1.Cemented Total Hip Replacement Arthroplasty.

The use of cement in total hip replacement arthroplasty has long controversial. However, since the 1980s, osteolysis has occurred with high frequency in cementless total hip replacement arthroplasty, and has been a significant cause for loosening. Recently, a cemented femoral stem has been frequently used because of improvements in cement techniques, materials for joint arthroplasty, design, etc. Also, the use of an acetabular cup with cement seems desirable where indications are for a revision procedure or where there is an aged patient with severe osteoporosis, and where the patient requires a broad bone graft due to an acetabular bone defect. The purpose of this article was (i) to review how to fix an acetabular cup with cement and a femoral stem in current hip replacement arthroplasty procedures and (ii) to review possible directions for further development.

3.Distal Radius Fractures Using Acu-Loc Volar Plate.

OBJECTIVE: We determined the radiographic outcome of distal radius fractures with Acu-loc volar plate. METHODS: Forty-one patients were recruited between August 2009 and September 2010. There were 10 males and 31 females, with a mean age 61.2. Fractures were radiologically classification the Frykman. Fifteen fractures were group 8, eight were group 7, ten were group 6, four were group 5 and four were group 3. Distal part of the fractures was dorsally inclined in 37 wrists and inclined to volar side in four wrists. Lateral and anteroposterior radiographs taken after operated day, the fracture were compared with radiographs of the injured wrist and the differences in palmar tilt, ulnar variance; radial height; radial shift and radial inclination were measured. RESULTS: There was significant improvement in the measurements of radial height, radial inclination, volar tilt, ulnar variance and radial shift postoperatively. The radial height improved from an average of 8.5 mm (range, 3 to 15 mm) to 11.0 mm (range, 8 to 15 mm), the radial inclination improved from an average of 21.5 degree (range, 10 to 40 degree) to 28.1 degree (range, 19 to 44 degree), the palma tilt improved from an average of 12.9 degree (range, 6 to 22 degree) to 17.2 degree (range, 7 to 27 degree), the ulnar variance improved from an average of -2.3 mm (range, -6 to 4 mm) to 1 mm (range, -3 to 7 mm) and the radial shift improved from an average of 18.7 mm (range, 15 to 26 mm) to 17.3 mm (range, 12 to 21 mm). CONCLUSION: Acu-loc volar plate is a safe and effective device.

4.Bilobed Sensate Radial Forearm Free Flap for Functional Reconstruction Following Hemiglossectomy.

After hemiglossectomy due to tongue cancer, such factors as mobility, sensation, volume and the shape of the tongue are critical elements for successful rehabilitation. As long as there is a residual segment of mobile tongue with an intact motor nerve supply, the mobility of native tongue can be preserved by using tissue with thin, pliable and sensory potential. The residual tongue motion can be maximized by separating the reconstruction of mobile tongue from the reconstruction of the floor of mouth. Six patients who underwent significant glossectomy for squamous cell cancer were reconstructed with a bilobed design of sensate radial forearm flap. One lobe of the flap is used to restore the shape and volume of the tongue, while the other lobe is used to resurface the floor of the mouth. There were no flap failure and fistula formation. In all patients, the tongue mobility was preserved and the earliest recovery of sensation was noted 5 weeks following the surgery. The articulation was good enough for them to perform daily activities. With the bilobed design, we can maximize the mobility of the residual tongue by preventing tethering of the root of tongue to the inner table of the mandible. In this article e present our successful experiences with this method, especially in view of mobility and sensation.

5.Symptom Prevalence of Work-Related Musculoskeletal Disorders and Related Factors among Bank Workers by Visual Dispaly Terminal Use .

The purpose of this study is to investigate the relationship between the self-reported musculoskeletal symptoms and related factors among VDT operators in bank. The subjects were 950 women workers. The characteristics of subjects were 26.6 years of average age, 5.7 years of average VDT use duration and 6.5 hours of daily VDT use time. The results were as follows. The prevalence of self-reported musculoskeletal symptoms more than severe in Likert scale were 51.4%, 43.9% at the shoulder, 38.3%, 31.8% at the lower back, 38.0%, 31.3% at the neck, 31.2%, 25.2% at the upper back, 21.7%, 16.5% at the wrist and 13.6%, 8.8% at the fingers (while on duty, after home-coming). General characteristic variables affecting the musculoskeletal symptoms were married, number of child and daily house-working time. Work-related factors affecting the musculoskeletal symptoms were VDT working career, daily working time, daily VDF use time, VDF use time without rest, daily work-load and regular rest. As the result of the multiple logistic regression analysis, daily working time, daily VDT use time and VDT working career were significant variables accounting for the musculoskeletal symptoms.

6.Assessment of dietary quality and nutrient intake of obese children in Changwon area

Purpose:
In this study, we assessed the intake of nutrients and food groups and analyzed the nutrition quotient of obese children in South Korea. The hypothesis was that dietary quality and nutrient intake are related to the body mass index (BMI) of obese children.
Methods:
The participants included 48 children (20 boys and 28 girls). Based on the guidelines for the age-for-body mass index provided by the Korean National Growth Charts for children and adolescents, the participants were divided into 3 groups: overweight, BMI ≥ 85th percentile; obese, BMI ≥ 95th percentile; severely obese, BMI ≥ 99th percentile.
Results:
The energy distribution showed that the carbohydrate ratio was significantly higher in the overweight group (p < 0.01), whereas the fat ratio was significantly higher in the obese and severely obese groups (p < 0.05). Thus, in the overweight and obese groups, the percent body fat was negatively associated with the carbohydrate ratio (p < 0.05) but was positively associated with the fat ratio (p < 0.05). The Dietary Reference Intakes (DRI) data revealed that the intake levels of protein, vitamin B 1 , vitamin B 12 , and sodium were higher in all groups. Intakes of fiber, calcium, potassium, and magnesium were insufficient in all groups, and the DRI percent for protein was significantly higher in the obese and severely obese groups than in the overweight group. No significant differences were obtained in food group patterns or Nutrition Quotient (NQ) scores among the 3 groups. According to the evaluation results by food record and NQ, a significant difference was obtained in the dietary quality of obese children.
Conclusion
We conclude that a higher intake of fat enhances weight gain. Based on the study results, we propose that the guidelines should consider the energy distribution of carbohydrate and fat intake to prevent and control obesity among Korean children.
7.A Case of Pure Red Cell Aplasia Associated with Sinus Histiocytosis with Multiple Lymphadenopathy.

