5.Malignant Acanthosis Nigricans: Report of a Case.

One case of malignant acanthosis nigicans associated with gastric adenocarcinoma (scirrhous type) and papilloma on the oral mucosa is presented. The patient, 36 year old Korean male, visited S.N.U.H, complaining of sore-mouth and sore-tongue of 20 days duration. He had been affected with severe papillomatosis on the oral mucosa and with hyperkeratosis on the periorbital area, the perioral area, the neck, the axillae, the palms, the umbilical area, the inguinal area, the anogenital area and the soles which were moderately hyperpigmented with grayish to brown color. He had not been complaining of G-I troubles except sore-mouth and sore-tongue, but he was diagnosed as stomach carcinoma by gastroendoscopy, He got an operation of extended total gastrectomy with isoperistaltic jejunal interposition. Post-operative histopathologic examination rvealed well differentiated adenocarcinoma of the stomach which had already metastasized to the neighboring lymph nodes. After operation the papillomatosis on the oral mucosa and skin eruptions had been improved but becarne aggravated again one and half month after discharge.

7.Pulmonary coccidioidomycosis Diagnosed in an Immigrant.

Coccidioidomycosis is a common infectious disease in southwestern North America, which is caused by the soil fungus, Coccidioides immitis. Due to the mobility of the modern population, increasing numbers of cases are being diagnosed outside the endemic areas. Moreover, diagnosis in non-endenmic areas may be delayed or confused due to its rarity. Here we report a case of pulmonary coccidioidomycisis in an immigrant. The patient presented with unexplained pulmonary symptoms. A history of recent travel to or immigration from an endemic area may suggest coccidioidomycosis. In addition an early tissue biopsy is helpful in establishing this diagnosis.

8.Evaluation of Nutritional Status of Inpatients with Medical Health Problems.

PURPOSE: To investigate nutritional status of inpatients by using subjective. and objective evaluation methods and to find the relationship between them. METHOD: The subjects were 101 inpatients with medical health problems at a university hospital. Nutritional status was evaluated by the Subjective Global Assessment (SGA) and physical assessment including percentage of weight loss, serum albumin, hemogloin, and hematocrit. Data were analyzed using frequency, percentage, mean, Kendall's tau. RESULTS: Grouping by the SGA, 61.4% were classified as severe malnutrition group. When applying the objective methods{physical assessment). 1.9~42.6% were diagnosed as malnutrition each item. The percentage of weight loss during previous 1-6month(r=.43~.54. P=.0001), serum albumin(r=-.26, P=.0003), hemoglobin of male(r=-.38, P=.0001), and hematocrit of male(r=-.34, P=.0001) were significantly correlated with SGA score. The coincidence rate of nutrition evaluation between the objective methods and SGA were 27.7 35.6%, 20.8%, 47.5%, 58.4% in percentage of weight loss, albumin, hemoglobin, and hematocrit. CONCLUSION: These findings showed a majority of inpatients were exposed to the risk of malnutrition. We recommend to evaluate inpatients' nutritional status periodically and to develop nursing intervention to solve their nutritional problems.

9.Relationship between the Discomfort of Tinnitus and the Quality of Life in Tinnitus Patients.

PURPOSE: To investigate audiometery and characteristics related to tinnitus in tinnitus patients. and to understand the relationship between the discomfort of tinnitus and the quality of life. Methods: The subjects were 110 patients with tinnitus. The data were collected using tinnitus handicap questionnaire. quality of life, and pure tone audiometry, which measured hearing level in dB and frequency in Hz matching of tinnitus. RESULTS: Majority of tinnitus patients (75.5%~81.8%) had normal auditory sensation. but 82.7% of patients indicated that they have heard tinnitus frequently. or always, and the major sounds of tinnitus were wing, wung, wang, The matching sounds of tinnitus had various range in hearing level(dB) and frequency(Hz). Tinnitus patients reported moderate levels of the discomfort of tinnitus and the quality of life, and there were negative correlation between the discomfort of tinnitus and the quality of life(r=-.314, p .00l). CONCLUSION: These findings suggest that special attention is needed for tinnitus patients because the physical. psychological. and social impacts of tinnitus could be decrease their quality of life, Specialized nursing intervention considering specific conditions related to tinnitus is required to solve various health problems of tinnitus patients.

10.Results of Decompression and Internal Fixation for Spinal Stenosis more than 5 year Follow up.

STUDY DESIGN: Thirty-four cases of surgically treated spinal stenosis which were followed up for more than 5 yeras were analysed. OBJECTIVES: To know the relationship between long-term clinical results and various clinical factors. SUMMARY OF LITERATURE REVIEW: Pedicle screw fixation is a common procedure after wide decompression for spinal stenosis. But, there is few report showing its long-term result. MATERIALS AND METHODS: We evaluated the long term results of surgically treated spinal stenosis patients according to the sex, method of fusion, level of fusion, radiologic change of adjacent level, hardware problem and complication. There were 11 males and 23 females. Average age was 53.2 years(range 34-68 years). Radiographically follow up was averaged 72 months(range 60-104 months). Clinically follow up period was averaged 76 months(range 60-113 months). Eight patients were operated with PLIF and 26 patients were with posterolateral fusion. One level surgery was done in 18 patient and more than two level in 16 patients. RESULTS: Twenty-seven patients(79%) showed satisfactory result at 2year and 5 year follow-up. Two patients showed change of clinical result during follow-up. Clinical result was much better in monosegmental surgery and in male sex. Six patients showed narrowing of disc space at non-surgery level and 2 patients had compression fracture. Complications were 1 deep infection, 2 dural tears, 9 donor site pain, 2 metal failures and 1 metal pull-out. CONCLUSIONS: We concluded that there is no significant change of clinical results between two year follow-up and five year follow-up. Clinical complications and metal problems are not related with the unsatisfactory results. Multiple level surgery and female sex are two variables related with the unsatisfactory clinical outcome.
