasion, IVP finding, tumor multiplicity were not correlated with survival rate. The treatment method (nephroureterectomy only group-13.2 months vs. adjuvant M-VAC group-115 months), and tumor grade (GII; 12 cases, 39.2 months vs. GIII; 8 cases, 16.3 months) has significant prognostic value in survival rate. CONCLUSIONS: Many prognostic factors such as age, bladder invasion, IVP finding, stage and tumor multiplicity have no influence on the survival of patients with UUT TCC while tumor grade seems to be correlated with the survival. Survival rate of studied M-VAC group is superior to that of control surgery-only group. So adjuvant M-VAC chemotherapy is the valuable and tolerable treatment moldality in order to prolong the survival time for the patients with high stage UUT TCC.

8.Carbon Monoxide Ameliorates 6-Hydroxydopamine-Induced Cell Death in C6 Glioma Cells.

Carbon monoxide (CO) is well-known as toxic gas and intrinsic signaling molecule such as neurotransmitter and blood vessel relaxant. Recently, it has been reported that low concentration of CO exerts therapeutic actions under various pathological conditions including liver failure, heart failure, gastric cancer, and cardiac arrest. However, little has been known about the effect of CO in neurodegenerative diseases like Parkinson’s disease (PD). To test whether CO could exert a beneficial action during oxidative cell death in PD, we examined the effects of CO on 6-hydroxydopamine (6-OHDA)-induced cell death in C6 glioma cells. Treatment of CO-releasing molecule-2 (CORM-2) significantly attenuated 6-OHDA-induced apoptotic cell death in a dose-dependent manner. CORM-2 treatment decreased Bax/Bcl2 ratio and caspase-3 activity, which had been increased by 6-OHDA. CORM-2 increased phosphorylation of NF-E2-related factor 2 (Nrf2) which is a transcription factor regulating antioxidant proteins. Subsequently, CORM-2 also increased the expression of heme oxygenase-1 and superoxide dismutases (CuZnSOD and MnSOD), which were antioxidant enzymes regulated by Nrf2. These results suggest that CO released by CORM-2 treatment may have protective effects against oxidative cell death in PD through the potentiation of cellular adaptive survival responses via activation of Nrf2 and upregulation of heme oxygenase-1, leading to increasing antioxidant defense capacity.

10.Lumbar Lordosis of Spinal Stenosis Patients during Intraoperative Prone Positioning.

BACKGROUND: To evaluate the effect of spondylolisthesis on lumbar lordosis on the OSI (Jackson; Orthopaedic Systems Inc.) frame. Restoration of lumbar lordosis is important for maintaining sagittal balance. Physiologic lumbar lordosis has to be gained by intraoperative prone positioning with a hip extension and posterior instrumentation technique. There are some debates about changing lumbar lordosis on the OSI frame after an intraoperative prone position. We evaluated the effect of spondylolisthesis on lumbar lordosis after an intraoperative prone position. METHODS: Sixty-seven patients, who underwent spinal fusion at the Department of Orthopaedic Surgery of Gwangmyeong Sungae Hospital between May 2007 and February 2012, were included in this study. The study compared lumbar lordosis on preoperative upright, intraoperative prone and postoperative upright lateral X-rays between the simple stenosis (SS) group and spondylolisthesis group. The average age of patients was 67.86 years old. The average preoperative lordosis was 43.5degrees (+/- 14.9degrees), average intraoperative lordosis was 48.8degrees (+/- 13.2degrees), average postoperative lordosis was 46.5degrees (+/- 16.1degrees) and the average change on the frame was 5.3degrees (+/- 10.6degrees). RESULTS: Among all patients, 24 patients were diagnosed with simple spinal stenosis, 43 patients with spondylolisthesis (29 degenerative spondylolisthesis and 14 isthmic spondylolisthesis). Between the SS group and spondylolisthesis group, preoperative lordosis, intraoperative lordosis and postoperative lordosis were significantly larger in the spondylolisthesis group. The ratio of patients with increased lordosis on the OSI frame compared to preoperative lordosis was significantly higher in the spondylolisthesis group. The risk of increased lordosis on frame was significantly higher in the spondylolisthesis group (odds ratio, 3.325; 95% confidence interval, 1.101 to 10.039; p = 0.033). CONCLUSIONS: Intraoperative lumbar lordosis on the OSI frame with a prone position was larger in the SS patients than the spondylolisthesis patients, which also produced a larger postoperative lordosis angle after posterior spinal fusion surgery. An increase in lumbar lordosis on the OSI frame should be considered during posterior spinal fusion surgery, especially in spondylolisthesis patients.
