1.Predictors of radiation pneumonitis and pulmonary function changes after concurrent chemoradiotherapy of non-small cell lung cancer.

PURPOSE: To evaluate the predictive factors of radiation pneumonitis (RP) and associated changes in pulmonary function after definitive concurrent chemoradiotherapy (CCRT) in patients with non-small cell lung cancer (NSCLC). MATERIALS AND METHODS: Medical records of 60 patients with NSCLC who received definitive CCRT were retrospectively reviewed. Dose volumetric (DV) parameters, clinical factors, and pulmonary function test (PFT) data were analyzed. RP was graded according to the CTCAE ver. 4.0. Percentage of lung volume that received a dose of threshold (Vdose) and mean lung dose (MLD) were analyzed for potential DV predictors. PFT changes were calculated as the difference between pre-RT and post-RT values at 3, 6, and 12 months after RT. RESULTS: Twenty-two patients (37%) developed grade > or =2 RP. Among clinical factors, tumor location in lower lobe was associated with RP. Among the DV parameters, only MLD >15 Gy was associated with grade > or =2 RP. There were statistically significant decreases in PFT at all points compared with pre-RT values in grade > or =2 RP group. MLD was associated with forced vital capacity (FVC) and forced expiratory volume in 1 second (FEV1) changes at 6 and 12 months. V10 was associated with FVC changes at 12 months. V20 and V30 were associated with FEV1 changes at 6 months and FVC changes at 12 months. CONCLUSION: After definitive CCRT in patients with NSCLC, MLD >15 Gy and lower lobe tumor location were predictors of grade > or =2 RP. Pulmonary functions were decreased after CCRT and the magnitude of changes was associated with DV parameters.

2.Traumatic Lipoma.

The occurrence of a lipoma-like condition associated with trauma would tend to be regarded as a subset of or a separate entity from ordinary lipoma. We report herein a case of traumatic Lpoma occuring on the dorsum of the left hand. The patient was a 46-year-old male, who had struck his left hand against a window about two months previously and noted casually an asymptomatic, firm, dermal lesion at the site of injury. On gross examination, the enucleated nodule was a smooth-surfaced, yellow fatty mass measuring 0.3 × 0.5 × 0.3cm which looked like a kernel of corn. Microscopic examination revealed encapsulated mature fat consistent with lipoma.

5.A Clinical Study on Childhood Systemic Lupus Erythematosus.

To Evaluate the clinical characteristics of childhood-onset systemic lupus erythemoatosus (SLE) and analyse the factors related to outcome of renal function in lupus nephritis, we reviewed medical records of 18 cases of SLF diagnosed at the Department of Pediatrics. Pusan National University Hospital from January 1981 to December 1990. The results were as follow: 1)Male to female ratio was 1:2.6. 2) Nephropathy was the most common initial impression, and 8 cases were diagnosed as SLE at first. 3) The clinical menifestation. was ANA (+), malar rash, renal disorder, hematologic disorder in frequency. 4) Diffuse proliferative lupus nephritis was the most common in renal histopathologic studies. 5) Among the follow-up cases, active lupus was 39%, remission was 28%. 6) As factors related to outcome of lupus nephrits were, hypertension and proteinuria were siginificant statistically.

8.Binding of B Cell - Derived Autocrine Growth Factor to Hemoglobin.

Normal human B cells produce autocrine growth factor in response to Staphylococcus aureus Cowan I strain (SAC). However, the functional role and molecular nature of the B cell derived-B cell growth factor (B-BCGF) are largely unknown. We have tried to investigate the nature of B-BCGF using mAb for several years. We report here that B- BCGF is capable of binding to hemoglobin (Hb). The concentrated culture supernatant from tonsillar B cells stimulated with SAC for 24 h was loaded into the fast protein liquid chromatography and ion-exchange chromatography. The peak with BCGF activity was shown to have a M.W. of 16-18 Kda in polyacrylamide gel electrophoresis followed by silver stain. Amino acid sequence of the fraction was found to identical to human hemoglobin (Hb) by more than 85%. However, Hb itself had no BCGF activity. The presence of Hb in culture supernatant was due to the contamination of SRBC during B cell purification. SRSC were completely removed from B cells by percoll-gradient centrifugation and B cells were stimulated with SAC and exogenous Hb was added to the cultures. The Hb fraction from FPLC again showed a BCGF activity. These data strongly suggested that BCGF binds to Hb. We confirmed this in dot blot as well as Western blot. The M.W of Hb-binding BCGF was 20 Kda. This information may provide a rapid progress in research of BCGF.

10.Clinical Observation of Skin Manifestation in the Patients with Hepatic Problems.

From September 1983 to August 1984, the clinical observations regarding cutaneous manifestations of 26(j cases of in-patients in the Department of Internal medicine with hepatic problems were observed at the Department of Derrnatology in Chonnam University Eiospital, The results were as follows: In the 266 patients, 201 patients(75.6%) revealed various cutaneous manifestaiions; Among them vascular spider occured in 100 (37. 6%), bleeding tendency in 67(25. 2%), pruritus in 65(24.4%), jaundice in 59 (22.2%), palmar erythema in 59(22. 2%), and melanosis in 56(21. l%). The incidences of skin changes by pattern of liver disease were as follows; In acte hepatits, prurit;us(46.8%) and acute urticarial & maculapapular eruption(36. 4%) were found in order. In chronic hepatitis, vascular spider(42. 6%) and melanosis (27. 9%) in order. In cirrhosis, vascular spider (62. 5%) and bleeding tendency (42. 0%) in order. In hepatic cancer vascular spider (24. 2%) and jaundice (21. 2%) in order. There were no statistically significant differences in sex distribution. In the incidences of skin changes by age groups, jaundice(p<0. 01) and melanosis were more frequently observed in the patients under the age of 29 and vascular spider (p<0.01 ) and melanosis in the patients are the age of 50. The longer the duration of illness were, the higher the incidences of skin changes except jaundice were.
