1.A Case of Multicentric Glioblastoma Multiforme.

A 46 year-old male with drowsy mentality and left hemiparesis had been treated. Right carotid angiogram showed a mass effect on the posterior temporal region. Preoperative CT brain scan revealed two separated hypodense masses with ring enhancement on the each hemisphere. The right-sided mass was totally removed and there was no invasion into the ventricle and meninges. The histological diagnosis was a glioblastoma multiforme. The left-sided mass was followed with the repeated CT scan, which showed the similar CT findings of a glioblastoma multiforme, and there was no evidence of commiccural extension. The above findings were consistent with the criteria of multicentric tumor.

2.Growth and Metabolic Consequences in Children Born with Small for Gestational Age at Term Gestation.

PURPOSE:The purpose of this study was to assess the growth status and to evaluate the factors affecting the development of metabolic syndrome in children born with small for gestational age (SGA) at term gestation. METHODS:We retrospectively reviewed medical records of 73 (36 boys, 37 girls) children born with SGA at term gestation who were visited for short stature and metabolic problems at Seoul National University Children' Hospital between 1994 and 2003. We assessed several clinical parameters as follows:birth weight, height standard deviation score (SDS), weight SDS, weight for height, bone age (BA), chronologic age (CA), predicted adult height (PAH) and midparental height (MPH). We evaluated the factors affecting height SDS according to pubertal status. We also evaluated the factors affecting development of type 2 diabetes mellitus, dyslipidemia or obesity. RESULTS:Pubertal children had greater height SDS, weight SDS and difference between BA and CA than prepubertal children, respectively (P<0.05). Height SDS positively correlated with difference between BA and CA (r=0.43, P<0.01), but BA advancement (BA>CA) was not observed in prepubertal children. BA advancement was observed in all pubertal children except one. The children who had greater weight SDS than height SDS had significantly lower height SDS and delayed BA, respectively (P<0.05). The children who had metabolic problems had higher weight SDS (P<0.05). CONCLUSION: This study suggests that rapid skeletal maturation may develope during transition from prepuberty to puberty and catch-up growth may contribute to the development of metabolic syndrome in children born with SGA, but further study will be required.

OBJECTIVE: Using the standardization of disease diagnosis of disease, and surgical procedures, the authors have made a clinical application for the analysis of inpatient data with the ability to search for information pertinent for writing of clinical articles. METHODS: A client-server system and database software was developed for networking. For clinical application, a computerized daily report has been developed. Data from Neurosurgical patients admitted at Soonchunyang University Hospital from January to December 1998 were analyzed with this system. Data for clinical articles was obtained using the search mode, information such as orbital infarction syndrome following intracranial aneurysm surgery and epidemiological analysis or geriatric neurosurgical patients. RESULTS: For the daily report it takes approximately 10 minutes to input the patients demographic information, name of disease, diagnosis and surgical procedure. The daily report also numbers and sorts the inpatients according to large categories of diagnosis, reports the ratio between inpatients and operative patients. The annual report that was obtained was very accurate and gave rapid statistics for the one year. By retrospective study for the past 18 years, we calculated the incidence of orbital infarction syndrome following intracranial aneurysm surgery as 1.4%, and also estimated the population of geriatric inpatients as 18.3% by retrospective study. CONCLUSIONS: It has been found to be most useful to make a daily and annual report for tracking and research purposes. For use in clinical articles, it can be possible to do a search of the patients using the standardized disease, diagnosis and neurosurgical procedures application and obtain pertinent information in a timely manner.

OBJECTIVE: The the international classification of disease and surgical procedure has been found to be a lengthy and time-consuming text for use by neurosurgeons. A more subject matter related classification system is needed for use by this specialty. The author has developed a more comprehensive and standardized classification system specified for diagnosis of neurological disease and neurosurgical procedures. METHODS: Standardization of disease was completed by modifying the name of disease according to that which was used for daily report past 10 years, and from textbook of neurosurgery. The fields of international coding are continued in each. Neurosurgical procedures were designated and modified according to Current Procedure Terminology 96. Client-server system will be used for networking and database software applications have been developed. RESULTS: Disease was classified in 14 large categories and 379 subcategories. Diagnosis was classified in 12 large categories and 43 subcategories. Neurosurgical procedure has 20 large and 202 subcategories. The international coding system such as ICD-10 and ICD-9CM is maintained for the diagnosis and procedures to each category. CONCLUSIONS: It could be possible to make and use a standardized database model of disease, diagnosis and neurosurgical procedures to be used by physician.

5.Acute Sciatica from Sacral Screw Impingement on the Lumbosacral Plexus: Emphasis on the Safe Zones for Sacral Screw Placement.

The authors present the case of sciatica due to bicortical sacral screw impingement on the lumbosacral plexus across the anterior sacrum. The placement of sacral screw across the anterior sacral cortex carries significant inherent risks to neurovascular and visceral structures. However, the clinical reports of lumbosacral plexus involvement by the misplaced screw are not well documented in the literature. This is an unique case of sciatica due to sacral screw impingement on the lumbosacral plexus after motor vehicle accident(MVA), and confirmed by CT scan and intraoperative electrical stimulation. We reviewed sacral anatomy and preventive measures for avoiding complication of this type are discussed.

