3.Human Caliciviruses in Korea: A New Prevalent Group Defined by RNA-Dependent RNA Polymerase Diversity.

Human caliciviruses (HuCVs) cause sporadic cases and outbreaks of acute gastroenteritis (AGE). Three maior genogrovps of HuCVs have been described including the Norwalk virus (NV)-, the Snow Mountain virus (SMA)-, and the Sapporo-genogroups. This study describes the detection and genetic variation of HuCVs from hospitalized infants with AGE in Korea by RT-PCR and sequencing. The cDNA fragments of 206 to 470bp corresponding to the region of 3 primer pairs (36/35, 35/51 or 3/51) in the polymerase region of NV were generated. Of 185 stools screened, 8% were positive by RT-PCR and their sequences showed that all strains contained the GLPSG and YGDD motifs which are conserved for HuCVs. Amino acid (aa) sequence analysis showed that these strains can be divided into 3 maior genogroups. High conservation was observed in that one strain shares 100% of as sequence with Southampton virus, another shares 99% with the Sapporo virus, and six strains share 90 to 95% with Snow Mountain virus. However, significant sequence variation was also found in other strains. This study indicates that all maior genogroups of HuCVs are circulating in Korea.

5.Analysis of Histological Findings and Estrogen and Progesterone Receptor Status in Luteal Phase Defect Caused by Delayed Ovulation and Clomiphen Treatment.

Luteal phase defect (LPD) is an ovulatory disorder of considerable clinical importance that leads to delayed endometrial maturation and inadequate endometrium for blastocyst implantation. This disorder is implicated in infertility and recurrent spontaneous abortion. We analyzed the endometrial histological maturation, and the estrogen receptor(ER) and progesterone receptor(PR) status in patients with unexplained delayed ovulation, and in patients with ovulation induction by clomiphen citrate. The purpose of this study was to determine whether the length of the follicular phase influences the endometrial histological maturation and the hormonal receptor status, and to know the effect of clomiphen citrate on the endometrium. In the delayed ovulation group, the endometrium was characterized by an irregular outpouching and dilated lumina of the glands, a decreased secretory activity and predecidualization, and a decreased number of granulocytes in the stroma. In the clomiphen citrate-ovulation induction group, glandular proliferation was markedly diminished with poorly convoluted and narrow glands, secretory activity was decreased, stroma was undecidualized, and there was an absence of granulocytes. ER expression was increased in the glandular epithelium in the delayed ovulation group, and both ER and PR expressions were markedly decreased in the glandular epithelium and stromal cells in the ovulation induction group. Endometrial maturation and differentiation may be diminished by increased estrogenic and relatively deficient progesterone effects in patients with delayed ovulation. The hormonal receptor status might also be influenced by hormonal changes. Clomiphen citrate successfully induced ovulation in patients with delayed ovulation, however, endometrial proliferation and maturation were markedly suppressed. This might be related to the lower pregnancy rate and higher abortion rate in patients with clomiphen citrate induced ovulation.

6.Use of microplate on flxation of orbital rim fracture.

The orbitozygomatic area occupies a key anatomic position in midface, is prone to injury, and plays a prominent role in facial aesthetics. So the facial appearance including orbital shape can be altered by complications of orbitozygomatic fractures. Most possible initial complications include blindness, hyphema, retinal detatchment, and paralysis or entrapment of extraocular muscles. Long term sequelaes include infraorbital nerve dysfunction, loss of malar projection, enophthalmos, and dystopia. Accurate anatomic reduction and rigid fixation is essential for management of orbitozygomatic fractures to minimize those late sequelaes. Conventional fixation devices to fix displaced fracture of facial bone are interosseous wire and miniplate. But interosseous wirings are unstable for primary bone healing and time consuming. Miniplates have great deal in rigid fixation but their high profile and palpability are the main complaints in many patients, especially in orbital rim area. In this article, we reviewed the 30 cases of zygomamaxilla complex fractures with orbital rim fracture fixed with microplates, and discussed the stability of microplate and superiorities in final aesthetics result. The use of microplates in these area permits enough stability of fracture segment with ease of procedures and superiorities in final results without any palpability.

7.The healing of membranous bone of rabbit after osteotomized by nd-yag laser.

