1.Clinical Study of 326 Cases of Vitiligo.

We evaluated the clinical manifestations of 326 patients of vitiligo who had visited Seoul National University Hospital. The results were as follows : 1) There were 141 males (43.3%) and 185 females (56.7%). 2) The mean age of onset was 19.6 years (male : 18.3 years, female : 20.6 years). 3) The mean age at the first visit wss 23.9 years (male : 22.5 years, female : 24.9 years). 4) Duration of disease was less than 2 year in 156 patients (47.9%). 5) The most common site of initial involvement was face (36.2%). The common sites of involvement were face (55.2), abdomen (32.5%), neck (26.7%) and scalp (24.8%) in decreasing order of frequency. 6) There were 167 cases (51.2%) of generalized type, 118 cases (36.2%) of localized type, 31 cases (9.5%) of dermatomal type and 10 cases (3.1) of halo nevus. The most frequent site of dermatomal type was neck. Gray hair was noted in 91 cases (27.9%) and mucosal involvement was present in 43 cases (13.2%). 7) Family history was obtained in 24 cases (7.4%). Koebner phenomenon was present in 48 cases (14%). There were no precipitating factors in 309 cases (94.8%) prior to development of vitiligo. 8) The associated diseases were 1 csse of thyroiditis and 3 cases of alopecia areata.

3.A Case of Type IV Solar Urticaria.

Solar urticaria is a rare skin disease characterized by urticarial reactions immediately following exposure to sunlight or artificial radiation. A 24-yr-old man had solar urticaria that was activated by visible light. Positive result of passive transfer of the reactivity with the patients serum to the skin of normal recipient suggested that his condition was attributable to an allergic response. Result of reverse passive transfer studies was negative. We report a case of solar urticaria which appeares to belong to type N in the classification of Harber.

4.The frequency of the cutaneous problems and the influence of hemodialysis in patients with chronic renal failure.

The purpose of this study was to examine the frequency of eutaneous disorders in patients with chronie renal failure presently on hemodialysis(HD). The results were as follows. 1. Cutaneous lesions were present in all 78(100% ) patients with chronic renal failure. 2. The frequency of msjor cutaneous problems associated with chronic renal failure was as follows: xerosis(82.1%), pruritus(74.4%), nail changes(74.4%), hyperpigmentation(70.5 % ), xerostomia(42.3'Yo), poor wound healing(37.2%), easy bruisability(30,8%), hypotrichosis(21.896), and purpura(14.1 % ). 3. Cutaneous problems which appeared after the initiation of HD were. appearance of new pigmented nevi or lentigines(9 patients), appearance or aggravation of acne(6), contact dermatitis at AV fistula site(2), gynecomastia(1), extensive flat warts(1), extensive tinea versicolor(1), and extensive vitiligo(1). 4. Among 55 patients with hyperpigmentation, 11 patients reported decreased pigmentation following HD. In 15 patients, hyperpigmentation worsened following HD, and in 9 patients hyperpigmentation first appeared after HD was initiated. 5. Cutaneous disorders favorably affected by HD were as follows: easy bruisability(3/ 24), xerostomia(3/33), gingival friability(2/7), and hypotrichosis(5/17). 6. Poor wound healing and xerosis were not improved by HD. 7. Among 58 pruritic patients, 7 patients were improved after HD, 7 patients became worse during each HD, and 3 patients were not pruritic only for 2 to 3 days after each HD. Four patients experinced pruritus only during each HD. 8. There was no significant statistical difference between the frequency of pruritus and xerosis and the levels of blood urea nitrogen, calcium, phosphorus, and parathyroid hormone.

5.A Case of Hyperimmunoglobulinemia E Syndrome.

The hyperimmunoglobulinemia E (Jobs) syndrome (HIES) is characterized by marked elevated levels of IgE, recurrent cutaneous and systemic staphylococcal infections, atopic-like dermatitis, and defective neutrophil chemotaxis. Three cases of HIES have been reported in Korea, but not in the dermatology literature. We report a case of HIES with cutaneous infections and MRSA (methicillin-resistant Staphylococcus aureus). A 15-month-old girl presented with intractable pruritic excoriated papular pustular skin lesions and multiple subcutaneous abscesses. Surgical drainage of the abscesses and a course of antibiotic treatment in addition to topical steroids for about 7 weeks resulted in a remarkable improvement.

6.A Clinical Observation in 14 Children with SLE.

