1.Dislocation of Peroneal Tendons Two Cases Report

Dislocation of peroneal tendons are caused by forceful domifiexion of the foot accompanied by a powerful contraction of the peroneal muscles. This mechanism of injury tears the superior pemneal retinaculum and allows the tendons to snap anteriorly. It is an important, although infrequent, cause of disability of the ankle. The acute injury can be treated by immobilization in a plaster cast, but when there is chronic recurring dislocation, surgical reconstruction is necessary. Authors experienced two cases of dislocation of pemneal tendons which were treated by use of Sarmiento's and Platzgummer's(modified) operative procedures with good results.

5.Comparison of clinical efficacy between acitretin and etretinate in psoriasis.

Etretinate, an effective retinoid in the treatment of pustular, erythrodermic and chronic plaque type psoriasis, has the disadvantage of a long terminal elimination half-life. On the other hand, acitretin, the active metabolite of etretinate, has much shorter terminal elimination half-life and is being reported as an agent with good antipsoriatic activity by several authors. To evaluate the clinical efficacy of acitret.in in comparison with etretinate, we treated 10 patients wit,h acitretin at a dose of 30mg per day and 11 patients with etretinate at a same dose for 12 weeks. The PASI score at 12 week was significantly reduced in each group as compared with baseline PASI score. In the acitretin treated group the initial PASI score of 14.5 reduced to 3.9, while the etretinate group PASI score reduced from 12.0 to 3.1. The PASI score differ ences between the acitretin and etretinate groups at each time during therapy and the end of therapy were not statistically significant. The severity of adverse reactions with acitretin was similar to those with etretinate but their incidence was higher. The change in laboratory parameters in the acitretin group was simlar to that of the etretinate group. In view of these results and the known pharmacokinetic advantage of acitretin, that is the short terminal elimination half-life, it is conceivable that acitretin may be a useful alternative to etretinate in the treatment of psoriasis.

6.A study of garlic sensitivity in patients with hand eczema.

The importance of garlic as a cause of hand eczema has been a matter of debate. Diallyldisulfide is considered as the main allergen in garlic. Twentv two antigens of the modified European standard series and diallyldisulfide of 5%, 2%, 1%, 0.5%, 0.1% in petrolatum were patch tested in 44 patients with hand eczema. Diallylclisi.!lfide was also tested in 13 women as a control. The results were summerized as follows. l. In patch testing with the European standard series, nickel sulfate (22.7%), cobalt chloride (18.2%), fragrance mix (13.6%), balsam of Peru (9.1%) and termerosal (6.8%) were the cornmon allergens which showed positive reactions. The overal positive rate was 59.1 % (26/44). 2.5% diallyldisulfide showed an irritant reaction in 6 (46.2%) out of 13 controls and in 16 (36.4% ) out of 44 patients. 2% diallyldisulfide showed an irritant reaction in 2/13 (15.4 %) in the control group and 6/44 (13.6%) in the patient group. All 13 controls and 44 patients were negative iin tests with diallyldisulfide of 1%, 0.5%, 0.1%. Our results indicate that patch testing with diallyldisulfide of 1% petrolatum is reommended in order to avoid an irritant reaction when contact allergy to garlic is suspetel. 3. Garlic is suspected as an irritant in provoking hand eczema. True contact allergy to garlic in hand eczema seems to be rare.

11,609 of childhood new outpatients seen in Department of Dermatology of Seoul National University Hospital from 1975 to 1984 were analyzed statistica,lly. The study results were summarized as follows: l. 11, 609 out of 48, 188 new outpatients were under 15 years old age, amounting to 24. l %. 2. The 10 most common dermatoses were atopic dermatitis, vitiligo, urticari, hemangiorna, contact dermatitis, fungal disease, molluscum contagiosum, scabies, impetigo and nevocellular neuvs. The most freguent age groups were 2 to 6 years amounting to 47 2p, af total outpatients. 4. The peak incidence occurred in August, while the lowest incidence occurred in November and October. 5. Dermatoses which showed increasing tendency were nevocellular nevus and vitiligo, those of decreasing tendency were contact dermatitis and impetigo,

8.Renal Replacement Therapy in Acute Kidney Injury: Indication, Proper Initiation, and Prescription.

Despite substantial advances in dialytic techniques and machines, acute kidney injury (AKI) requiring renal replacement therapy (RRT) is associated with up to 60% in-hospital mortality. But, there is no full detail of definite RRT to overcome the significant morbidity and mortality of AKI. What is most important in the treatment for AKI is that RRT is not a cause-specific therapy but a life-supportive management. This review discusses the indications of, proper initiation of, and optimal prescription for RRT to improve the survival of the patients with AKI.

9.Esthetic restorations of maxillary anterior teeth with orthodontic treatment and porcelain laminate veneers: a case report.

If orthodontists and restorative dentists establish the interdisciplinary approach to esthetic dentistry, the esthetic and functional outcome of their combined efforts will be greatly enhanced. This article describes satisfying esthetic results obtained by the distribution of space for restoration by orthodontic treatment and porcelain laminate veneers in uneven space between maxillary anterior teeth. It is proposed that the use of orthodontic treatment for re-distribution of the space and the use of porcelain laminate veneers to alter crown anatomy provide maximum esthetic and functional correction for patients with irregular interdental spacing.