The osteoplasties of maxilla and mandible as the craniomaxillofacial surgery is popular and markedly developed in technically. In traditional osteoplasty, we have been use the mechanical saw, burr, cutting bar which is big sized instrument to transmit the energies to the saw or burr tips. So it is technically difficult to use such big instrument in narrow operative field Recently, the development of laser technology makes wider influence in the medical field. So, the laser is applying to high energy-using osteotomy in addition to hemangioma, tatoo, laserbrasion, hypertrophic scar and endoscopic surgery. The objective of the present study is to evaluate the extent of damage of bone and subsequent healing between the conventional mechnical saw osteotomies and the ND-YAG laser used osteotomies on the rabbit mandible angles by the histologic examination. Fifteen New Zealand white rabbits were used for the study. After exposure of rabbit's mandibular angle, one side of mandible angles were osteomized used by Nd-YAG laser and the other by the saw randomly. We compared the extent of damage and healing of laser and saw osteotomies at immediate, 2 week, 4 week, 3 months, 6 months postoperatively. The result revealed that, in two weeks after operation, the one of damaged bone was looks wider in laser osteotomies sites than by conventional mechanical saw osteotomies sites, but there is not remarkable differences between the laser and saw used group after 4 weeks, and we believe that it is possibility to use alser in craniomaxillofacial field saftly in near future.

8.Effect of Radiation on Cultured Human Normal Keratinocytes and Melanocytes.

BACKGROUND: Radiation has been used in t,he medical field of dragnosis and treatment. There is widely used ionizing radiat:ion such as naturally occuring r-rays or machine-made X-ray. This radiation is able to induce the structural and functional alterations of the mammalian cells. But we have few detailed reports on the effects of radiation on epidermal cells and their immune functions. OBJECTIVE: This study was performed to investigate the effect of radiation on cultured human keratinocytes and melanocytes. MATERIALS AND METHODS: Cultured human keratinocytes and melanocytes were irradiated with 2,6, l0Gy from a Co saurce and stimulated by 100 U/ml of ekratinocyte immediately after irradiation. We investigated cell numbers and morphological changes, DNA synthesis and HLA-DR antigen expression. RESULTS: After exposure to r-ray, the proliferation of keratinocytes and melanocytes decreased in a time and dose dependent fashion to each control group. Tliey showed decreased density, a larger size and a round appearance after radiation exposure and an especially shortened and decreased number of dendrites in the melanocytes. In DNA synthesis counted using [H]-thymidine incorporation, the keratinocvtes decreased values in a dose depen(lent manner at 24 and 72 hours after irradiation but no differense was observed at 168 hours. In melanocytes, there was a greater decrease than that of keratinocytes. The melanin content/cell in all radiation exposed groups increased in a time and dose dependent fashion t,o each contr ol group. HLA-DR antigen expression on keratinocytes after radiat,ion exposure decreased to the control group, but there were no significant differences acccirding to the dose of radiation, And there were no significant diifferences of HLA-DR antigen expression on the melanocytes betweer. controls and the radiation exposed groups. CONCLUSION: Antiproliferative activity was dependent on the exposure time and dose of r-ray exposure. According to the time after radiation exposure, melanogenic activity was stimulated. The expression of HLA-DR, antigen decreased in keratinocyte after radiation exposure but there was no decrease in melanocytes.

10.Seasonal variation of blood pressure in patients with hypertension.

BACKGROUND: Seasonal variation of blood pressure (BP) is well documented. It means that season should be considered in diagnosis and treatment of hypertension. This study was done to evaluate seasonal influence on blood pressure of hypertensive patients who had been taking the same antihypertensive medicine for a long time. METHODS: In medical records, we reviewed the blood pressure recordings of 328 hypertensive patients who had been taking the same medicines through the consecutive summer (Jun., Jul., and Sep.) and winter(Dec., Jan., and Feb.) period. We evaluated the blood pressure difference between summer and winter, and also evalauated influence of age, sex, BMI, smoking, alcohol drinking and exercise on seasonal difference of blood pressure. RESULTS: An average of four blood pressure readings per patient, two for summer and two for winter were recorded. Ambient temperature was over 22degrees C in summer and below 4degrees C in winter. The systolic blood pressure in summer and winter were 135.5mmHg and 140.5mmHg, respectively, and the diastolic blood pressure in summer and winter were 85.0mmHg and 87.4mmHg, respectively. Both systolic and diastolic blood pressure increased significantly in winter. Both in systolic and diastolic blood pressure, sex, smoking, alcohol drinking, exercise did not have an influence on seasonal difference. Age and BMI did not have an influence on seasonal difference of systolic blood pressure. But in diastolic blood pressure, age and BMI had an influence on seasonal difference and elderly patients had a greater seasonal difference in diastolic blood pressure than younger patients. CONCLUSIONS: In hypertensive patients who had taken the same antihypertensive medicines for a long time, both systolic and diastolic blood pressure was significantly creased during the winter. Therefore season should be considered in the treatment of hypertension even though patients are stable with the same medicines.
