1.Fibroelastolytic Papulosis of the Neck.

Fibroelastolytic papulosis of the neck(FEPN) was introduced by Balus et al in 1997. Before this term was used, white fibrous papulosis of the neck(WFPN) and pseudoxanthoma elasticumlike papillary dermal elastolysis(PDE) had differently been used. These two disease entities had clinical similarities such as asymptomatic, white to yellow discolored, papular eruptions on the neck in elderly persons. As names implicated, however, they showed variable histological findings, for examples, fibrosis in WFPN or elastolysis in PDE or both in FEPN. Taken into ac-count together, instead of using WFPN or PDE separately, FEPN might be preferred to describe these two kinds of skin diseases. We report a case of a 36-year-old male patient with papules compatible with FEPN, whose biopsy mainly showed elastolysis.

3.A Clinical Observation of Primary Epithelial Skin Cancers.

BACKGROUND: The incidence of primary epithelial skin cancers is rising steadily, but little is khown of the clinical features of primary epithelial skin cancers, and limited data are available in Korea. OBJECTIVE: Our purpose was to analyze the clinical characteristics of primary epithelial skin cancers. METHODS: A retrospective analysis was conducted of 106 cases of primary epithelial skin cancers observed between 1989 and 1995. RESULTS: 1. Basal cell curcinoma(BCC), 41 cases, was the most common skin cancer. The numbers of cases of squamosis cell carcinoma(SCC) and Bowens disease(BD) were 33 and 24, respectively. There were 3 cases of mixed type(BCC + SCC), 2 cases of Pagers disease and 3 cases of extramammary Pgets disease. 2. The average age at ciragnosis and the mean duration of disease were as follows . BCC(65.5 years old 4.34 years), SCC(70.2 years old-1.63 years), BD(59.3 years old-2.85 years) 3. The face was the most common anatomic site for BCC(93%) and SCC(70%), but the trunk was the most common site for BD. 4. The most common c inical type of BCC was the noduloulcerative type(65% ) and the solid type was t.he most frequert histologic type of BCC. 5. In histologic grading of SCC, grade I and II lesions occupied in 88% of total cases. The numbers of special variants of SCC were as follows aenoid(4), spindle cell(1), verrucous(2) CONCLUSIONS : 1. The average age of patient with primary epithelial skin cancers was older thar: in earlier reports in Korea, and was closer to the recent, reports in western count.ries. 2. The number of cases of BD was much higher than that reported previously. This emphasizeci the importance of tissue examination for the pigmented lesions of the skin, especially on the trunk and extremities.

4.Psoriasiform Sarcoidosis.

We repart herein a case of psoriasifomi sarcoidosis. A histopathological examination reveaied sarcoidal ulomas throughout the dermis. There wa no systemic nvolvement, The skin lesions resolved aftier one month following treatment with low dose oral prednisolone. This is the first case of psoriasiform sarcoidosis in Korea.

6.Foreign-body Granuloma After Honeybee Acupuncture.

Bee sting therapy is said to have been practised since the age of ancient Egypt. Some people believe in the benefit of apitherapy for articular inflammation, hematoma, sciatica, hay fever, etc. However, honeybee has several venoms that may induce various clinical and histopathological findings. We report a case of foreign-body granuloma after honeybee acupuncture for polyarthralgia. To our knowledge, this is the first report of foreign-body granuloma developing at an acupunctured site in English literature.

We report a case of histiocytoid hemangioma (angiolymphoid hyperplasia with eosinophilia) in a 60 year-old female patient who was effectively treated with dapsone. The therapeutic effect of dapsone may support the hypothesis that immunologically mediated reactive process plays a role in the pathogenesis of histiocytoid hemangioma.

8.Immunohistochemical study on granulomatous skin deseases.

BACKGROUND: A definition of granuloma is a focal chronic inflammatory response to tissue injury evolved by a poorly soluble substwice characterized by the accumulation and proliferation of the mono-nuclear histiocytic cells. The accuracy with which rnononuclear cells may be identified in skir. is much improved by the use of both heteroantisera and monoclonal antibodies directed against selected cellular antigens, OBJECTIVE: Our purpose was to examine the staining patterns of anti-lysozyme, anti-a-1-antitrypsin, anti-S-100 protein antibodies, and MAC-387 monoclonal anibody in granulomatous skin diseases. METHOD: We performed imminoperoxidase staining(the labelled str prvidin-biotin peroxidase complex method on the formalin-fixed, paraffin-embedded tissue specimens of granulomatous skin diseases. RESULTS: S-100 protein positive dendritic cells were demonstrated in the granulomatous infiltrates as scattered pattern and MAC-387 positive cells were predominantly found in the center of granulomas, The staining pattern and percentage of positively stained cells of a--antitrypsin were similar to those of lysozyme. A1Pha-1-antitrypsin and lysozyme positive cells w re present in the center as well as lymphohistiocytic infiltrates of granulomas. CONCLUSION: These data sugget that histiocytes are composed of heter igeneous groups of cells such as the mononuclear-phagocyte system and dendritic cell system.

9.Infection-Induced Panniculitis Associated with a Cardiac Abscess.

Infection-induced panniculitis develops either through direct inoculation or as a manifestation of sepsis. However, it has rarely been considered as a disease entity within broader context of panniculitis. Moreover, panniculitis associated with a cardiac abscess without the evidence of sepsis has not been reported. We describe a case of infection-induced panniculitis associated with a cardiac abscess. We suggest that infection should be considered as a potentially important etiology of panniculitis, especially in the case of immunosuppression, and in such a case, meticulous efforts should be done to find the focus of infection.

10.Clinical and Histopathologic Features of Actinic Granuloma.

BACKGROUND: Actinic granuloma is an annular inflammatory reaction that develops in skin that has had long-term sun-exposure. OBJECTIVES: The purposes of this study were aimed to characterize the clinical and histopathologic features of actinic granuloma. METHODS: Examinations were performed on eight patients with actinic granuloma regarding the age, sex, duration, clinical morphology, distribution, associated diseases, and treatment and course. Histo-pathologic studies of ten specimens obtained from eight patients were performed with special stains, such as Verhoeff-van Gieson, Gomori-Methenamin silver and alcian blue.
