BACKGROUND: Chronic cutaneous lupus erythematosus(CCLE) is a well-known disease entity. But there has been no data about its clinical behavior and histopathologic features in Korea. OBJECTIVES: This study was conducted to elucidate the clinical, laboratory, and histopathologic features of CCLE, and the relationship between CCLE and SLE. MATERIALS AND METHODS: We investigated 48 cases of CCLE that visited the department of dermatology at the Seoul National University Hospital from January 1990 to June 1997. Gout is a hereditary condition of disturbed uric acid metabolism in which urate salts are deposited in articular, periarticular and subcutaneous tissues. Clinically, it is characterized by recurring attacks of acute arthritis, intervals of freedom from pain, and in late stages by crippling deforming arthritis, nephritis, urinary calculi and cardiovascular lesions. Seventeen patients with gout managed at the department of orthopedic surgery, National Police Hospital, from February, 1972 to December, 1977, were reviewed and the following results were obtained. 1. Most of cases were between 30 and 49 years old and all the cases were male. 2. The most common season of occurrence of acute attack was winter and the least common season was summer. 3. The recurring attacks were more frequent than the first attacks. 4. The precipitating factors could be found usually including family history, dietary indiscretions, exposure to cold and stress conditions. 5. The average serum uric acid level during acute attacks showed 12. 9mg% and all the cases except 2 cases noted increased ESR with average 33.9mm/hr. The leucocytosis were noted in 6 cases. 6. The first involved joints were the M-P joint of big toe in 13 cases and the knee joint in 3 cases. 7. The radiological multiple punched-out bone lesions were found in 8 cases. 8. One tablet (0. 65mg) of colchicine was given every hour until the diarrhea appeared or the pain subsided, in 13 cases. Among them, when 10 tablets of colchicine were taken, the diarrhea appeared in 10 cases and the pain subsided in 8 cases. So we recommend 7~8 tablets for relief of G-I discomfort in management of gout. 9. The cases of gouty arthritis may be found more and more in our country because the stress condition and meat intake might be increased such as in Europe and America.

5.Adult-onset Kawsaki Disease Complicated by Splenic Infarction and Coronary Aneurysm.

Kawasaki disease or mucocutaneous lymph node syndrome is an acute inflammatory illness of childhood characterized by systemic panvasculitis. It presents with high fever, dramatic changes of the skin and mucous membranes, and lymphadenopathy. Adult-onset Kawasaki disease is rare and reports on coronary involvement in adult are even rarer. Herein, we report a case of adult-onset Kawasaki disease complicated by splenic infarction and development of coronary aneurysm even despite of treatment with intravenous gamma globulin. A 20-year-old man presented with fever, erytheatous rash, induration and desquamation of hands and feet, pulmonary edema and shock due to cardiomyopathy, splenic infarction, bilateral conjunctivitis, jaundice, and cervical lymphadenopathy. After Kawasaki disease was suspected, intravenous gamma globulin (2 g/kg once) and aspirin (6 g/day) were administered. On the 30th hospital day, transesophageal echocardiography showed one coronary aneurysm and coronary angiography showed three aneurysms. Eight months after the first admission, follow-up coronary angiography showed normalization of the previous coronary abnormalities.

7.The effects of various hormones and growth factors on the growth of human insulin-producing cell line in serum-free medium.

An insulin-producing cell line, HE, was derived from immortalized human pancreatic p-cells transformed by SV40 DNA, To make serum free medium specific for the cells, it was necessary to select basal medium, hormones, growth factors and coating materials. First of all, human insulin-producing cell line was adapted serially to the culture medium containing minimum required (1%) fetal calf serum (HB-1) from 10% fetal calf serum, The cells had a tendency to attach to the culture dishes coated with collagen or gelatin in the serum-free medium and showed a stronger attachment to collagen than to gelatin, For the basal medium selection, medium containing an equal mixture of RPMI 1640, Dulbecco's modified Eagle's medium, and Ham's F-12 (RDF) was shown to be the most suitable for the cells among 7 differently mixed basal media, The cells exhibited highly positive response to 7 factors including insulin-transferrin-selenium (ITS)complex in the basal medium. In addition, other 16 factors also influenced the cell growth positively, In contrast, 4 factors including aldosterone inhibited the cell growth, Cell growth was enhanced remarkably when the cells were cultured in the serum-free medium supplemented with all the seven positive factors, compared with in RDF serum-free medium. ITS and linoleic acid-BSA were found to be essential for the cell growth. The growth was much higher in the medium containing 6 strong positive factors without either dexamethasone, glucagon, or thyrocalcitonin than in the medium containing all the 7 strong positive factors, It is suggested that the interaction between the individual positive factors must be considered in choosing the factors with which the serum-free medium is supplemented.

8.A Case of Calyceal Diverticulum Treated by Partial Nephrectomy.

Calyceal diverticula are eventrations of the upper collecting system, lying within the renal parenchyma, communicating with the main collecting system via a narrow channel and lining by transitional tell epithelium. Most are asyptomatic and rarely require any form of treatment but complicated one needs surgical correction. We experienced one case of calyceal diverticulum associated with stone in 47 year old man. This calyceal diverticulum was removed by partial nephrectomy.

9.The change of rotational freedom following different insertion torques in three implant systems with implant driver.

STATEMENT OF PROBLEM: Implant drivers are getting popular in clinical dentistry. Unlike to implant systems with external hex connection, implant drivers directly engage the implant/abutment interface. The deformation of the implant/abutment interface can be introduced while placing an implant with its implant driver in clinical situations. PURPOSE: This study evaluated the change of rotational freedom between an implant and its abutment after application of different insertion torques. MATERIAL AND METHODS: Three kinds of internal connection implants were utilized for the current study (4.5 x 12 mm Xive, 4.3 x 11.5 mm Inplant Magicgrip, 4.3 x 12 mm Implantium MF). An EstheticBase, a 2-piece top, a Dual abutment was used for its corresponding implant system. The rotational freedom between an implant and its abutment were measured before and after applying 45, 100 Ncm insertion torque. Repeated measures ANOVA was used for statistical analysis. RESULTS: Under 45 Ncm insertion torque, the rotational freedom between an implant and its abutment was significantly increased in Xive (P = .003). However, no significant change was noted in Inplant Magicgrip and Implantium MF. Under 100 Ncm torque, both in Xive (P = .0005) and Implatium MF (P = .03) resulted in significantly increased rotational freedom between the implant and its abutment. DISCUSSION: The design of the implant/implant driver interface effectively prevented the deformation of implant/abutment interface. Little change was noted in the rotational freedom between an implant and its abutment, even though the insertion torque was far beyond clinical application. CONCLUSIONS: The implant/abutment joint of internally connecting implants were quite stable under insertion torque in clinical situation.

10.A Case of Beneficial Effect of Short-Term Growth Hormone Treatment for Intractable Heart Failure in Cardiomyopathy Combined with Neuromuscular Dystrophy.

We report a case of 15-year-old man with beneficial effects of short term growth hormone treatment presenting with cardiomyopathy combined with neuromuscular dystrophy. Transthoracic echocardiography revealed that LV chamber was markedly dilated and global LV systolic function was severely reduced. The findings of electromyography were compatible with neuromuscular dystrophy. Under the impression of cardiomyopathy combined with neuromuscular disease, maximal medical treatments such as inotropic agents, diuretics and ACE inhibitor were tried but we could not attain remarkable clinical improvement. Finally, we started growth hormone injection and after treatment for 3 months, we could attain remarkable clinical and hemodynamic improvement without any side effect.
