1.A Case of Asymmetric Septal Hypertrophy Combined with Conn's Syndrome.

We report a case of a 44 year old femele with unilateral aldosterone-proudcing adrenal adenoma characterized by hypertension, plasma aldosterone excess, and low plasma renin, commonly but not invariably with hypokalemia. She also had asymmetric septal hypertrophy of left ventricle established with two-dimensional echocardiography. The electrocardiogram showed inverted T wave and prominent U wave with high QRS voltage on precordial leads. In the case of this patient, we are not sure whether asymmetric septal hypertrophy was caused by secondary hypertension and chronic aldosterone excess of primary aldosteronism, or hypertrophic cardiomyopathy per se, so further long=term follow-up is required to determine it. Following the successful unilateral adrenalectomy, however, the systemic pressure fell down to the normal level and electrolyte abnormalities were corrected immediaterly within a few days and the modest regression in septal hypertrophy was noted in one year, suggesting that the promary aldosteronism contributes to the development or porgression of asymmetric septal hepertrophy.

2.Successful hybrid operation of an acute mobile thrombus in the abdominal aorta induced by chemotherapy.

Acute mobile thrombus of the abdominal aorta after chemotherapy is a very unusual finding, which can be a potential source of arterial embolism. We report here on a case of an acute mobile aortic thrombus with renal infarction. We successfully treated the patient with hybrid operation-open surgical and endovascular approach. Our case shows that hybrid treatment using wire-directed balloon catheter thrombectomy is a feasible, minimally-invasive treatment for a mobile aortic thrombus.

4.Evaluation of Cobas Integra 700 and Distribution of High Sensitivity C-reactive Protein Levels in Koreans.

BACKGROUND: Prospective studies suggest that C-reactive protein (CRP) is a novel and indepen-dent risk factor of coronary heart disease. High-sensitivity methods for CRP (hs-CRP) measurement are needed for this purpose. In this study, we evaluated the performance of Cobas Integra 700 (Roche Diagnostics, Basel, Switzerland) and aimed to establish the distribution of serum CRP concentrations in healthy Koreans with the hs-CRP method. METHODS: The subjects of this study were 853 males (age; 45.9 +/- 9.8 years) and 546 females (age; 45.7 +/- 10.8 years) who visited the Health Promotion Center of the Asan Medical Center for routine health checks. Hs-CRP was measured with the Cobas Integra 700. The within-day precision, the between-day precision, the degree of correlation, the linearity, and the detection limit of hs-CRP were also analyzed with the Cobas Integra 700. RESULTS: The within-day coefficients of variation (CVs) were 0% and 1.3%. The between-day CVs were 4.0% and 3.0%. The percentage of relative nonlinearity was 0.78%. The correlation coefficient of the Cobas Integra 700 with the Behring Nephelometer was 0.994. The lower detection limit was 0.064 mg/L and the functional detection limit was 0.472 mg/L. The geometric mean concentration of CRP in Koreans was 0.69 mg/L. The mean CRP of the male population was 0.79 mg/L, which was higher than that of Korean females (0.54 mg/L) (P<0.0001). There was a significant trend to higher CRP values with increasing age (P<0.05). CONCLUSIONS: The linearity, the precision, the correlation, the detection limit of the Cobas Integra 700 were satisfactory. The age and sex-specific guidelines for hs-CRP would make the early intervention for coronary heart disease possible.

5.A Case of Pacemaker Syndrome.

Although ventricular pacing alone initially had deemed adequate for most clinical situations, some patients did not do well after ventricular pacing was initiated, and developed various symptoms attributed to this mode of pacing. The pacemaker syndrome is complex of clinical signs and symptoms related to the adverse hemodynamic and electrophysiologic consequences of ventricular pacing in the absence of other causes. Neurologic symptoms or those congestive heart failure predominated. We recently experienced a case of pacemaker syndrome in a 44-year-old female who had suffered sick sinus syndrome and was implanted with dual chamber pacing system being programmed to VVI pacing. She complained of chest discomfort, dyspnea, and near-fainting in a day after being programmed to VVI. Blood pressure was decreased to 9/60mmHg. Electrocardiography showed toPwave onT wave, representing retrograde ventriculoatrial conduction. The symptoms and signs were disappeared immediately after the pacing system was programmed to DDD pacing.

6.A Case of Central Retinal Artery Occlusion after Chiropractic Manipulation of the Neck.

Here we report a case of central retinal artery occlusion after chiropractic manipulation on the neck. A 49-year old man presented at the hospital because of sudden visual loss in his right eye after chiropractic neck manipulation. He had received chiropractic manipulation of the neck by a chiropractor eight days prior. When he first visited us, his best corrected visual acuity in his right eye was hand motion. A full ophthalmic examination was performed. There was cherry-red spot in the macula in his right eye. We performed a fluorescein angiogram and cervical color Doppler. The arterio-venous transit time in the fluorescein angiogram was delayed, and we detected stenosis of the right internal carotid artery with diffuse atherosclerotic plaques in the right common carotid artery. We prescribed ginko biloba extract (Tanamin). Three years after his first visit, the best corrected visual acuity of his right eye was 20 / 200.

7.A Case of Retinal Injury During a Laser Show.

