In cholangiographic techniques, the close relationship between choledochal cyst and anomalous union of pancreaticobiliary duct has attracted medical attention. There have been rare cases in which the papilla of Vater was found in a position other than its normal position, and such cases have been reported sporadically. However, such cases are interesting in the anatomical context. In this review, we present our experience of choledochal cyst in a 30-month-old boy in whom the papilla of Vater was positioned in the third portion of the duodenum.

We treated 617 patients who were diagnosed as rotator cuff disease at the Department of Orthopaedic Surgery, Asan Medical Center in Korea between March, 1995 and December, 1996. We evaluated 175 cases who were followed up over five months and were treated according to our home treatment protocol. Average follow-up period was eleven months(range, five to eighteen months). We educated every patient who were not suspected of having full thickness tear of the rotator cuff based on clinical and MRI findings. According to patients condition, we treated them with both stretching and strengthening exercise or strengthening exercise only. This physician-directed home treatment was occasionally supported by the use of NSAID in case severe pain. Patients did stretching and strengthening exercise in 93(53%)cases, and strengthening exercise initially in 82(47%)cases. One hundred and forty five(83%) cases had satisfactory results. Home treatment of the rotator cuff disease is effective, convenient and not dangerous. We recommend this physiciandirected home treatment for primary management of almost all the patient with rotator cuff disease except full thickness tear of the rotator cuff tendon.

Hepatic masses show different enhancing patterns in N bolus computed tomography: Hepatocellular carcinoma shows high-attenuation in the early enhancing phase and low-attenuation in the late enhancing phase, hemangioma shows peripheral dot-like high-attenuation in the early enhancing phase and central high-attenuation in the late enhancing phase, and metastatic cancer and cholangiocelluar carcinoma show peripheral high-attenuation rim in the early enhancing phase and central portion gradulally high attenuation in the late enhancing phase. but sometimes enhancing patterns of the hepatic masses are confuse. To evaluate the significance of the AFP level and HBsAg in differentiation of the hepatic masses, we retrospectively analyzed AFP level and HBsAg' in 228 pathologically or radiologically confirmed hepatocellular carcinomas, and 137 pathologically nonhepatocellular cacinomas. The results were as follows In hepatocellular carcinoma, AFP level above 20ng/ml was 77.8% and HBsAg positve was 72.6%. In nonhepatocellular carcinoma, AFP level above 20ng/ml was 3.7% and HBsAg positve was 16.1%. We concluded that AFP level and HBsAg are helpful to distinguish hepatocellular carcinoma from nonhepatocellular carcinoma, when IV bolus computed tomogram finding is uncertain.

Livedo reticularis with summer ulceration was first reported by Feldaker et al in 1955, that is characterized by livedo reticularis and ulcerations of the legs or feet beginning primarily the summer or warmer months of the year, and healing the winter months. A 32-year old rnan has suffered from periodic painful ulcerations, which were occurred during the summer, on the both ankles and brown to bluish colored reticulated discoloration on the lower legs for about 10 years. Histopathological findings showed endothelial proliferation and partial occlusion of the dermal capillaries, fibrinoid material on the superficial blood vessels, and partial epidermal necrolysis. This patient. was treated with antiplatelet aggregating medications and antihypertensive drug.

From September 1983 to August 1984, the clinical observations regarding cutaneous manifestations of 26(j cases of in-patients in the Department of Internal medicine with hepatic problems were observed at the Department of Derrnatology in Chonnam University Eiospital, The results were as follows: In the 266 patients, 201 patients(75.6%) revealed various cutaneous manifestaiions; Among them vascular spider occured in 100 (37. 6%), bleeding tendency in 67(25. 2%), pruritus in 65(24.4%), jaundice in 59 (22.2%), palmar erythema in 59(22. 2%), and melanosis in 56(21. l%). The incidences of skin changes by pattern of liver disease were as follows; In acte hepatits, prurit;us(46.8%) and acute urticarial & maculapapular eruption(36. 4%) were found in order. In chronic hepatitis, vascular spider(42. 6%) and melanosis (27. 9%) in order. In cirrhosis, vascular spider (62. 5%) and bleeding tendency (42. 0%) in order. In hepatic cancer vascular spider (24. 2%) and jaundice (21. 2%) in order. There were no statistically significant differences in sex distribution. In the incidences of skin changes by age groups, jaundice(p<0. 01) and melanosis were more frequently observed in the patients under the age of 29 and vascular spider (p<0.01 ) and melanosis in the patients are the age of 50. The longer the duration of illness were, the higher the incidences of skin changes except jaundice were.

Magnetic resonance (MR) is rapidly emerging technique that provides high quality images and potentially provides much more diagnostic information than do conventional imaging modalities. MRI is conceptually quite different from currently used imaging methods. The complex nature of MRI allows a great deal of flexibility in image production and available information, and key points are as follows. 1. MR offers a non-invasive technique with which to generate in vivo human images without ionizing radiation and with no known adverse biological effects. 2. Imaging mechanism of MRI is quite different from conventional imaging modality and for more accurate diagnostic application, It is necessary for physician to understand imaging mechanism of MRI 3. M.R. makes available basic chemical parameters that may provide to be useful for diagnostic medical imaging and more specific pathophysiologic information which are not available by alternate techniques. 4. M.R. can be produced by number of different methods. This flexibility allows the imaging technique to be applicated for particular clinical purpose. Multiplanar and three dimensional imaging may extend the imaging process beyond the single section available with current CT. 5. Future directions include efforts to; a. Further development of hard ware b. More fastening scan time c. Respiratory and cardiac gated imaging d. Imaging of additional nuclei except hydrogen. e. Further development of contrast media f.
