5.Nursing Students' Knowledge, Attitudes to Advance Medical Directives and Ethics Values

PURPOSE: The purpose of this study was to identify the relationships among nursing students' knowledge, attitudes to advance medical directives and ethics values. METHODS: The study was a cross-sectional and descriptive survey, carried out with 232 students from two schools of nursing, one located in B and C city. Student nurses' knowledge, attitudes for advance medical directives and ethics values were measured using structured self-report questionnaires. Data were analyzed using t-test or one-way ANOVA, and Pearson correlation coefficients. RESULTS: The scores for knowledge, attitudes for medical advance directives were 7.18 and 3.07, and for ethics values, 3.47. Nursing students' knowledge and attitude showed a significant difference depending on grade. Nursing students' ethics values showed significant differences depending on whether a study on ethics had been completed. A positive correlation was observed between nursing students' knowledge, attitudes for advance medical directives and ethics values. CONCLUSION: The results of this study indicate that basic data for the development of an effective education program on ethics is important in establishing a positive attitude toward advance medical directives and the appropriate ethical values in student nurses.

6.Experience of Hospital Administrative Nurses' Work Adjustment

PURPOSE: This study was aimed to explore the work experience of nurses who transitioned from the patient nursing department to administrative departments.METHODS: The participants were 11 nurses who had worked in departments of human resources, education and training, medical insurance review, information technology, and quality improvement. Data were collected through in-depth individual interviews done between December 2017 and July 2018 and were analyzed in terms of Giorgi's phenomenological methodology. The research question was ‘What kind of experience did you have as you transition to a new role from a nurse to a hospital administrative nurse? What are the challenges in carrying out a new role? How do you overcome obstacles and adjust to being a hospital administrative nurse?’RESULTS: The main essence of the administrative nurses' work adjustment experience is represented by six components; 1) Beginning of a new challenge in conflict, 2) Running into barriers, 3) Struggling to take root, 4) Recognizing changes in the hospital, 5) Gradually adjusting to change, 6) Becoming a proficient hospital administrative nurse.CONCLUSION: Based on these findings, it is essential to create systems that enable administrative nurses to adjust to their work without confusion, and continuously develop programs to enhance their competence.

8.Quantitative Histological Analysis of the Ureteropelvic Junction Obstruction.

The ureteropelvic junction is one of the most common site of obstruction in the upper urinary tract, though the etiology is still unclear. In the present study, a quantitative histological analysis of the ureteropelvic junction(UPJ) obstruction was performed on 23 patient specimens(10 children, 13 adults) and 14 normal control specimens(6 children, 8 adults). Color image analysis system was employed to examine and compare collagen and smooth muscle components of the lamina muscularis layer. Tissue matrix ratio, that is collagen-to-smooth muscle ratio, was calculated using Bio Scan OPTIMAS Ver 4.02. Relative ratio of lamina propria and lamina muscularis to UPJ was calculated as percentage provided by "Img Pro" computer program. The results showed an increased tissue matrix ratio of 0.68+/-0.42 in the child patient group compared with the value of 0.30+/-0.10 in the child control group(p<0.05), also the increased tissue matrix ratio of 1.45+/-1.67 in the adult patient group compared to 0.48+/-0.25 in the adult control group(p<0.05). But relative ratio of the lamina propria and lamina muscularis to UPJ was not statistically significant for each group(p>0.05). Our findings emphasize a significant increase in the tissue matrix ration(collagen-to-smooth muscle ratio) of the UPJ obstruction.

9.The Effect of Lidocaine HCI Administered into Epidural Space on the Neuromuscular Blocking Action of Vecuronium.

Most local anesthetics intravenously administered inhibit neuromuscular transmission or enhance the neuromuscular block of both nondepolarizing and depolarizing musde relaxants. Local anesthetics used to treat cardiac arrhythmias intraoperatively or postoperatively may, therefore, greatly augment a residual neuromuscular block. To obtain adequate balanced anesthesia and postoperative analgesia, general anesthesia with epidural anesthesia is frequently selected. In the present study we investigated the effects of lidocaine HC1 administered into epidural space on the neuromuscular blocking action of vecuronium. Eighty adult gynecological patients of ASA class I or II were studied. Patients were divided into four groups as follows ; Group I (n=20); vecuronium 0.1 mg/kg iv administered only Group II(n=20); vecuronium 0.1 mg/kg iv and lidocaine 6 mg/kg administered into epidural space Group III(n=20); vecuronium 0.1 mg/kg iv and lidocaine 8 mg/kg administered into epidural space Group IV(n=20); vecuronium 0.1 mg/kg iv and lidocaine 10 mg/kg administered into epidural space Patients of experimental groups II, III and IV were anesthetized by epidural block with 2 % lidocaine followed by general anesthesia 15 minutes later. Before induction with fentanyl(3 ug/kg), droperidol(0.04 mg/kg), thiopental sodium (5 mg/kg) and vecuronium(0.1 mg/kg) iv, neuromuscular monitoring was set by using ABM(Anesthesia and Brain activity Monitor, Datex Co, Finland) which stimulating ulnar nerve at the wrist with supramaximal stimuli at a frequency of 2 Hz and 20 seconds interval. The electromyographic response of hypothenar muscles was recorded. Following stabilization of the control twitch height, vecuronium(0.1 mg/kg) was intravenously injected, and tracheal intubation was performed when the first twitch height of the train of four twitch response(T1) was 5 % of the controL Anesthesia was maintained with O2 (2 L/min) and N2O (4 L/min). The time of onset of action (time from vecuronium iv to 0 % of T1), time of maximal twitch depression (time from loss of T1 to reappearance of T1), Recovery index (time from recovery of 25 % of T1 to 75 % of T1) and T4 ratios (ratio of the height of the fourth twitch to T1) at 25 %, 50 % and 75% of T1 were measured and compared among the groups. The results were as follows ; 1. The time of onset of action was 3.99+/-0.92 minutes in the control group and 2.94+/-0.89 minutes in the lidocaine 10 mg/kg administered group(P<0.05). 2. The time of maximal twitch depression was 19.055.46 minutes in the control group and 24.02+/-6.94, 25.33+/-5.84, 27.07+/-5.26 minutes in the lidocaine 6 mg/kg, 8 mg/kg, 10 mg/kg administered groups(P<0.05). 3. Recovery index was 14.76+/-5.40 minutes in the control group and 20.39+/-4.56, 22.36+/-7.98 minutes in the lidocaine 8 mg/kg, 10 mg/kg administered groups(P<0.05). 4. The lidocaine administered into epidural space has little effect on the recovery of T4 compared with that of T1(P>0.05). These results suggest that the effect of lidocaine HC1 administered into epidural space on the neuromuscular blocking action of vecuronium showed prolonged depression and recovery time, and reduced time of the onset of action with dose larger than lidocaine 8 mg/kg.
