We present four cases of herpes zoster treated successfully with vidarabine-5-monophosphate, two patients receiving chernotherapy and radiation therapy due to Hodgkins lymphoma or colon cancer, a patient combined with viral meningitis, and a patient with herpes zoster ophthalmicus. Viclarabine can be used to treat herpes zoster in immunosuppressed patients or patients with malignancy or patients with severe complications.

A 56-year-old man received subtotal gastrectomy for an early gastric carcinoma type IIa+IIc with submucosal invasion. The tumor was made up of mixed papillo-tubular adenocarcinoma and solid carcinomatous portion, the latter comprising approximately four-fifths of the total tumor mass. The solid portion was confined within the submucosa and revealed a mixture of trabecular, compact and pelioid patterns of large polyhedra cells, resembling hepatocellular carcinoma of the liver(Edmondson-Steiner grade 2). Sinusoid-like vascular stroma of classical trabecular hepatocellular carcinoma intervened the tumor cell nests but was not associated with endothelial-cell lining. Immunohistochemical stainings with alpha-fetoprotein and alpha1-antitrypsin gave a strong reactivity in those areas of hepatoid differentiation and in the adjacent minute portion of adenocarcinoma. The findings suggest that a portion of gastric carcinoma may transdifferentiate into cells with hepatoid features along the line of endodermal lineage.

Ulcerative colitis is a chronic, inflammatory disease of colon which is rare in Korea. In the course of ulcerative colitis numerous extraintestinal complication may develop on the joints, skin, liver, kidney, vessel, oral cavity and eyes. The most common ocular lesions complicated in patients with ulcerative colitis are episcleritis and iritis. And the cases of retinal vascular disease in associated with ulcerative colitis are extremely rare. We experienced nonischemic central retinal vein occlusion complicated in a patient with ulcerative colitis, who is 32-year-old male and complained decreased visual acuity and metamorphosis.

Immediate postoperative changes in intraocular pressure (IOP) were prospectively studied in 60 patients after extracapsular cataract extraction and posterior chamber lens implantations, according to three different preoperative managements for cecreasing IOPs. In group 1 (24 patients), no ocular pressure reducing agents were given but only ocular massage was performed preoperatively. In group 2 (17 patients), intravenous mannitol (1.52g/kg of body weight) and/or 60ml of 50% oral glycerine was administered 1 hour before surgery. In group 3 (19 patients), 500mg of acetazolamide was administered orally 90 minutes before operation in addition to the regimen of group 2. A significant lllcrease in mean IOP was found 6 hours after the operation in group 1 by 5.6mmHg and in group 2 by 8.8mmHg when compared witt baseline IOP (p<0.05). However, there was no significant IOP rise in group 3. Postoperative IOPs in excess of 21mmHg occurred in 10 patients (42% )of group 1, seven patients (41 %) of group 2 and six patients(30%) of group 3. An IOP greater than 30mmHg was found in two patients (8%) in group 1, two patients (12%) in group 2 and one patient (5%) in group 3. From this study, it was concluded that acetazolamide 500mg given 90 minutes before operation resulted in a reduction of immediate postoperative IOP rise.

One patient with bilateral acute retinal necrosis underwent encircling scleral buckle, vitrectomy, and intravitreal acyclovir on both eyes. This procedure was performed on the right eye while the retina was attached. The retina of the right eye was reattached by performing fluid-gas exchange and modified panretinal photocoagulation when the retina subsequently detached. Soon after the development of retinal detachment in the left eye, the above surgical procedures were performed on the left eye, and the retina was successfully reattached.Bilateral acute retinal necrosis with significant vitreous opacification, which is a devastating ocular disease causing possible blindness in both eyes, requires more aggressive, early surgical management.

Formaldehyde in adhesives is used as preseratives as well as adhesives themeseles such as phenol-formaldehyde, carbamide-formaldehyde and melarnine-forma 1dehyde. Formaldehyde content in 43 kinds of adhesives used at present in Korea was determined by lutidine method. The study result was as follows. 1. The The distribution of formaldehyde concentrations of 43 different kinds of adhesives was in the 0 to 333. 9 ppm ranges. 2. High concentration of formaldehyde was observed in 3 kinds of wall paper adhesives and 1 kind of stationary adhesive, which has an important clinical significance because of their frequent contact sources of domestic uses.

Lymphangiomas are made up of multiple lymphatic channels lined by benign-appearing endothelial cells. The lesions can occur anywhere in the body but most commonly affect head, neck and axilla. It rarely involves the gastrointestinal tract and particularly lymphangioma of the colon is very rare. Histologically, it is classified three types as simple or capillary, cavernous, and cystic. We experienced three cases of cavernous lymphangioma of the ascending colon. Case 1, 56-year-old male, had lymphangioma accompanied by ulcerating cancer of the cecum. Case 2, 51-year-old female, had lymphangioma of cecum. Case 3, 41-year-old male had lymphangioma at ileocecal valve. All cases could be diagnosed by colonoscopic snare polypectomy.

Three cases of successful pregnancy outcome after intensive care in the patients with rare mullerian anomaly of complete uterine septum, hypoplastic cervical duplication and longitudinal vaginal septum in infertility clinic are reported. These presented three cases are the reports of supporting the embryologic hypothesis of bidirectional mullerian development which fusion and resorption begins at the isthmus and proceeds simultaneously in both the cranial and caudal directions rather than the classical views of unidirectional(caudal to cranial) mullerian development. Thus, we present here three cases of rare mullerian anomaly and their pregnancy outcomes in an attempt to alert gynecologist to the possible occurrence of such a malformation. In 2 cases, successful pregnancy outcomes were achieved after abdominal or hysteroscopic uterine septotomy before pregnancy and in the last case, successful pregnancy outcome intensive obstetric care was achieved even under uncorrected uterine anomaly and complete removal of uterine septum resulted in uneventful postoperative courses at the same time of cesarean section.

The diagnosis of acute inflammation of the placenta, represented as acute chorioamnionitis, is important in that it is associated with a poor clinical outcome for both the mother and the fetus, including major perinatal morbidities such as sepsis, respiratory distress syndrome, and CNS damage. However, current medical trends in Korea seem to overlook the significance of a histopathological diagnosis of acute placental inflammation, mainly due to the indifferences of clinicians and pathologists. Since late 1993, histopathological examinations have been performed on preterm placentas at Seoul National University. These examinations have demonstrated acute placental inflammation in a significant number of cases. In the present study the incidence of acute placental inflammation was analyzed in 521 placentas which were submitted for pathological examinations in 1995. Examinations were performed to provide basic information on the incidence and profile of acute placental inflammation in this hospital and, thereby, to emphasize the significance of histopathological examinations of the placenta in the routine surgical pathology service. Among the 521 placentas, acute inflammation was found in 194 cases (37.2%). In preterm placentas acute inflammation was found in 39.6% of the cases (67/169), while 36.1% (127/352) of term placentas showed acute inflammation. Taking the delivery mode into account, 26.3% (49/186) of the placentas delivered by cesarean section showed acute inflammation, while 43.3% (145/335) of the transvaginally delivered placentas showed inflammation. The present analysis demonstrates the existence of acute inflammation in a significant proportion of placentas with different clinical settings. The importance of a histopathological examination in routine hospital practice should be emphasized.