PURPOSE: To report a case of macular injury after exposure to a high energy laser beam used in a laser show. CASE: A 19-year-old female presented 2 days after exposure to a high energy laser beam at a laser show in a night club with decreased vision in her right eye. The patient's best corrected visual acuity of the right eye was hand motion. Fundus examination reveald a retinal swelling in the macular area approximately 5 disc diameter in size and a submacular hemorrhage. Fluorescein angiography of the right eye showed marked hypofluorescence in the macular area and optical coherence tomography (OCT) showed a neurosensory retinal detachment with a macular edema. Three years after exposure, the visual acuity of the right eye improved to 20/600. The fundus revealed scar and depigmented area at the macula. CONCLUSIONS: High-energy laser devices at laser shows should be used carefully with safety education and strict control and can provoke irreversible eye damage if not managed adequately.

8.Predictors of Acute Thrombotic Occlusion after Coronary Intervention in Acute Myocardial Infarction.

BACKGROUND: The most important acute complication of percutaneous transluminal coronary angioplasty (PTCA) is abrupt closure by dissection and thrombus, which account for the majority of deaths and emergency coronary artery bypass procedures associated with PTCA. We sought to determine the relationship between clinical, angiographic characteristics and abrupt thrombotic closure related to coronary intervention. METHODS: One hundred thirty two patients (61.6+/-8.0 year, 98 male) underwent PTCA or stenting under the diagnosis of acute myocardial infarction were analyzed at Chonnam University Hospital between Jan '97 and Jun '98. Patients were divided into two groups, one, 14 patients (Group A, 61.7+/-8.0 year, 9 male), who developed thrombotic occlusion, and the other, 118 patients (Group B, 61.5+/-8.0 year, 89 male) who did not develop abrupt closure related to the coronary intervention. RESULT: There were no significant differences in age, sex, risk factors, activated partial thromboplastin time, fibrinogen, erythrocyte sedimentation rate, C-reactive protein, location of lesion, branch involvement, lesion severity, AHA/ACC morphology between two groups. The incidence of intra-coronary thrombus was greater in Group A than in Group B (44% vs. 2%, p=0.025). Acute thrombotic occlusion related to the coronary interventions developed more frequently in the lesions within two days after the symptomatic onset (55% vs. 19%, p=0.035) and in the right coronary artery (RCA) lesions (55% vs. 24%, p=0.041). CONCLUSION: Predictors of abrupt thrombotic occlusion during coronary intervention in patients with acute myocardial infarction are intracoronary thrombus, earlier intervenion within 2 days after onset of aucte myocardial infarction and RCA lesion.

9.Clinical Experiences of Coronary MAC (Maximum Arterial Re-Creation) Stent.

BACKGROUND AND OBJECTIVES: We previously reported the effects of MAC (Maximum Arterial Re-Creation) stent on stent restenosis in a porcine model. The clinical trial was performed in patients with ischemic heart disease after MAC stent implantation. MATERIALS AND METHOD: We analyzed the clinical and angiographic results in 20 patients in 22 lesions (15 M, 5 F, 59+/-11 year), who underwent MAC stent at Chonnam University Hospital between Nov '97 and Aug '98. Clinical diagnosis was 13 unstable angina (65%), 6 acute myocardial infarction (30%) and 1 old myocardial infarction (5%). RESULTS: Indications for stent were 3 de novo lesion (13.6%), 7 restenosis (31.8%), 8 suboptimal angioplasty result (36.4%) and 4 bail-out procedure (18.2%). Target stented coronary arteries were 15 left anterior descending coronary arteries (67.3%), 2 left circumflex coronary arteries (9.1%) and 5 right coronary arteries (22.7%). Morphologic types were 13 type B1 (59.1%), 5 B2 (22.7%) and 4 C (18.2%). Minimal luminal diameter (MLD) before stent was 0.75+/-0.35 mm and percent diameter stenosis (DS) was 75+/-11.5%, which were improved 2.97+/-0.28 mm in MLD and 2.79+/-5.4% in DS after stent. MAC stent was placed successfully in all patients and one case of acute stent thrombosis was improved after ReoPro infusion. Mean follow-up period was 5.2+/-3.2 months and 100 % event-free survival was observed in all patients. Follow-up coronary angiography was performed in 5 patients and showed no stent restenosis. CONCLUSION: The MAC stent is one of effective and safe devices in the treatment of coronary artery diseases without significant complications and target vessel revascularization.

10.Color Doppler Imaging of Hepatic Tumors.

PURPOSE: To evaluate the color Doppler imaging patterns of hepatoma, hemangioma, and metastasis. MATERIALS AND METHODS: The study comprised 37 patients with liver mass lesions: 12 patients had hepatoma, 15 had hemangiomas and10 had metastatic liver cancers. All 37, who showed hepatic mass on gray scale ultrasonogrphy, were analyzed prospectively. Color mapping of tumors was performed for these patients, and the fastest or next fastest vessel was spectrally analysed to show blood pattern and to measure peak velocity. RESULTS: In all the 12 patients with hepatoma, spectral analysis revealed exteusive pulsating arterial blood flow within the tumor ; peak velocities were 7~65cm/sec(mean 34cm/sec). In ten hemangiomas(66%) color mapping revealed blood flow within the tumor and spectral analysis revealed this possibility in eight of the ten tumors. In none of these patients was a pulsating continous wave observed ; peak velocity was 5~21cm/sec(mean 12.8cm/sec). In no mestastatic tumors was blood flow observed. CONCLUSION: Color Doppler imaging is helpful in the differentiation of hepatoma, hemangioma, and metastatic tumor in the liver.
