Based on the theory that benign prostatic hypertrophy may be induced by androgenic effect of testosterone derivatives, especially 5-alpha - dihydrotestosterone, on prostatic tissue, Chlormadinone acetate(CMA), potent oral synthetic antiandrogen was investigated in the treatment of benign prostatic hypertrophy. Twenty-two patients of prostatic hypertrophy were studied over six months period with a special reference to uroflowmetry and following results were obtained : 1) Chlormadinone acetate induced improvement of obstructive urinary symptoms in terms of uroflowmetric measurement. 2) It is very worthwhile to initiate medical treatment before undergoing any surgical intervention or when surgery is contraindicated.

PURPOSE: In patient with blunt trauma of chest, supine AP x-ray cannot differenciate the lung contusion, laceration, atelectasis, and hemothorax definitely. Therefore, computed tomographic evaluation is needed for accurate evaluation of the injuries. In our knowledge, there are few reports about CT findings of blunt chest trauma, in our country, therefore we tried to fiud the characteristic CT findings in patients with blunt trauma. MATERIALS AND METHODS: We analyzed the plain x-ray and CT image of 4 patient with blunt chest trauma. Location and morphology of lung parenchymal contusion and laceration, hemopneumothorax, chest wall injuries and location of chest tube. RESULTS: Lung parenchymal contusion was noted in 53 segments. of 16 patiants'infiltration(n=27 segment), and multiple nodular pattern was noted in 15 segment, pattern of consolidation along the lung periphery was seen in 11 segment. Laceration was noted in 18 lesion and most commonly located in paravertebral area(b=8). CONCLUSION: CT scan of chest in patient with blunt chest trauma, provides accurate informations of the pattern of injuries, and localization, therefore, should be performed as po9ssible.

Forty five patients with urinary tract infection were treated with 100mg Norfloxacin twice daily for 3 or 7 days according to the seriousness of infection. Of the 19 patients with uncomplicated urinary tract infection including 5 patients with acute bacterial prostatitis or acute epididymitis, none had bacteriuria 7 days after therapy and 1 patient was reinfected 14 days after therapy discontinuation. Of the 26 patients with complicated urinary tract infection, 17 patients(65%) were uninfected 7days after therapy and among them, 3 patients were reinfected I4 days after discontinuation of therapy. Of the 9 patients with persistent infection, 4 patients had resistant bacteriuria and 5 patients had bacteriuria different from that found before therapy. Eradication rate of E. coli, Enterobacter spp., Proteus spp., Pseudomonas spp., and Serratia spp. were 100%,, 100%,, 100%,, 80% and 40%, respectively. Further clinical envaluation seems worthwhile especially in pseudomonas infections since it offers one of the very few alternatives for oral treatment of pseudomonas infections which today may require hospitalization only for the purpose of injectible antibiotics.

Ganglion cysts of the cruciate ligament are quite rare. Ganglion cysts on the anterior cruciate ligament(ACL) should be suspected in any patient having pain and clicking sensation during terminal knee extension. A few cases have been reported concerning ganglion cysts present on the surface of the anterior cruciate ligament. We report three cases of a ganglion cysts on the ACL that was treated with arthroscopic excision.

The application of the arthroscope to the shoulder joint has evolved in recent years for the purpose of diagnosis and treatment. This paper presented 11 cases with various shoulder diseases treated by arthroscopy during the period from January, 1984 to September, 1985 in the Department of Orthopedic Surgery, College of Medicine, Soon Chun Hyang University. The advantages of the arthroscopy were as follows: 1. The arthroscopy made it easier to diagnose the unsolved cases by direct biopsy on the pathologic lesions in the joint. 2. The damage on the inside and outside of the shoulder joint was minimal and the post-operative incision scar was acceptable cosmetically. 3. The limitation of the joint motion did not remain due to early exercise post-operatively. 4. The complications such as infection, neurovascular injury, scuffing of joint cartilage, etc. could be avoided.

Malignant gastrointestinal stromal tumors (GISTs) are rare non-epithelial, mesenchymal neoplasms of the gastrointestinal tract that metastasize or recur in 30% of patients who undergo surgical resection with curative intent. A 59-year-old man visited our hospital for an examination of a palpable mass in the left abdomen. Fourteen months prior to his visit, the patient underwent gastric wedge resection to remove a GIST of the gastric cardia. At the time of surgery, no evidence of metastatic disease was observed and the pathological interpretation was a high-risk GIST. A follow-up computed tomography scan of the abdomen revealed a partially necrotic solid mass (9.8 x 7.6 cm) and enhancing mass in the spleen (2.3 cm). On exploration, multiple masses were found in the liver, greater omentum, and mesentery. Here, we report a case of recurring GIST of the stomach that metastasized to the spleen. To the best of our knowledge, few reports of metastasis to the spleen exist.

Disagreement exists about the accuracy of excretory urography in the staging of bladder cancer. During a 2O year period 1O8 of 537 patients with transitional cell carcinoma of the bladder had ureteral obstruction on excretory urography at the time of initial diagnosis. 93 patients (86%) had muscle invasion at staging transurethral resection or open resection. Of those patients 37 had total cystectomy, 4 had partial cystectomy, 5 had curative radiation therapy and the remainder had no further treatment. Ureteral obstruction at the time of initial diagnosis of bladder cancer usually indicates muscle invasion and/or metastases. We believe that our data support the usefulness of excretory urography as a staging tool in carcinoma of the bladder.

A retrospective analysis of 25 patients seen at Seoul National University Hospital in whom transitional cell carcinoma of the bladder developed at age thirty or younger was conducted to determine whether age alone influenced the course of disease or not. The tumors are usually low grade and low stage. There was no progression of disease in recurrent cases. There was no correlation between recurrence and size or multiplicity of bladder tumor in patients less than 31 years old. In cases of invasive and metastatic tumors, no evidence was found to suggest that transitional cell carcinoma of the bladder in patients thirty years of age and under differs clinically from that of older patients.
