1.A Study on the Vertebral Body and Intervertebral Disk Indices of the Normal Korean People (A Preliminary Report)

The vertical and sagittal diameter of the vertebral body from the 12th thoracic to the 4th lumbar spine was measured by the true lateral roentgenogram. The material consisted of 110 normal Koreans ranging from the age of newborns up to adolescents, in which the vertebral body index (Ivb = v/s) and the intervertebral disk index (id = d/v) of each spine were measured and its interrelationship with normal growth rate was evaluated. The authors obtained the following results. 1. The vertebral body index showed gradual decrease with the advance of the age. It was significantly high in the group Ⅰ compared with other groups. 2. The difference between the boy and the girl was more significant at the 12th thoracic and 1st lumbar vertebral body than the othervertebrae in the group V. 3. Among children of more than 120 cm in height and older than 12 years of age, girls had a higher index of vertebral body than boys, the latter show a less pronounced vertical growth of the body. 4. The intervertebral disk index began to decrease after the 1st month of life possibly because of the accelerated vertical growth of the body from that period. 5. Of all the age groups, the intervertebral disk index showed a higher value in boys than in girls.

2.A case report of Angle's Class III malocclusion.

A girl aged 18 years and 1 month, had a Angle's Class Iff malocclusion, characterized by anterior crossbite, anterior crowding, and constriction from right lower 2nd p emolar to right lower 2nd molar This ~i5 5s patient underwent scaling and 84148 extraction, and multibanded system was placed. After 14 months, anterior crossbite and crowding was corrected, andbothdental arches were improved. After 6 months from debanding, band space disappeared, and any relapse was not detected.

3.Agents increasing intracellular calcium levels ameliorate the antodepressant-induced reduction of submandibular salivation in cast.

Tri- and tetra-cyclic antidepressants are known to cause dry mouth among other several major complications. The present study was designed to compare the degree of reduced salivation due to antidepressants and to explore whether intracellular calcium-increasing agents ameliorate the salivation. Effects of antidepressants and agents increasing intracellular calcium on the cholinergic submandibular secretion and blood flow induced by the chorda stimulation or intra-arterial acetylcholine were observed in anesthetized cats. Effects of antidepressants and calcium-mobilizing agents on K+ efflux were also observed in excised gland slices. The results obtained were as follows: 1. Salivary secretion in response to the chorda stimulation (3 V, 20 Hz, 1 msec) was significantly attenuated by antidepressants in a dose-dependent manner, whereas the blood flow was not affected. 2. Salivary secretion and increased blood flow evoked by intra-arterial acetylcholine (20 microgram/kg) were markedly diminished by antidepressants, the magnitude of which was amitryptyline>imipramine >mianserin in order. 3. Cholinergic salivation was significantly decrease by cyclopiazonic acid, a calcium pump inhibitor of the endoplasmic reticulum, or by BAPTA/AM, a specific intracellular calcium chelator. 4. Caffeine and ryanodine potentiated the cholinergic salivation and ameliorated the depressed salivary secreation due to antidepressants. 5. Calcium ionophore A 23187 ameliorated the depressed salivation due to antidepressants. 6. Antidepressants inhibited the K+ efflux, which were restored by caffeine or A 23187. These results suggest that the depressed salivary secreation due to antidepressants is ameliorated by increasing intracellular calcium levels.

6.Patterns of Vascular Invasion of Intrahepatic Peripheral Cholangiocarcinoma Examined with Angiography and Angiographic CT.

PURPOSE: To evaluate the radiological patterns of vascular invasion in peripheral cholangiocarcinomas. MATERIALS AND METHODS: Hepatic arteriography and portography in 20 cases with cholangiocarcinoma including 12 cases with anglographic CT were retrospectively analized. RESULTS: The arteriography showed no arterioportal shunt, hypertrophy of tumor vessel, or tumor staining extending to central portion of the mass in all cases. However, doughnut shaped peripheral tumor staining was seen until late hepatogram phase in 12 cases and compensatory hyperperfusion around the mass was seen in six cases(eight cases if include arterial CT). Encasement of tumor vessel was seen in 12 cases, and hypertrophy of feeding vessel in nine cases. On portogrphy, the filling defect on segmental portal branch could be demonstrated only in 11 cases. Shape of the portal defect was tapered narrowing in six cases, abrupt narrowing in two cases but intraluminal nodular filling defect was not seen. Remainning three cases were difficult to define the shape. On seven cases of CT during arterial portography, three cases showed mass shaped defect and four showed segmental defect but three of them could demonstrate the partially preserved portal flow in defective portal area. CONCLUSION: Hepatic arteriography in peripheral cholagiocarcinoma showed no evidence of hypertrophy of tumor vessels and tumor stain extending to central portion but peripheral staining on late hepatogram phase and compensatory hyperperfusion could be seen. Portal vein was more commonly involved through perivascular connective tissue invasion rather than by direct extension into the portal lumen.

8.Epidemiologic Survey of the Serlogic Markers of Hepatitis B Virus and Serologic Tests for Syphilis in Institutionalized Persons.

To survey the seroprevalence of hepatit!s B virus(HBV) in relation to serologic tests for syphilis(STS), using VDRL and TPHA test, the tests were done in the seri of 1252 institutionalized persons, who lived in Taegu Hope Village for the poor or crippled, and 1831 controls, who had the regular routine physical examination from March to May, 1987. The positive rates nf HBsAg and anti-HBs were 11.7%(146) and 27.0%(338'). compared with 6.25%(ll) and 18.4%(337) in controls, respectively. The prevalence of HBV including HBsAg and anti-HBs was higher in institutionalized persons (38,7+i) than in controls(24.6%)(p<0.0001). The positive rate of STS was higher in institutionalized persons(7.5%) than in controls(2.9%)(p<0.0001). There was a tvend for a higher rate of seropositivity in women than in men of institution- alized persons in the both diseases. The prevalence of HBV among STS-positive cases wa.s 42.6%(40) of instituti- onalized persons and 26.4%(14) of controls, but not, statistically significant, the gre,ater part of them distributing in 4th and 5th decades. The positive rates of I-tBi.Ag in institutionalized persons was 45.5%(5) of STS-positive cases and 25. 9%,'35) of STS-negative cases. No correlation was found between the serologic ma kers of HBV and VDRL titration.

10.Sural Vessels as Recipient Vessels for Free Flap Transfer to the Single Vessel Leg.

Free flaps have been widely used in lower leg reconstruction. However, in cases with extensive injury to the vessels as well as to the bone and soft tissues, and having only one intact major vessel in the lower leg(single vessel leg), careful selection of recipient vessels for the free flap transfer is mandatory for preventing further vascular compromise of the distal lower leg. The sural artery and its vena comitantes are frequently protected form externally harmful forces by the bulky surrounding gastrocnemius muscle and can be used as recipient vessles for free flaps without any detrimental influence on the vascularity of the distal lower leg. In our department, three latissimus dorsi muscle free flap were transferred with sural vessels as recipient vessels in the reconstruction of the single vessel legs. Except for one case of flap failure due to serious infection at the microvascular anastomoses site, all the other cases were successfully reconstructed without any necrosis of the gastrocnemius muscle or the distal leg. In addition, the sural vessels were easily accessed in any patient position and their caliber was similar to that of the vascular pedicles of the donor muscle flaps. In conclusion, the sural vessels can be another useful option in selecting recipient vessels for free flap transfer in the reconstruction of the single vessel leg.
