1.A Case of Multiple Intraosseous Lipomas

Fatty tumours are,the most common soft tissue tumours whereas they are among the rarest tumours in bone. Dahlin quoted an incidence of 1 in 1, 000 of all bone tumours, and fatty tumours in bone are almost single intraosseous lipoma and of four types; I. Soft tissue lipomata or liposarcomata, secondarily affecting bone. 2. Parosteal lipomata. 3. Intraosseous lipomata. 4. Liposarcoma of bone. Here we report a case of multiple intraosseous lipomas in a 23 year old woman.

2.Clinical Study of the Fracture of the Carpal Scaphoid

Ten cases of fracture of the carpal scaphoid analysed clinically and treated by admission at Department of Orthopedic Surgery, National Police Hospital during the period of 1975 to 1980. The results were summerized as follows: 1. The mode of the injury were consist of 3 in football game, 2 in judo, 2 in army training and 1 in fallen from height, 1 in slipping down, 1 in direct blow and almost falling down by outstretched hand. 2. Of the 10 cases, there were 9 male and 1 female patients. This result showed marked predominence of male over female patients and most commonly occured between 20 and 29 of age. 3. In 8 cases, the fracture was found on the waist of the carpal scaphoid. 4. Nonunion of the carpal scaphoid were noticed in 8 cases. 5. 2 cases of the recent fracture, there were appeared by rechecking X.-ray after 2 weeks. 6. The good results were obtained by bone graft according to method of Russe.

4.A Case of Woringer Kolopp Disease Treated with Topical PUVA.

A 26-year-old Korean man had an erythematous scaly plaque on his right thigh for fifteen years. He was diagnosed with chronic eczema and treated with topical steroid, but no clinical improvement was obtained for 15 years. Laboratory studies including complete blood cell count, urinalysis, liver and renal function test, and peripheral blood smear were normal. A biopsy specimen showed hyperkeratosis, acanthosis, and mononuclear cellular infiltration in the epidermis. These cells had a perinuclear halo and showed a Pautrier's microabscess like configuration. In the dermis, there was a band like infiltration of inflammatory cells. Infiltrates of the epidermis were negative for leukocyte common antigen(CD45) and positive for Pan T cell(CD45RO). He was treated with topical PUVA twice a week for twelve weeks. Clinical and pathological improvements were obtained. We propose PUVA may be a useful therapeutic modality to treat Woringer Kolopp disease.

5.Apoptosis and Expression of bcl-2, p53, and Ki-67 in Mycosis Fungoides.

BACKGROUND: Mycosis fungoides(MF) is a form of cutaneous T cell lymphoma with clonal differentiation of helpr' T cell. It has a patch, plaque, and tumor stage. But pathogenetic factors controlling the development and progression of MF are still unclear. Apoptosis plays a major role in developmental biology and homeostasis. The bcl-2 oncogene prolongs ce11 life by inhibiting apoptosis. The mutant pS3 gene induces apoptosis indirectly. Ki-67 antigen is the cell proliferation marker. Recently, it has been shown that the relationships among them are important in the tumorigenesis of the various tumors. OBJECTIVE: The aim of this study was to examine the expression of these genes and apoptotic rate and clarify the relationship among them in the development and progression of MF. METHODS: The eighteen specimens from 8 patients with MF and 10 specimens from benign lymphocytic infiltrating diseases including 5 lichen planus, 3 lupus erythematosus, and 2 contact dermatitis were included. We performed immunoperoxidase staining(LSAB technique) using monoclonal antibodies including bc1-2, p~53, and Ki-67(MIB1). We used ApoptaqTM(Oncor) in situ labelling kit for detecting apoptotic cell.

6.CT Arthrographic Finding in Shoulder Instability.

