1.A Normal Pregnancy and Delivery by Assisted Activation Following Roundheaded Spermatozoa (Globozoospermia) Injected into Oocytes.

Globozoospermia is a rare type of teratozoospermia. It occurs in 0.1% of all andrological patient's and used to be considered sterile. Globozoospermic patient has 100% round headed spermatozoa, but the spermiogram is normal. The spermatozoa show oval-shape head, the lack of a nuclear envelope, acrosome, and post acrosomal sheath. OBJECTIVE: To report that a couple with infertility secondary to globozoospermia received ICSI treatment. MATERIAL AND METHOD: Case report. RESULTS: In the first trial, fertilization was failed. In the second trial, 40% of oocytes were fertilized and all of these embryos were cleaved, but pregnancy did not achieved. In the third trial, sperm injected oocytes were exposed to 10 micrometer calcium ionophore for 15 min. All of the injected oocytes were fertilized and proceeded to develop. Triplet pregnancy was achieved after the transfer of six embryos in their third cycle. One embryo vanished and the remaining twins (female) were delivered at 33 weeks of gestation by Caesarean section. CONCLUSION: This result shows that assisted activation following ICSI may overcome infertility associated with globozoospermia.

3.Sleep and Nocturia in Under Age 50 Male Patients with Obstructive Sleep Apnea Syndrome

Objectives:
To investigate the prevalence of nocturia in young patients with obstructive sleep apnea syndrome (OSAS) and to find out the risk factors of nocturia in the patients.
Methods:
We enrolled 415 untreated male OSAS patients (apnea-hypopnea index, AHI ≥5/h on polysomnography) who were under 50 years old (mean age 37.5±8.0 years). Participants completed Korean version of Beck depression inventory II (K-BDI-II), Pittsburgh sleep quality index-Korean (PSQI-K), insomnia severity index (ISI), and Epworth sleepiness scale (ESS). Participants were classified into nocturia and non-nocturia group according to the following question “Do you find yourself waking up to urinate more than twice each night?” All information was compared between two groups.
Results:
22.7% (94/415) of patients reported to have nocturia. Patients with nocturia had higher score of K-BDI-II, PSQI-K, ISI, and ESS although their age and the proportion of consumption of alcohol or caffeine and metabolic diseases were not different from patients without nocturia. AHI was not significantly higher in nocturia group (38.0±29.2/h) than non-nocturia group (32.8±25.7/h, p=0.118), however, parameters indicating sleep quality were worse in nocturia group, i.e. lesser non-rapid eye movement sleep stage 3 (N3) sleep % and higher arousal index, total apnea index, and 90% oxygen desaturation index (ODI). 90% ODI and N3 sleep % were revealed to be independent factors associated with nocturia.
Conclusions
Considerable numbers of male OSAS patients who are under 50 years suffer from nocturia and they have worse sleep quality compared to non-nocturia patients. Higher oxygen desaturation and lesser N3 sleep % are highly predictive factors for nocturia rather than AHI in these patients.

The endoplasmic reticulum (ER) stress results from disrupted protein folding triggered by protein mutation or oxidation, reduced proteasome activity, and altered Ca2+ homeostasis. ER stress is accompanied by activation of the unfolded protein response (UPR) and cell death pathway. We examined if the UPR and cell death pathway would be activated in Alzheimer's disease (AD). RT-PCR experiments revealed increased splicing of X-box binding protein-1 (XBP-1), an UPR transcription factor, in AD compared with age-matched control. Among target genes of XBP-1, expression of protein disulfide isomerase (PDI), but not glucose-regulated protein 78 (GRP78), was increased in AD, suggesting disturbed activation of the UPR in AD. C/EBP homologous protein (CHOP), caspase-3, caspase-4, and caspase-12, downstream mediators of cell death pathway, were activated in AD. Neither the UPR nor cell death pathway was induced in aged Tg2576 mice, a transgenic mouse model of Alzheimer's disease that reveals both plaque pathology and some cognitive deficits. The present study suggests that disturbed induction of the UPR and activation of the pro-apoptotic proteins contribute to neuropathological process in AD irrespective of amyloid beta and senile plaque.
5.The Incidence Rate of Lymphadenitis after Bacille Calmette-Guérin (BCG) Vaccination.

PURPOSE: Bacille Calmette-Guérin (BCG) lymphadenitis is a relatively frequent local adverse reactions after BCG vaccination. Its incidence rate is usually <1%. However, this rate may be different according to BCG strain, vaccination method or skill, etc. In the Republic of Korea, two BCG strains are used: intradermal Danish-1331 or percutaneous Tokyo-172. We surveyed the incidence rates of BCG lymphadenitis. METHODS: This survey was performed in total 25 centers (5 general hospitals, 20 private pediatric clinics). Immunized type of BCG strain in study subjects was verified by directly observing the scar. The occurrence of BCG lymphadenitis was asked to their parent. In cases of BCG lymphadenitis, location, diameter size, progression of suppuration, and treatment method were investigated, as well. RESULTS: The total number of study subjects was 3,342. Among these, the subjects suitable for enrollment criteria (total 3,222; Tokyo strain 2,501, Danish strain 721) were analyzed. BCG lymphadenitis regardless of its size developed in each five of subjects per strains, therefore, its incidence rate was 0.20% in Tokyo and 0.69% in Danish strain, respectively (P=0.086). However, when applying the WHO criteria - the development of lymph node swelling with diameter 1.5 cm or more, the incidence rate of BCG lymphadenitis was 0.16% (4 cases) in Tokyo and 0.42% (3 cases) in Danish strain, respectively. CONCLUSIONS: The incidence rate of lymphadenitis in two BCG types, percutaneous Tokyo and intradermal Danish strain BCG, is 0.20% and 0.69%, respectively. Both rates are acceptable.
