1.Allergic Contact Dermatits due to Cambison Ophthalmic Ointment.

Cambison ophthalmic ointment is an easily available topical medicament in Korea, However, the allergic contact dermatitis lo Cambison ophthalmic ointment has not been reported in Korean dermatology literature. We report, a case of allergic contact dermatitis in a 28-year-old female who suffered from itchy, erythematous patches on both periorbital areas. She wore contact lenses and for the last year, she hac applied Cambison ophthalmic ointment to both periorbital areas. The skin lesions were aggravated gradually. A patch test showed positive reactions to Cambison ophthalmic ointment, neomycin, mercury and thimerosal. We suspect neomycin contained in Cambison ophthalmic ointment to be the etiologic agent. When contact dermatitis occurs on periorbital areas, topical ophthalmic ointment or lens cleaner may be considered as a causative agent.

2.Clinical and Histopathological Study of Calcinosis Cutis.

BACKGROUND: Calcinosis cutis may occur when connective tissue is abnormal (dystrophic), or where calcium or phosphate levels in the blood are high(metastatic); alternatively, there may be no obvious underlying cause(idiopathic). The exact incidence of calcinosis cutis in dermatologic patients is not well-kniown and the pathomechanism of it remains to be elucidated. OBJECTIVE: This study was performed to analyze the incidence, clinical and histopathological features of calcinosis cutis. METHODS: We studied 72 patients with calcinosis cutis who visited our department between January 1985 and December 1996. The patients were classified into 3 types (dystrophic, metastatic, and idiopathic) and were analyzed clinically and histopathologically. Results : The results were summerized as follows; 1. There were 60 cases(83.3%) and 12 cases(16.7%) of the dystrophic and idiopathic types, respectively but the metastatic type was not observed. The frequency of dystrophic calcification in individual disorders was 88.2% in pseudoxanthoma elasticum, 66.7% in trichilemmal cyst and dermatomyositis, and 64.6% in pilomatrichoma. 2. In the idiopathic type, it was common in females aged over 50 years and the mean duration of lesions was 2.8+/-2.0 years. The predilection site was the flank(5 cases) and most of the lesions(7 cases) showed skin-colored nodules 3. Histopathologically, calcified material of the idiopathic type was located predominently in the dermis(10 cases) and was present as large masses surrounded by a foreign body reaction. 4. In the dystrophic type, calcified deposits were noted predominently within the shadow cells of pilomatricoma, in the degenerated elastic fibers of pseudoxanthoma elasticum, and in the keratinized area of basal cell carcinoma, squamous cell carcinoma, trichilemmal cyst, and epidermal cyst. Foreign body reactions with giant cells and mononuclear cell infiltrations were often found around large deposits of calcium. Conclusion : The incidence of calcinosis cutis in dermatologic patients is relatively low, but a variety of disorders can be associated with cutaneous calcification. Therefore, dermatologists should be familiar with the different forms of cutaneous calcification and the dermatoses that manifest them.

4.Topographic Anatomic Difference of the Eyelid According to Age in Korean.

PURPOSE: To analyze the morphologic changes of the eyelids among different age groups in Korean, and also th find out factors that influence there changes. METHODS: Tow hundred thirty six subjects without any ocular diseases were selected, and sorted by the age (ranging from teenage to the sixties and above), the gender, and according to the existence of the double fold. The pictures of frontal and lateral side of the subjects were taken. After scanning the pictures, MRD1 (marginal reflex distance 1), MRD2 (marginal reflex distance 2), the vertical distance from upper eyelid to eyebrow, the vertical and horizontal distance from medial to lateral canthus, and the horizontal distance between lateral canthus and corneal center were measured using computer-based AutoCAD R14. The measured distances were compared in each subjects. RESULTS: MRD1, MRD2, the vertical distance between medial and lateral canthus, and the horizontal distance between lateral canthus and corneal center were longest in people of the 3rd and 4th decade, and decreased. The horizontal distance between medial and lateral canthus was longest in people of the 5th decade, and decreased thereafter. The vertical distance between upper eyelid and eyebrow was shortest in people of the 4th decade and increased with aging. CONCLUSIONS: It is essential to acknowledge and understand the morphologic changes of the eyelids according to aging so that more satisfactory surgical outcomes may be achieved in cosmetic eyelid operation.

5.The Effect of Transfer of p21/WAF1 Gene on Kidney and Bladder Cancer Cell Lines.

PURPOSE: To identify antitumor effect, indication and antitumor mechanism of p21/WAF1 gene therapy in human kidney and bladder cancer. MATERIALS AND METHODS: We cloned cDNA of wild type human p21 gene by reverse transcription-polymerase chain reaction(RT-PCR) technology and constructed a mammalian expression plasmid vector carrying cDNA of wild type human p21 and CMV enhancer/promotor(pCMV-p21). pCMV-p21 was administered in vitro in complex with HDL cationic polyliposome into human kidney cancer cells(CURC-1 and CURC-2), bladder cancer cells(RT4 and T24), and liver cancer cells(Hep3B and HepG2), followed by analysis of viable cell number, cellular morphology, cell cycle distribution and development of apoptosis. RESULTS: The expression of p21 was preserved in p53-wild type cells and markedly decreased or absent in p53-mutant cells. Administration of pCMV-p21:HDL complex induced high level expression of p21 and significant suppression of growth in all of the cancer cells examined, regardless of their genetic status of p21 and p53. Transfer of p21 to cancer cells induced not only decrease of proportion of cells in G2+M/S phase but also apoptosis. CONCLUSIONS: HDL cationic polyliposome-mediated p21 gene transfer may become a promising therapeutic modality for human kidney and bladder cancer and that p21 may have a novel function to induce apoptosis to human bladder and kidney cancer cells. Further studies are necessary on in vivo antitumor effect of p21 gene transfer and molecular mechanism of p21 gene-induced apoptosis.

We report a case of histiocytoid hemangioma (angiolymphoid hyperplasia with eosinophilia) in a 60 year-old female patient who was effectively treated with dapsone. The therapeutic effect of dapsone may support the hypothesis that immunologically mediated reactive process plays a role in the pathogenesis of histiocytoid hemangioma.

8.A Case of Captopril-induced Pemphigus Foliaceus.

A 65-year-old male received captoPnl therapy (25mg three times a day, for the treatment of congestive heart failure. Seven months after starting the captopril therapy he developed flaccid vesiculobullae and erythematous patches with oozing, crusted surfaces on his trunk and the proximal portion of his extremities. The mucous membranes were not involved. The mucous membranes were not involved.) The histopathologic findings showed subcorneal bulla containing a few acantholytic cells and direct immunofluorescent studies revealed intercellular deposition of IgG and C3 throughout entire epidermis, consistent with pemphigus foliaceus. After captopril therapy was discontinued, the skin lesions gradually improved within 1 month. Pemphigus foliaceus; is a rare complication of captopril therapy and this is, to our knowledge, the first case report in Korea.