The purpose of this study is to evaluate the morphologic findings of the labrum and capsuloligamentous structures in the normal and unstable shoulders and to compare the effectiveness of double contrast arthrographic findings which were taken with supine and prone position. We reviewed the computerized tomographic arthrogram in 9 cases of normal shoulder and l3 cases of shoulder instability. We evaluated the shape of the labrum, the attachment of anterior capsule and the amount of capsular laxity. Capsular laxity was compared each others in normal, the unidirectional instability and the multidirectional instability. Among these materials, we simultaneously performed CT arthrogram with supine and prone position and evaluated which is more reliable and accurate. Scapular inclination was ahout 25 degrees in the supine position and ahout 60 degrees in the prone position. Normal glenoid labrum varied in size and shape, so its lack of a classic appearance might not indicate a labral tear. Variations existed in the anterior capsular attachment onto the glenoid. The anterior capsule was used to attach far away from the glenoid rim. but it might not indicate an anterior instability. With the prone position, the labral lesion took more excellent visualization due to the contrast coating on the glenoid labrum in both anterior and posterior. Anterior capsular expansion was seen well with prone position in anterior instability. In summary, for proper interpretation of the instability, awareness of the morphologic variations of the labrum and capsuloligamentous structure is important. CT double contrast arthrography with prone position has made a significant contribution to improving and easing the documentation of variety of anatomic abnormalities. Now we recommend the prone position in CT arthrogram for imaging glenoid labrum pathology in shoulder instability.

9.The Morphometric Study on Cruciate and Patellar Ligaments in Korean Adults

In the endoscopic one-incision anterior cruciate ligament(ACL) reconstruction, placing the graft's tendon-bone interface flush with the intraarticular femoral tunnel results in frequent distal graft protrusion. On the contrary, we occasionally found that the intraarticular posterior cruciate ligament(PCL) length was longer than patellar tendon length and had a great difficulty in performing the endoscopic one-incision technique of PCL reconstruction. The purpose of this study is to get a certain guideline in the endoscopic one-incision technique of cruciate ligament reconstruction by measuring intraarticular distance of ACL, PCL and patellar tendon and by measuring distance between the insertion of pes anserinus and medial tibial plateau in 19 cadaveric knees. The results were as follows; 1. The average patellar tendon length was 36.2±4.7mm(range 24.7-45.0). 2. The average ACL length was 23.7±3.9mm(range 16.5-30.3). 3. The average PCL length was 32.7±3.8mm(range 25.4-41.0). 4. The average distance between the insertion of pes anserinus and medial tibial plateau was 38.3±5.4mm(range 27.2-53.6). Clinical Relevance: Since Korean patellar tendon is shorter than Caucasian's and intraarticular distance of ACL is same as Caucasian's, graft-tunnel mismatch doesn't seem to be a major problem with the accurate technique of the endoscopic one-incision ACL reconstruction in Korean patients. It is possible to interfere with the pes anserinus if the length of the tibial tunnel is needed more than 44.8 mm, It is very difficult to perform an endoscopic one-incision PCL reconstruction if the length of patellar tendon is less than 35mm because intraarticular distance of PCL is sometimes longer than patellar tendon in such cases.

10.A Clinical Study for Oxytocin use of Labor Induction.

OBJECTIVE: The objective of this study is to compare the effectiveness and safety in the labor induction between the high dose oxytocin method and the new low dose oxytocin method. STUDY DESIGN: Firstly, we selected 125 pregnant women hospitalized, having the indication of labor induction from March, 1995 to August, 1996. Of them, we selected 61 pregnant women tothem the high dose oxytocin method was used, as the control group, and in- creased the quantity of 2.5 mU/min every 20 minutes with the start dose of 2.5 mU/min to them. On the other hand, with the start dose of 1.25 mU/min, we increased the quantity of 1.25 mU/min every 20 minutes to the study group of low dose oxytocin method, 64 pregnant women. RESULTS: No statistical significance was found in the time from the effective uterine contraction to the delivery in the study group, in contrast to that of the control group to them the labor induction was conducted by using the high dose oxytocin. Maximum amount used to the high dose oxytocin was significantly more than that of the low dose oxytacin, but in the total given dose, there was no significant difference between two groups. Maxi- mum uterine contraction of the control group did not show any significant. difference from that of the study group, and there was also no significant difference in the frequency of generating the complications such as fetal distress. CONCLUSION: There was no difference in the labor.induction -to delivery time, and the complications of fetus, between the existing high dose oxytocin method and the new low dose oxytocin method. Therefore it is thought the low dose oxytocin method may reduce the possibility of a complieation compared with the high dose oxytocin method. However, it is considered this matter must be investigated further in the futrre.