To elucidate the clinical characteristics of childhood onset systemic lupus erythematosus (SLE), we carried out a retrospective reivew on medical records of fourteen patients who were diagnosed as SLE on the base of "the 1982 revised criteria for the SLE"(by american Rheumatism Association) at the Department of pediatrics, Hanyang University Hospital from January 1980 through August 1992. The results were summerized as follows: 1) Eleven girls and three boys were recruited in our study, The eight of them were early teens at the time of SLE diagnosis. 2) The frequencies of their clinical manifestations on admission were persistent facial rashes, edema, hepatosplenomegaly, renal abnormalities were proteinuria and hematuria. 3) Immunologic abnormalities were noted as follows: Positive ANA 100%. positive LE cell 64%, elevated anti DS-DNA antibody 64%, and decreased only C3 level 28%, decreased both C3 & C4 64% of the patients. 4) Serum anti-DS DNA(>10units) titer was increased in clinical exacerbated three patients and decreased in four resolved. 5) The high ANA titers( >1;80)obtained in eleven and/or homogenous pattern shown in seven strongly suggested SLE. 6) Renal biopsies of the four patients revealed lupus nephritis and three of the four patients had lupus band in their biopsied skin. 7) Three of 14 patients were recoverd spontaneously with supportive care. Other 11 patients were treated: 7 with prednisolone only, 4 combined prednisolone and other agents. After treatment five were clinically improved. But 4 patients were frequently exacerbated over 2 years, and 2 patients were died of their complications. In conclusion, serum anti-DS DNA level and ANA may be useful for the screening of SLE and for predicting clinical course and prognosis of SLE.

7.The significance of nucleolar organizer regions in bladder epithelial lesions induced by N-butyl-N-(4-hydroxybutyl)nitrosamine in rat.

It is known that the activity of tumor cells correlates with the malignant potential of tumors. Nucleolar organizer regions(NORs) are loops of DNA which occur in nucleoli and which possess ribosomal DNA genes. Ribosomal DNA genes transcribe to ribosomal RNA by RNA polymerase I and are of vital significance in the ultimate synthesis of protein. Proteins associated with the NORs are stained with silver nitrate(Ag-NORs). Ag-NORs were studied in various tumors and might reflect the activity of cells and might be and indicator of the degree of malignancy in tumors. 0.05 % N-Butyl-N(4-hydroxybutyl) nitrosamine(BBN) in drinking water was given for 8 and 14 weeks to induce pre-neoplastic and neoplastic vesical epithelial lesions in female Wister rats. Ag-NORs were stained by the simple one-step silver colloid staining in routine processed, formalin-fixed paraffin sections of bladder lesions induced by N-butyl-N(4-hydroxybutyl) nitrosamine(BBN) in rat. The mean number of silver stained NORs(Ag-NORs) was as follows : normal transitional epithelium was 1.64+/-0.25(mean+/-SD; n=10), simple hyperplasia was 2.24+/-0.24(n=7). nodular hyperplasia was 2.63+/-0.12 (n=6), transitional cell papilloma was 3.24+/-0.28(n=6) and transitional cell carcinoma was 4.52+/-0.32(n=10). Based on the above results, we concluded that the mean number of Ag-NORs showed a stepwise increase from normal transitional epithelium through simple hyperplasia and nodular hyperplasia to papilloma and carcinoma of bladder epithelium induced by BBN in rat and may reflect proliferative activity of cells. Further investigations may be needed to confirm whether the number or quantification of Ag-NORs is a useful method for evaluating proliferative activity of neoplastic lesions or another indicator of prognosis in human bladder cancer.

8.A Case of Papillary Tubular Adenoma (Tubulopapillary Hidradenoma).

Tubular apocrine adenoma and papillary eccrine adenoma are rare sweat gland neoplasms that appear as a small solitary lesion on the scalp or extremities, respectively. Although these lesions are thought to be distinct entities, there are enough similarities between them to group them under the term tubulopapillary hidradenoma or papillary tubular adenoma. We describe a case showing many tubular structures with papillary projection, syringocystadenoma-like structures, and eccrine hirocystoma-like structures in the axillary area. The term of papillary tubular adenoma or tubulopapillary hidradenoma may be prefered in this case.

9.Recurrence of Acne after Oral Isotretinoin.

BACKGROUND: The clinical efficacy of oral isotretinoin in the treatment of severe acne is well established but there is no published data about the recurrence rate and the optimum dose for lowest recurrence in Korean patients. OBJECTIVE: The objective of this study was to determine the recurrence rate after one course of isotretinoin in oriental cases. METHODS: Sixty eight cases were available which fulfilled the inclusion criteria ; clearing af- l ter one course more than 10 weeks and at least two year follow-up thereafter. Isotretinoin was used with oral dosage 0.3-1 mg/kg/day (0.6±0.1) with duration from 10 to 46 weeks (19.7±6.2). RESULTS: Overall, 78% of patients had relapsed with most requiring topical antibiotic therapy only. In patients receiving more than 120 mg/kg, the percentage without recurrence was 75 % which was significantly higher (p<0.001) in comparison with only 15 % among patients receiving less than 120 mg/kg. Doses between 100 and 120 mg/kg were not as effective as those ones with more than 120 mg/kg for preventing recurrence. CONCLUSION: The results indicate that moderate doses of isotretinoin are well tolerated and pro-duce excellent clearing. However, in order to minimize recurrence rate, the optimum dose may be at least over 120 mg/kg of the cumulative dose which means more than 6 months of treatment in most of the Korean patients with acne.

10.A Case of Malignant Fibrous Histiocytoma.

A 57-year-old Korean man with malignant fibrous histiocytoma (MFH) on the leg is presented. Histopathologic examination was consistent with a pleomorphic -storiform subtype of MFH with highly pleomorphic cellular features and frequent mitosis. According to recent evidences, MFH might be a heterogenous group of poorly differentiated tumors rather than a distinct entity. Our case was only positive to CD68 and vimentin immunohistochemically.
