1.A Study of the Upper Gastrointestinal Polyp.

The authers had diagnosed and treated 560 upper gastrointestinal polys by endoscopy on 479 patients who had visited to the gastraeaterologic department of Hae Wha hoapital, Medical College of Korea University from January, l980 to June, l991. The results were as follows; 1) The incidence of upper gastrointestinal polyps among 52,031 endoscopy cases was 0.92% with no sexual difference and increased after 5th. decade. 2) The moet prevalent location of upper gastrointestinal polypes was stomach(90.0%), among which antrum occupied 52.9%. 3) The main symptoms of patients with upper gastrointestinal polyp were epigastric pain(77.7%), indigestion(57.0%) and acid belching(26.5%). 4) Histopathologic examination of 504 polyps in stomach revealed that 411(81.5%) were hyperplastic polyp and 80(15.9%) were adenomatous polyp. In duodenum, 35(68.6%) polyps were hyperplastic and 7(l3.7%) were adenomatous. All of 5 esophageal polyps were hyperplastic.

5.Five Cases of Salivary Gland Swelling Developed after Upper Gastrointestiinal Endoscopy.

As upper-gastrointestinal endoscopy has been more widely used for the diagnosis and treatment of upper gastrointestinal diseases, some complications such as bleeding, perforation, arrhythmia, aspiration pneumonia, infection, and drug reaction of premedication were reported. Also a few case of salivary gland swelling complicated by endoscopy was described, but the precise mechanism of this benign complication is not well-known. The hypothesized mechanisms for salivary gland swelling are 1) air distension of blind branchial cleft remmants, 2) compression of its duct between the mylohyoid and hyoglossus muscles from pressure above, during a time of increased salivary flow from oral stimulation, 3) forward and downward dislocation of gland by the force that occurs when endoscope pushes the tongue, and 4) reaction to topical anesthesia less likely. Here we report 5 cases of salivary galnd swelling with a review of the literature, explaining possible mechanism of each case.

7.Immunohistochemical study on sweat gland tumors with monoclonal antibodies against S-100 subunits.

S-100 protein is a mixture of three proteins, that is, S-100 ao(aa), S-100 a(ab) and, S- 100 b(bb). Twenty-two case, of sweat gland tumors were stained with immunoperoxidase technique (ABC method) for the presence of S-100a and b-subunit. Four syringomas, four eccrine poromas, two eccrine porocarcinomas, two ecerine spirdeiomas, one papillary eccrine adenoma, three clear cell hidradenomas, three mixed tumr rs of the skin, two papillary syringocystadenomas, and one cylindroma were included. All specimens were formalin-fixed and paraffin-embedded. The results were as follows : 1) The staining patterns of anti-S-100a and b-protein antibodies we e simillar to those of anti-S-100 protein antibody except in eccrine poroma and porocare nomal. 2) In eccrine poroma and porocarcinoma, scattered S-100-positive dendritic cells within tumor cell nests were stained by S-100-protein antibody (3/6), but not by anti-S-100a protein antibody. S-100p is present in normal Langerhans cells. Therefore this finding suggests that these cells niay be Langerhans cells

8.An Evaluation of Sunscreen Efficacy Using Mouse Ear Swelling Reaction.

This study was undertaken to investigate the efficacy of 5% PABA cream using mouse ear swelling reaction(ESR). Mice were exposed to 100mJ/cm of UVB, five times a week for four weeks, on the both ventral aspect of the ear, with application of 5% PABA cream on the right ear. The results were as follows : 1. The intensity of ear swelling reaction of 5% PABA protected group was reduced greater than unproteeted group after the first 3 days of UUR. 2. The intensity of ear swelling reached at peak after 1 week of the ultraviolet radiation. Thereafter it has decreased gradually the following 4 weeks. The difference of ear swelling between the two groups was the greatest after 1 week, and the sunscreening efficacy of 5% PABA cream has remained persisted for 4 weeks. 3. The number of mice which have shown severe inflarnmatory response after ultravioiet radiation was more in unprotected group than that in 5% PABA protected group. 4. Determination of mouse ESR is considered a good method for the evaluation of longterm efficacy of sunscreen preparation.

9.Immunohistochemical study on granulomatous skin deseases.

BACKGROUND: A definition of granuloma is a focal chronic inflammatory response to tissue injury evolved by a poorly soluble substwice characterized by the accumulation and proliferation of the mono-nuclear histiocytic cells. The accuracy with which rnononuclear cells may be identified in skir. is much improved by the use of both heteroantisera and monoclonal antibodies directed against selected cellular antigens, OBJECTIVE: Our purpose was to examine the staining patterns of anti-lysozyme, anti-a-1-antitrypsin, anti-S-100 protein antibodies, and MAC-387 monoclonal anibody in granulomatous skin diseases. METHOD: We performed imminoperoxidase staining(the labelled str prvidin-biotin peroxidase complex method on the formalin-fixed, paraffin-embedded tissue specimens of granulomatous skin diseases. RESULTS: S-100 protein positive dendritic cells were demonstrated in the granulomatous infiltrates as scattered pattern and MAC-387 positive cells were predominantly found in the center of granulomas, The staining pattern and percentage of positively stained cells of a--antitrypsin were similar to those of lysozyme. A1Pha-1-antitrypsin and lysozyme positive cells w re present in the center as well as lymphohistiocytic infiltrates of granulomas. CONCLUSION: These data sugget that histiocytes are composed of heter igeneous groups of cells such as the mononuclear-phagocyte system and dendritic cell system.

10.Effects of Oral Clonidine on Cardiovascular Response to Tracheal Intubation.

Induction of general anesthesia with tracheal intubation is routine proeedure but causes varying degrees of sympathetic stimulation such as increases in blood pressure and heart rate. Many approaches have been tried to attenuate these hemodynamic responses To evaluate the effects of clonidine, a-2 adrenergic receptor agonist, on the hemodynamic changes induced by intubation, we administered clonidine(5 ug/kg, P.O.) 90 minutes before induction of anesthesia. We measured systolic pressure, diastolic pressure and heart rate before premedication, before induction, after intubation and 5, 10 minutes after intubation and compared with control group. The results were as follows: 1) After intubation systolic and diastolic blood pressures increased significantly in both group(P<0.05) but the increasing rate was significantly less than in clonidine group. (P<0.05) 2) After intubation the increaae in heart rate in clonidine group was significantly less than in control group (p<0.05). 3) The trend of increase in heart rate in clonidine group was more stabilized than control group. 4) Systolic blood pressure and heart rate in clonidine group were significantly decreased in preinduction period after premedication.
