1.The Effect of Sensory Stimulation on Mother-Infant Interaction in Premature Infants.

PURPOSE: This study was designed to investigate effect of sensory stimulation on the mother-infant interaction in premature infants. METHOD: The subjects of this study consisted of 36 pairs of premature infants and their mothers from NICU of one university hospital located in Taegu, 18 pairs for intervention group and 18 pairs for control group. The data were collected from May, 1999 to October, 2000. For the intervention group Field's sensory stimulation(tactile and kinesthetic stimulation) was applied 2 times a day for 10 days(10:00-11:00 in the morning and 7:00-8:00 in the afternoon by researcher and mother). To determine mother and infant interaction during feeding, tool developed by Kim Mi-Ye(1999) was used. Collected data were analyzed with SAS program using chi-square test and t-test. RESULT: Significant differences were found in mother-infant interaction between two groups(t=-5.38, p=.00). It indicates that sensory stimulation was effective in improving mother-infant interaction. In the aspects of the quality of mother-infant interaction, sensory stimulation was most effective in improving sensitivity to mother and infant's synchronic behaviors(t=-5.43. p=.00) and followed by growth fostering(t=-5.07, p=.00), sensitivity to infant's cues(t=-4.53, p=.00), clarity of infant's cues(t=-3.03, p=.00) and responsiveness to the mother's behaviors(t=-2.14, p= 0.04). CONCLUSION: Based on the result of this study, it is suggested that maternally administered sensory stimulation should be applied clinical practice to improve interaction of premature infants and their mothers.

2.Characteristics of Light-evoked Retinal Ganglion Cell Activity with Postnatal Maturation in SD Rat.

As part of Korean retinal prosthesis project, we have provided preliminary experimental results regarding voltage parameters for the stimulation of chemically degenerated rabbit retina. Since our APB-treated chemically degenerated retina is only ON-pathway blocked, now we switch our experiments to more appropriate retinal degeneration model, genetically degenerated retina model (RD mouse: rd/rd (C3H/HeJ)). Before studying with RD mouse, we started control experiments with normal SD rat to understand characteristics of retinal ganglion cell activity with postnatal maturation in rodents. Ganglion cell activities were recorded with 8x8 multi-electrode array. Moving spontaneous bursts appeared until postnatal day of 15. During pre-eye opening period, no light evoked response appeared. After postnatal day of 2 weeks (post-eye opening period), ON-, OFF- and ON/OFF response appeared. The fractional distributions of ON, OFF, and ON/OFF ganglion cell is about 40%, 50%, and 5%. The percentage (%) of light evoked response in each dorso-temporal, ventral, and dorso-nasal area of eye is about 50%, 37.5% and 12.5%, respectively. We concluded that the optimal period for experiment in rodent is about postnatal day of 2~3 weeks.

3.Factors Influencing Health Literacy in Community-Dwelling Adults.

BACKGROUND: Health illiteracy is a problem often unrecognized by health care providers. It influences medical costs and the health status of adults. The purposes of this study were to determine the level of health literacy in community-dwelling adults and to identify the factors influencing it. METHODS: A cross-sectional survey was conducted in Seoul, Gyeonggi and Chungcheong province. A total of 420 adults aged 18 or older were interviewed by trained nursing students between November 1 to December 30, 2011. Health literacy was measured using the Short Test of Functional Health Literacy in Adults. Data were analyzed using the PASW 18.0 program. RESULTS: The mean score of health literacy was 50.64+/-19.18. In the multiple linear regression analysis, health literacy was significantly associated with education (beta=0.17, P=0.001), alcohol use (beta=-0.12, P=0.010), and perceived health status (beta=0.11, P=0.029). These factors accounted for about 7% of health literacy. CONCLUSIONS: Health literacy is a very important public health issue. Our findings showed that educational level, alcohol use and perceived health status should be considered when assessing this issue in patients. Furthermore, the development of a standardized Korean assessment tool for health literacy and specified interventions for enhancing health literacy are needed to improve health outcomes.

4.Comparison of Retinal Waveform between Normal and rd/rd Mouse.

Retinal prosthesis is regarded as the most feasible method for the blind caused by retinal diseases such as retinitis pigmentosa or age-related macular degeneration. One of the prerequisites for the success of retinal prosthesis is the optimization of the electrical stimuli applied through the prosthesis. Since electrical characteristics of degenerate retina are expected to differ from those of normal retina, we investigated differences of the retinal waveforms in normal and degenerate retina to provide a guideline for the optimization of electrical stimulation for the upcoming prosthesis. After isolation of retina, retinal patch was attached with the ganglion cell side facing the surface of microelectrode arrays (MEA). 8x8 grid layout MEA (electrode diameter: 30micrometer, electrode spacing: 200micrometer, and impedance: 50 k omega at 1 kHz) was used to record in-vitro retinal ganglion cell activity. In normal mice (C57BL/6J strain) of postnatal day 28, only short duration (<2 ms) retinal spikes were recorded. In rd/rd mice (C3H/HeJ strain), besides normal spikes, waveform with longer duration (~100 ms), the slow wave component was recorded. We attempted to understand the mechanism of this slow wave component in degenerate retina using various synaptic blockers. We suggest that stronger glutamatergic input from bipolar cell to the ganglion cell in rd/rd mouse than normal mouse contributes the most to this slow wave component. Out of many degenerative changes, we favor elimination of the inhibitory horizontal input to bipolar cells as a main contributor for a relatively stronger input from bipolar cell to ganglion cell in rd/rd mouse.

