2.Ultrastructural localization of 28 kDa glutathione S-transferase in adult Clonorchis sinensis.

Glutathione S-transferase (28GST) with molecular mass of 28 kDa is an antioxidant enzyme abundant in Clonorchis sinensis. In adult C. sinensis, 28GST was localized in tegumental syncytium, cytons, parenchyma, and sperm tails examined by immunoelectron microscopy. C. sinensis 28GST was earlier found to neutralize bioreactive compounds and to be rich in eggs. Accordingly, it is suggested that 28GST plays important roles in phase II defense system and physiological roles in worm fecundity of C. sinensis.

3.Treatment of Diskogenic Low Back Pain Using Epidural Steroid

Today, the exact causal relationship between prolapsed intervertebral disk and sciatica remains unclear. Murphy has proposed that inflammation rather than mechanical pressure is the basis of back pain and sciatica, and that causal irritant is likely to be a chemical product of disk degeneration. The administration of steroids into the epidural space reduces the inflammatory process of the neural structure is well known. We have studied the effect of injection of methylprednisolone acetate (Depomedrol) comparing with the effect of operative treatment. One hundred and eleven cases suffuring from diskogenic low back pain had been treated at Wallace Memorial Baptist Hospital during 2.5 years period from May 1979 to Nov. 1981. The results were as follows: 1. Sixteen patients (14%) stated that they had no relief from the injection, 24(22%) were moderately improved, 52(47%) were markedly improved and 19(17%) were completely relieved of symptoms. 2. The candidates for laminectomy which were confirmed by myelogram had complete improvement in 12%, marked improvement in 39%, moderate improvement in 21%, no relief in 28%.

4.Bone Density and Mechanical Property after Locked Intramedullary Nailing of the Long Bone Fractures: Preliminary Study of QCT & Lunar of 15 Long Bone Fractures.

The decrease of bone density after fracture is a well-known phenomenon. Several studies of the changes of bone density after fracture demonstrated bone density decreased not only at the fracture site, b ut also along the whole fractured bone. Being treated with locked intramedullary (IM) nailing, 15 patients with the long bone fractures of lower limbs were examined by Quantitative Computed Tomography (QCT) or Dual-Energy X-ray Bone Densitometer (Lunar DPX-L) a few days after nail removal. In the cases of QCT (5 patients of the tibia shaft fracture), cortical bone density, cortical thickness and geometrical shape (i.e. external antero-posterior and medio-lateral diameters) of the fracture site were compared to those of the contralateral side. On the other hand, in the case of Lunar' (5 patients of the tibia shaft fracture & 5 patients of the femur shaft fracture), only cortical bone density was compared to that of the contralateral side. The results of this study were as follows: 1. When treated by the locked intramedullary nailing of the long bone fractures, the bone density of the tibia & femur shaft fractures measured from the fracture site, proximal and distal sites to mid-fracture decreased. 2. There was a difference in the bone density of the uninjured long bone diaphysis of the tibia & femur shaft fractures. That is, in the case of the tibia shaft fracture, the bone density of the ipsilateral femur diaphysis measured by QCT increased 2 %, and increased 7 % when measured by Lunar. But, in the case of the femur shaft fracture, the bone density of the ipsilateral tibia diaphysis measured by Lunar decreased 4%. 3. In the case of the tibia shaft fracture, the cortical thickness in the fracture site measured by QCT increased 72 %, and external antero-posterior and medio-lateral diameters increased 7 % and 63 %. It seemed to be the formation of peripheral callus due to medullary reaming preceding intramedullary nail insertion. Therefore, this clinical study demonstrated that the mechanical property of the long bone fractures was restored and restricted weight bearing hardly needs to recommended when the nail was removed after the fracture union. 4. The bone densities of the tibia shaft fratures measured by QCT were not different with those measured by Lunar.

5.Influence of Type of Screw on the Bacterial Adherence and Infection.

The purpose of the study was to evaluate the effect of commonly used orthopaedic material, stainless steel (SS) screw (2.7xlOmm, Synthes) and titanium (Ti) screw (2.7x10mm, Synthes), on bacterial adherence and infection. The bacteria used for this study was Staphylococcus epidermidis for its ability of adherence. For in vitro assay, screws of each material were placed into test tube containing 107 colony forming unit (CFU) of S. epidermidis in trypticase soy broth for 18 hours. Following incubation, each screws were treated either with trypsin or with an ultrasonic cleaner. For in vivo assay, we inoculated 5x107 CFU of S. epidermidis into the proximal tibial metaphysis of the rabbits, and then inserted Ti screw on the right side and SS screw on the left side. The amount of in vitro adherence of S. epidermidis was greater for SS screw than for Ti screw. Especially following trypsin treatment, SS groups yielded significantly higher bacterial counts than Ti group, respectively (1.4x105 CFU vs 2.8x104 CFU, p<0.05). We found a significantly higher bacterial count after use of SS screw than after Ti screw in rabbits in vivo assay also. Trypsin treatment for identifying biofilm embedded bacteria from implanted metal may be useful in the diagnosis of infection. Ti screw appears to have biocompatibility to inhibit bacterial adherence compared with SS screw in the rabbit tibial model.

