Hemolytic disease of newborn due to Rhesus incompatibility occurs mostly by anti-Rh(D) antibodies. But recently, because of Rh(D) immune globuin prophylaxis, Rh(D) incompatibility is decreased and importance of minor group incompatibility is increasing. The majority of minor blood group incompatibilities are anti-c, anti-E or anti-Kell, but hemolytic diseases caused by combination of these antibodies are relatively rare. The 21 day-old male neonate was admitted because of pallor and poor feeding for 2 days. Laboratory data showed severe normochromic and normocytic anemia with increased reticulocyte and bilirubin. Patient's and his mothers blood type was B, Rh(D) positive. Minor blood groups of mother's and sons were different and anti-c and anti-E antibody were detected in mother's serum. After transfusions of two times with anti-c and anti-E free B, Rh(D) positive blood, the baby was discharged at 11th hospital day with good general condition.

Primary pulmonary hypertension is a rar, incurable, and progressive clinical entity. When associated with pregnancy, the prognosis of primary pulmonary hypertension is worsened with maternal mortality rates of at least 50%. The patient was a 29-year-old mother in her 2nd pregnancy, with previous uncomplicated gestation, 5 years ago. She had been well until the 28th week of present gestation when she was admitted because of increasing dyspnea on exertion. She was a housewife with no remarkable family and past histories. We have experienced a patient with primary pulmonary hypertension in pregnancy. She was treated with an oral calcium-channel blocker and low-molecular-weight heparin and was delivered vaginally with good maternal and fetal outcome. So we report a case of primary pulmonary hypertension associated with pregnancy and review literature.

The fibromatosis is a rare timorous with local invasion, but is not metastasized distantly. This term should not be applied to nonspecific reactive fibrous proliferations that are part of an inflammatory process of are secondary to injury of hemorrhage and have no tendency toward growth or recurrence. It arises principally from the connective tissue of muscle and overlying fascia or aponeurosis (musculoaponeurotic fibromatosis), and chiefly affects the muscle of shoulder, pelvic girdle, and extremity. The term 'aggressive fibromatosis' is also employed to describe this disease, but it is impossible to predict the clinical course in the individual case. The fibromatosis arising in the mediastinum is very rare, and the report about it is nearly absent. The plain radiography shows merely mass with soft tissue density. The CT demonstrates a poorly defined homogenous or heterogeneous mass, isodense with skeletal muscle on precontrast-images, and slightly hyperdense to muscle on postcontrast-scan. Accurate delineation between the tumor & surrounding tissue is vague or frequently impossible. The authors experienced one case of the mediastinal fibromatosis recently and report the case with review of concerned literature.

We reviewed 174 total hip arthroplasty (25 were revision procedure) and 65 bipolar hemiarthroplasty procedure followed by more than 6 months at Chungnam National University Hospital from July, 1992 to March, 1995. These were performed on 189 male and 50 female patients. We have followed up average 45.8 months and obtained following results; The dislocation rate is 3.76% (9 of 239 cases) and the dislocation rate of revision arthroplasties (16%: 4 of 25 cases) is higher than those of primary hip arthroplasties (2.6%: 5 of 214 cases). Among Nine dislocations, 8 cases (89%) were between fifth and seventh decade and all cases are men. All dislocations occured within 5 weeks postoperatively. Because 6 of 9 cases were due to poor compliance, the patient's cooperation during postoperative recovery is essential to prevent postoperative dislocation. The capsulectomy, surgical approach, or immobilization of patients after operation are unrelated to dislocaton. For the prosthetic factors, 7 cases dislocated posteriorly is appeared that the plane of the cup was between 3 and 9 degrees of anteversion, and also less than the normal range. Stable outcome without surgical operation was achieved in 7 of 9 dislocation. Among 2 patients who had recurrent dislocation after optimum conservative treatment, one had infected arthroplasty and the other had a impinged anterior thick capsule formation around the head. We concluded that the predisposing factors for dislocation after hip arthroplasty is multifactorial.

We performed a retrospective analysis of twenty-four consecutive hip arthroplasties (Total Hip Arthroplasty 18, Bipolar Arthroplasty 1, Cup revision 5) during which intraoperative frozen sections were analyzed to identify the occult active infection. We also reviewed the data such as laboratory findings (ESR, CRP), intraoperative culture, hip joint sonographic evaluation and permanent histologic section. Among the twenty-four patients those who received hip arthroplasties, 10 cases received revisional hip arthroplasties due to painful hip prosthesis loosening, 9 cases with infected hip prosthesis, 3 cases with infection after open reduction and internal fixation due to hip fracture and 2 cases with septic hip sequelae. Intraoperative frozen section had been obtained during staged revisional hip arthroplasties to determine the presence of active infection. We considered positive for infection if it shows more than 10 PML/HPF (Polymorphonuclear leukocyte /high power field) in at least 5 distinct microscopic fields from intraoperative tissue frozen section and postponed implantation of prosthesis. If it shows less than 5 PML/HPF, we performed revisional hip prosthesis implantation. We decided the implantation based on patient's condition, laboratory findings and intraoperative tissue conditions if it shows between 5 and 10 PML/HPF from intraoperative frozen section. At last follow up, 20 of 21 patients who had a revisional hip arthroplasties as below 10 PML/HPF from intraoperative frozen section remained free of infection (Specificity; 95%). We concluded that analysis of the intraoperative frozen sections is a reliable predictor as a guide to sepsis for the successful hip joint arthroplasties.

Charcot–Marie–Tooth (CMT) disease is the most common hereditary peripheral neuropathy, affecting the motor and sensory nerves and manifesting a range of systemic symptoms. CMT disease encompasses a range of genetic subtypes, with symptoms often emerging in childhood and progressing into adulthood. More than 70% of CMT patients exhibit cavovarus foot deformities that impair normal gait, often requiring the use of a brace. A diagnosis relies heavily on a thorough physical examination of each tendon contributing to the deformity. Conservative treatments, such as shoe modifications and braces, are initially preferred. Surgical reconstruction may be considered if conservative management fails. Patients who require surgery typically present with progressive cavovarus deformities, muscular imbalances, soft tissue contractures, and abnormal bone morphology. For a CMT foot reconstruction, joint-sparing surgery with soft tissue release and osteotomy is recommended. Fusion surgery is advised for deformities that are irreducible by joint-preserving surgery or in cases with painful arthritis. CMT foot surgery can enhance the patient’s ability to ambulate without a brace, leading to high satisfaction and improved quality of life.