Pure red cell aplasia (PRCA) is characterized by severe anemia with reticulocytopenia and absence of erythroblast from the bone marrow. Sinus histiocytosis with massive lymphadenopathy (SHML : Rosai-Dorfman disease) is rare systemic disease characterized by painless cervical, axillary, inguinal and mediastinal lymphadenopathy and frequent extranodal invasion. Histologically, lymph node sinuses are expanded by numerous distinctive histiocytes which contains well preserved lymphocytes. We report a case of sinus histiocytosis with massive lymphadenopathy in inguinal lymph node biopsy with polyclonal gammopathy and pure red cell aplasia in bone marrow biopsy who was infected by Epstein-Barr virus.

8.How to get the Best Results in Transaxillary Breast Augmentation with Anatomic form Stable Implants: Pocket Shapes, Insertion, and Maintenance.

Although IMF incision is known the best way for anatomic implant, most Korean doctors and patients hesitate IMF incision. Anatomic form stable implants have some benefits such as less prominent upper pole, less wrinkles and ripples, and less rupture rate than round cohesive type I implants. However more concern is necessory for placing the anatomic implants. The Korean Academic Association of Breast Surgery(KAABS) planned to support some tips for using anatomic form stable implants through axillary incision. The KAABS gathered and analyzed the concepts of Korean plastic surgeons who have experienced transaxillary breast augmentation with anatomic form stable implants. The KAABS requested them of their concepts of 9 basic categories: entrance dissection, pocket dissection, lubricant, inserting aids, skin protector, inserting direction, suction drainage, dressing, compression garment, and their key considerations. Eight expert surgeons suggested their own cutting edge methods of transaxillary breast augmentation with the anatomic form stable implant, however each surgeon should find his or her own method. Authors and KAABS hope that these developing and incomplete concepts help beginners to find their own concepts.

9.Health Evaluation of ex-workers occupationally exposed to Carbon disulfide: Subjective Symptoms and Related Factors according to compensation for CS2 poisoning.

OBJECTIVES: To investigate the chronological subjective symptoms and related factors in ex-workers who were occupationally exposed to Carbon disulfide. METHODS: One hundred and seventy-seven subjects (147 males and 30 females) were randomly selected among ex-workers and interviewed by well trained doctors, who filled out a structured questionnaire developed by authors on the subjective symptoms. The questionnaire was composed of symptoms in the head and neck, and those of the neuro-psychological, respiratory, endocrine, musculoskeletal, and reproductive systems. Other factors such as general and occupational characteristics including work department, work duration, duration since cessation of exposure, and compensation for occupational disease were also investigated. RESULTS: The mean subject age was 55.4 years. The mean cumulative exposure index (CEI) was 38.96 +/- 31.18. CEI was significantly higher in the compensated group (47.61 +/- 32.51) than in the non-compensated group (26.81 +/- 24.75)(p=0.000). Symptom complaints were significantly higher in the compensated group in all organ systems, and their incidence increased gradually until 10 years after cessation of work, and then decreased. The mean duration for cessation of exposure at the time of compensation was 10.97 years. The significantly different factors between the compensated group and the non-compensated group were total cumulative exposure index and the time interval between the cessation of work and the time of compensation. CONCLUSION: Our findings suggest that total cumulative exposure index and the time interval between the cessation of work and the time of compensation may be important factors affecting the compensation for occupational disease. Therefore, further studies will be necessary.

10.Impact of Visceral Adiposity Measured by Abdominal Computed Tomography on Pulmonary Function.

Although an inverse relationship between abdominal adiposity and pulmonary function has been suggested, direct measurement of abdominal adipose tissue has rarely been attempted. Our object is to determine the impact of abdominal adiposity on pulmonary function by directly measuring abdominal adipose tissue with abdominal computed tomography (CT). In this cross-sectional study, we included never-smokers between the ages of 18 and 85 yr, who had undergone spirometry and abdominal adipose tissue analysis with CT scans during November 1, 2005 to October 31, 2009 as part of the comprehensive health examination. Among a total of 3,469 participants, 890 (25.7%) were male. The mean body mass index and waist circumference among males and females were 24.6 kg/m2 and 87.8 cm and 23.0 kg/m2 and 83.0 cm, respectively. Although total adipose tissue (TAT) of the abdomen in males (269.1 cm2) was similar to that in females (273.6 cm2), the ratio of visceral adipose tissue (VAT)/subcutaneous adipose tissue (SAT) was different; 0.99 in males and 0.50 in females. In males, TAT, SAT, and VAT were inversely associated with the absolute value of forced vital capacity (FVC), and TAT and VAT were inversely associated with forced expiratory volume in one second (FEV1). However, in females, TAT and VAT, but not SAT, were inversely associated with absolute FVC and FEV1 values. In conclusion, the amount of abdominal adipose tissue directly measured using CT is inversely associated with lung function.