6.Intraarticular Arthroscopic Finding in Grade III MCL Rupture

The medial collateral ligament is the most commonly injured ligamentous structure of the knee. The management of the medial collateral ligament injuries were divided into surgical and conservative treatment. More recently, many investigators reported that isolated medial collateral ligament injuries did well under non-operative treatment. So, arthroscopic examination of the knee to rule out other intraarticular pathologic conditions such as cruciate ligament injuries, meniscal tear and osteochondral lesion is essential. From October 1993 to April 1995, we treated 45 cases of Grade III medial collateral ligament injuries and arthroscopic finding for cruciate ligament, meniscus, bony structure were analized in these cases. The result were as follows; 1. Anterior cruciate ligament injury:37.5% in +1 degree, 42.8% in +2 degree, 86.7% in +3 degree 2. Posterior cruciate ligament injury:12.5% in +1 degree, 13.4% in +2 degree, 33.3% in +3 degree 3. Meniscal injury:12.5% in +1 degree, 21.3% in +2 degree, 33.4% in +3 degree 4. Medial capsular ligament injury was more frequent and severe in the meniscofemoral ligament than in the meniscotibial ligament, but that finding was not correlated with degree of medial instability.

7.Comparison of Conservative and Operative Treatment of Achilles Tendon Rupture

The treatment of Achilles tendon rupture is controversial between surgical repair and conservative cast treatment. Until the 1970s, treatment of Achilles tendon rupture was predominantly surgical repair, but Lea and Smith reported better results in conservative treatment of Achilles tendon rupture and other authors confirmed their good results with cast treatment. This study was attempted to compare the subjective and objective results between operative and conservative treatments of patients with ruptured Achilles tendon. In a retrospective study, 30 Patients with ruptured Achilles tendon were analyzed from January 1990 to May 1994. 15 patients were surgically repaired(group I), and 15 patients were treated with gravity equines short leg walking cast(group II), Patient's subject satisfaction, duration from trauma to normal activity level, plantar flexion power, tendinous healing state checked by ultrasonography, and complications were compared, and following results were obtained. 1. All patients satisfied with conservative treatment in group II, while 12 patients were satisfied with operative treatment in group I. 2. The plantar flexion power of injured side was 89% in group I, 87% in group II comparing with uninjured side. 3. Ultrasonographically, mean thickness and ruptured midportion thickness in group II was thicker than those of group I(8.7, 10.6 vs 9.2, 13.2mm), but focal sonolucent area which indicates incomplete healing was observed more in group II than in group I(4 vs 2). 4. Three patients had complications in group I-1rerupture, 1 infection and 1 sural nerve injury.

8.The Distribution of I/D Polymorphism in the ACE Gene between Korean Young Controls and Athletes.

A number of genetic and environmental factors influence athletic performance. Cardiovascular fitness is an important factor of athletic success, and ACE gene is a good candidate for regulating cardiac and vascular function. Because younger subjects have less chance of being exposed to environmental factors than older ones, genetic factors have a relatively greater influence on younger subjects. The aim of this study was to investigate the distribution of I/D polymorphism in the ACE gene between Korean young controls and athletes. By association study, there were no significant differences in genotype and allele distributions between two groups, respectively (P> 0.05). When stratified by sporting disciplines, the significant difference in distribution was not also detected in our study (P> 0.05). These results do not support the hypothesis that the I/D polymorphism in the ACE gene is associated with endurance performance in Korean young subjects.

9.One Case of Sparganosis Infestation of the Thigh and Popliteal Area Combined with Avascular Necrosis of the Femoral Head.

Human sparganosis is one of the fhost common tissue parasitoses caused by helminthes in Korea. Human sparganosis is found most frequently in subcutaneous or adipose tissue of the abdominal wall, thoracic wall and inguinal region, it is rarely found in the orbital cavity, abdominal cavity, brain, breast, or lower extremities. We experienced one ease of sparganosis which developed into the thigh and popliteal area in combination with avascular necrosis of the femoral head. The case is a 59 years old male. The patient had the history of eating raw meat as a tonic food 3 years ago. The presenting symptom was left hip joint pain. We confirmed the sparganosis in microscopic findings of worms and by immunologic serum studies. The avascular necrosis of the femoral head was detected with plain roentgenography and magnetic resonance imaging. Many spargana were removed from the lesions by surgical excision. Two of them were alive, and others had degenerated or calcified. The life cycle and route of infestation of sparganosis were discussed briefly and related literatures were reviewed.