6.Development of Effective Cryopreservation Method for Mouse Oocytes.

OBJECTIVE: The purpose of this study was to evaluate the efficacy and effect of various cryopreservation method on the survival and the cytoskeletal stability of metaphase II mouse oocyte. METHODS: Mouse ovulated oocytes were collected and cryopreserved by a modified slow-freezing method with 1.5 M 1,2-propanediol (PrOH)+0.1 M sucrose or by vitrification using cryo loop and EM grid with 40% ethylene glycol+0.6 M sucrose. Four hours after thawing, intact oocytes were fixed and stained with fluorescein isothiocyanate (FITC)-conjugated monoclonal anti-beta-tubulin antibody to visualize spindle and propidium iodide (PI) to visualize chromosome. Spindle morphology was classified as follows: normal (barrel-shaped), slightly and absolute abnormal (multipolar or absent). RESULTS: Survival rate of the frozen-thawed oocytes in vitrification group was significantly higher than that of slow-freezing group (62.7% vs. 24.4%, p<0.01). Vitrification with cryo loop showed significantly higher survival rate than that with EM grid (67.7% vs. 53.5%, p<0.05). On the other hand, proportion of normal spindle and chromosome configurations of the frozen-thawed oocytes between two vitrification group was not significantly different. CONCLUSION: For mouse ovulated oocytes, vitrification with cryo loop may be a preferable procedure compared to slow-freezing method. Further study should be needed to investigate developmental competency of frozen-thawed mouse oocytes.

8.Tinnitus Assessment.

Tinnitus is a common medical condition where patients perceive unwanted noise in the absence of environmental stimuli. However, it is very difficult to find out definite causes of each patient's tinnitus. According to tinnitus classification systems, there are many different diagnostic tools. Representatively, tinnitus can be divided into objective and subjective tinnitus. Also, there are many etiologic factors for each classified tinnitus. Tinnitus assessment is defined as finding out the exact cause of tinnitus and prediction of treatment effect. It starts from history taking and physical examination to audiologic evaluations. Also, it is an important part in the relationship with the patient, as well as tinnitus objectification in the treatment process. And, it affects the consequent management of tinnitus. Otherwise, assessment of tinnitus is a challenging task due its subjective nature. Therefore, a multidimensional diagnostic approach to tinnitus is very important in appropriate treatment of tinnitus and in predicting the effect of the management.

9.CT Features of Appendiceal Mucocele.

PURPOSE: To evaluate the usefulness of CT features of appendiceal mucocele in the diagnosis and evaluation of complications. MATERIALS AND METHODS: We retrospectively reviewed CT findings and compared with operative findings in 7 cases of pathologically proven appendiceal mucocele. CT findings such as location and extent of the lesion, Issue density, thickness or calcification of the wall, presence of adjacent inflammatory infiltration, and visualization of normal vermiform appendix were analyzed. RESULTS: Appendiceal mucocele was found as homogeneous low density cystic mass adjacent to the cecum, which has no surrounding inflammatory infiltration except in one case of perforation and one case of intussusception. Mean CT number measured in 4 cases was 21 Hounsfield unit. Thin curvilinear calcifications were noted along the cystic wall in 2 cases. Normal vermiform appendix couldn't be demonstrated in all cases. CONCLUSION: Appendiceal mucocele is characterized by homogeneously low density and thin walled cystic tumor adjacent to cecum without surrounding inflammatory infiltration, and absence of normal vermiform appendix on CT. Therefore, CT is valuable in preventing operative complications of appendiceal mucocele.

10.Virulence and antimicrobial resistance genes of pathogenic Escherichia coli from piglets showing diarrhea before and after ban on antibiotic growth promoters in feed

Abstract: This study examined the prevalence of adherence factors, toxin genes, antimicrobial resistance phenotypes, and resistance genes in Escherichia coli (E. coli) isolated from piglets with diarrhea before and after the ban on antibiotic growth promoters (AGPs) in Korea from 2007 to 2018. In this period, pathogenic 474 E. coli isolates were obtained from diarrheic piglets. The virulence factors and antimicrobial resistance genes were assayed using a polymerase chain reaction, and the susceptibility to antibiotics was tested according to the Clinical and Laboratory Standards Institute guidelines. After the ban on AGPs, the frequency of F4 (12.5% to 32.7%) increased significantly, and LT (31.9% to 20.3%) and EAST-I (46.5% to 35.2%) decreased significantly. In addition, the resistance to streptomycin (45.8% to 67.9%), cephalothin (34.0% to 59.4%), and cefazlin (10.4% to 28.8%) increased significantly. Colistin resistance plasmid-mediated genes, mcr-1 and mcr-3, were detected after the ban on AGPs. The results of this study can provide useful data for analyzing the impact of the ban on AGPs on the virulence profiles and antimicrobial resistance of E. coli isolated from piglets with diarrhea in Korea.