METHOD: Data were collected from December 1, 2001 to June 30, 2003. A total of 35 mothers and their infants were assigned to a control(n=17) and an experimental(n=18) group. Mothers in the experimental group received education related to sensory stimulation program and administered to their infants twice a day for 6 months. Both groups were measured the weight, length, head circumference and chest circumference of infants at 6 weeks, 10 weeks, 14 weeks, 18 weeks, and 22 weeks and the mother-infant feeding interaction at 6 weeks, 10 weeks through the home visiting. RESULT: Compared to the control group, the experimental group had significantly larger increases in head circumference for intervention period. In addition to, the experimental group showed significantly higher score in mother-infant interaction than control group. CONCLUSION: These data suggested that sensory stimulation program administered by primipara may improve the physical growth of full term infant and mother-infant feeding interaction.

7.Spatiotemporal Analysis of Retinal Waveform using Independent Component Analysis in Normal and rd/rd Mouse.

It is expected that synaptic construction and electrical characteristics in degenerate retina might be different from those in normal retina. Therefore, we analyzed the retinal waveform recorded with multielectrode array in normal and degenerate retina using principal component analysis (PCA) and independent component analysis (ICA) and compared the results. PCA is a well established method for retinal waveform while ICA has not tried for retinal waveform analysis. We programmed ICA toolbox for spatiotemporal analysis of retinal waveform. In normal mouse, the MEA spatial map shows a single hot spot perfectly matched with PCA-derived ON or OFF ganglion cell response. However in rd/rd mouse, the MEA spatial map shows numerous hot and cold spots whose underlying interactions and mechanisms need further investigation for better understanding.

8.The Perception of Character Education in Medical School

The purpose of this study was to examine professors’ and students’ perceptions of curriculum that fosters character in medical school. ‘Character’ can be defined as a desirable personality and the ability to be a good person. A total of 264 subjects (professors=131, students=133) participated in the study. Survey questions were divided into the three parts (education needs, factors of character, and curriculum management strategy). Data were analyzed by using t-test and one-way analysis of variance. Both professors and students recognized the need for character education. Professors were more aware of the need for education than students (t=4.35, p<0.01), and clinical professors were more aware of the need for education than basic medical science professors (t=3.48, p<0.01). Premedical students were more aware of the need for character-centered education than medical students in the later stages of their education (t=3.41, p<0.01). Professors and students commonly referred to ‘consideration and communication’ as the most important factor in building character. Professors considered ‘self-regulation’ more important than the students recognized, while students perceived ‘wisdom’ as more important than the professors did. There was a difference in preference for curriculum development (creating new subjects vs. revising existing subjects) between the two groups. However, both groups agreed on the teaching and evaluation methods. In conclusion, both groups acknowledged the need for character education. However, there were differences in perception on the major factors of character and preference for curriculum development. The results of this study may assist in designing character education in medical education.
9.Sodium intake trend and current intake level by meal provision place among the citizens of Seoul

BACKGROUND/OBJECTIVES:
The diversity of meal provision places has increased in recent years and sodium intake can vary depending on where meals are eaten, particularly in large cities. In this study, an analysis of the recent trends in sodium intake was performed and a comparison of sodium intake level according to meal provision place among citizens of Seoul was performed.
SUBJECTS/METHODS:
Data from a 24-h recall dietary intake survey from the 2010–2019 Korea National Health and Nutrition Examination Survey (KNHANES) were used in order to determine the trends in sodium intake among citizens of Seoul, aged 3–74 years old. (n = 11,811). The trend of daily sodium intake was presented in absolute amount and proportion compared to the chronic disease risk reduction intake (CDRR) for each selected characteristic. A comparison of sodium intake level according to meal provision place by sex and age groups as a total amount per meal (mg), density per meal (mg/1,000 kcal), and proportion of the daily sodium intake was performed using the 2016–2019 KNHANES.
RESULTS:
Sodium intake levels showed a downward trend from 2010–2019. The highest level of sodium intake was observed for subjects aged 30–49, and the level for males aged 30–49 was 202.8% higher than the CDRR. Results of the analysis of sodium intake per meal according to meal provision place showed that the highest sodium intake was in the order of restaurant meal (RM) > institutional foodservice (IF) > home meal (HM) > convenience food (CF). A higher sodium density (mg/1,000 kcal) was observed for IF compared with RM in most adults. Adults aged over 50 years old consumed more than half of the daily sodium in HM.
CONCLUSION
Significant variation in the level of sodium intake was observed according to sex and age groups, therefore, different approaches and nutrition policies based on meal provision place are needed.
10.A Case of Benign Abducens Nerve Palsy of Childhood.

Benign abducens nerve palsy is rare in children. Identifiable causes of abducens nerve palsy include neoplasm, elevated intracranial pressure, infection and trauma. Isolated abducens nerve palsy with unknown etiology is classified as benign or idiopathic. The diagnosis is made by excluding underlying pathologies. Prognosis is favorable. Most patients have been found to recover spontaneously within 6 months. Recurrent palsy is observed in some patients and is more pronounced in younger girls with left-sided palsy. Even the recurrent cases, however, are still benign. We report a case of benign abducens nerve palsy presenting diplopia and headache with normal results from MRIs and microbiologic studies. The patient underwent rapid, spontaneous recovery.
